Could interventions on metabolic risk factors reduce disparities in stroke or death across BMI groups?: A causal mediation analysis using health register data

Abstract [en]

Background. Several risk factors for stroke are well established, including overweight and obesityas well as metabolic conditions such as high cholesterol, high blood pressure, and diabetes.However, because these risk factors are closely related, it is not fully understood how much of the increased risk among overweight and obese individuals could potentially be reduced through interventions on these metabolic risk factors.

Method. This thesis uses causal mediation analysis to estimate how much the difference in risk of first-time stroke or death between individuals with overweight or obesity and normal weight individuals could potentially be reduced through hypothetical interventions on high cholesterol, high blood pressure, and diabetes. The analysis is based on data from health examinations within the Northern Sweden Health and Disease Study (NSHDS), combined with data from the Swedish Stroke Register (Riksstroke) and the National Patient Register. The analysis focuses on stroke or death occurring approximately between 60 and 70 years of age.

Results. After adjustment for age, sex, education level, examination period, and smoking, the estimated risk of stroke or death was 7.63% among overweight and obese individuals and 6.72% among normal weight individuals. A hypothetical intervention shifting the joint distribution of cholesterol, blood pressure, and diabetes among overweight and obese individuals toward the joint distribution observed among normal weight individuals was estimated to reduce the risk difference by 0.43 percentage points (95% CI: (0.26, 0.60)). Separate interventions on blood pressure and diabetes were estimated to decrease the risk difference by 0.19 (95% CI: (0.06, 0.32)) and 0.25 (95% CI: (0.13, 0.36)) percentage points, respectively. The estimated effect for cholesterol was close to zero and not statistically significant, suggesting little evidence of a mediating role in this analysis.

Conclusions. The results suggest that high blood pressure and diabetes may be important targets for preventive interventions aimed at reducing differences in risk of stroke or death between BMI groups.

Bakgrund. Flera riskfaktorer för stroke är väl etablerade, bland annat övervikt och obesitas samtmetabola tillstånd som högt kolesterol, högt blodtryck och diabetes. Dessa riskfaktorer är nära sammankopplade, och det är inte fullt klarlagt hur mycket av den ökade risken bland individer med övervikt eller obesitas som potentiellt skulle kunna reduceras genom interventioner på dessa metabola riskfaktorer.

Metod. Den här uppsatsen använder kausal mediationsanalys för att uppskatta hur mycket av skillnaden i risk för första stroke eller död mellan individer med övervikt eller obesitas och normalviktiga individer som potentiellt skulle kunna reduceras genom hypotetiska interventioner på högt kolesterol, högt blodtryck och diabetes. Analysen är baserad på data från hälsoundersökningar inom Northern Sweden Health and Disease Study (NSHDS), kombinerat med data från svenska strokeregistret (Riksstroke) och Nationella patientregistret. Separata interventioner på blodtryck och diabetes skattades minska riskskillnaden med 0.19 (95% KI: (0.06, 0.32)) respektive 0.25 (95% KI:(0.13, 0.36)) procentenheter. Den skattade effekten för kolesterol var nära noll och inte statistiskt signifikant, vilket inte gav stöd för någon tydlig medierande roll för kolesterol i denna analys.

Slutsats. Resultaten tyder på att högt blodtryck och diabetes kan vara viktiga mål för preventiva interventioner som syftar till att minska skillnader i risk för stroke eller död mellan BMI-grupper.
