Bakgrund:  År 2024 vårdades cirka 12.000 personer i Sverige enligt lagen om psykiatrisk tvångsvård. Patienter som vårdats mot sin egen vilja har en förhöjd suicidrisk som kan kvarstanna i flera år efter vårdtillfället. Redan på 1800 talet när psykiatrivården började utvecklas fanns en oro för välbefinnande hos personer med psykisk sjukdom och i januari 1992 togs nuvarande lagstiftning i bruk i Sverige. Specialistsjuksköterskans roll är att värna om patienten och dennes rättigheter samtidigt som patienten behöver vårdas. Tvångsåtgärder inom psykiatrisk vård är en komplex fråga då det krockar med patientens autonomi och integritet vilket skapar ett etiskt dilemma för specialistsjuksköterskan. 

Syfte: Syftet med denna litteraturstudie är att sammanställa specialistsjuksköterskans upplevelser av psykiatrisk tvångsvård.

Metod: En strukturerad litteraturstudie gjordes med artiklar från databaserna Cinahl, Pubmed och Scopus där totalt 21 vetenskapliga artiklar ligger till grund för resultatet. Artiklarna analyserades enligt Thomas och Hardens analysprocess. 

Resultat: Litteraturstudien utmynnade i fyra huvudtema och två undertema. Huvudteman var: Allians, Säkerhet, Kontroll och Det professionella perspektivet. Underteman var: Makt och Struktur under tvångsåtgärd. 

Slutsats: Sjuksköterskor inom psykiatrisk omvårdnad upplever en moralisk stress kring genomförandet av tvångsåtgärder. Sjuksköterskorna har en etisk aspekt och en yrkesmässigaspekt för att skydda patienten mot skador men även att genomföra tvångsåtgärder så skonsamt som möjligt men utan att kränka patientens självbestämmande och autonomi. 

Background: In 2024, approximately 12,000 people in Sweden were treated under the Compulsory Psychiatric Care Act. Patients treated against their will have an increased risk of suicide that can persist for several years after the period of care. Concern for the well-being of people with mental illness existed as early as the 19th century when psychiatric care began to develop, and in January 1992, the current Swedish legislation was enacted. The role of the specialist nurse is to safeguard the patient and their rights while simultaneously providing necessary care. Restrictive practices in psychiatric care are complex issues, as they conflict with patient autonomy and integrity, creating an ethical dilemma for the specialist nurse.

Aim: The aim of this literature study is to compile specialist nurses' experiences of compulsory psychiatric care. 

Method: A structured literature study was conducted using articles from the databases CINAHL, PubMed, and Scopus, with a total of 21 scientific articles forming the basis of the results. The articles were analyzed according to Thomas and Harden's analysis process.

Results: The literature study resulted in four main theme and two subtheme. The main themes were: Alliance, Safety, Control, and The Professional Perspective. The subthemes were: Power and Structure during Restrictive Practices.

Conclusion: Nurses in psychiatric care experience moral distress regarding the implementation of restrictive practices. The nurses maintain both an ethical and a professional perspective, aiming to protect the patient from harm and implement restrictive measures as gently as possible, without violating the patient's self-determination and autonomy.
