Contralesional Grey Matter Volume as a Predictor of Postoperative Cognitive Decline in Glioma

Abstract [en]
Gliomas are the most prevalent malignant brain tumours in adults and are associated withsubstantial cognitive decline. The contralesional hemisphere undergoes grey matterreorganisation associated with cognitive function, but whether contralesional grey matter volume(GMV) at baseline predicts the postoperative cognitive trajectory remains largely unexamined.The present study tested whether greater contralesional GMV at baseline predicts slowerpostoperative cognitive decline in adult glioma patients, controlling for tumour grade andhemisphere. Thirteen adults with WHO Grade III or IV astrocytoma (including IDH-wildtypeglioblastoma) with pre- and at least one postoperative assessment formed the longitudinal sample(cross-sectional preoperative N = 23). Contralesional GMV was extracted via FreeSurfer andcorrected for estimated total intracranial volume. Three z-scored composites (Memory, ExecutiveFunction, Global Cognition) were tested in a linear mixed effects model with a Time × GMVinteraction. The pre-specified primary outcome, the Global Cognition Time × GMV interaction,was nominally significant (β = 0.043, 95% CI [0.002, 0.084], p = .043), as were the secondarycomposites (Memory β = 0.065, p = .048; Executive Function β = 0.044, p = .050). Effects weredirectionally consistent across six confound-controlled specifications (age, sex, tumour location,IDH status), but none survived Benjamini–Hochberg FDR correction (q = .05) and all threeweakened substantially once the follow-up window was extended beyond 800 days. Aftercorrection for multiple comparisons none of these associations was statistically reliable, so theresults do not provide confirmatory evidence for a contralesional reserve effect. The consistentdirection across all three composites, together with the limited statistical power of the sample, isbest read as a tentative indication that a study with a larger cohort is warranted rather than as anestablished effect.

Gliom är de vanligaste maligna primärtumörerna i hjärnan hos vuxna och förknippas medomfattande kognitiv försämring. Den kontralesionella hemisfären genomgår omstrukturering avgrå substans i samband med kognitiv funktion, men huruvida kontralesionell grå substansvolym(GMV) vid baslinjen förutsäger den postoperativa kognitiva trajektoren är till stor del outforskat.Föreliggande studie testade om en större kontralesionell GMV vid baslinjen förutsägerlångsammare postoperativ kognitiv försämring hos vuxna gliompatienter, med kontroll förtumörgrad och hemisfär. Tretton vuxna med WHO grad III eller IV astrocytom (inklusiveIDH-vildtyp glioblastom) med en preoperativ och minst en postoperativ bedömning utgjorde detlongitudinella urvalet (tvärsnittligt preoperativt N = 23). Kontralesionell GMV extraherades viaFreeSurfer och korrigerades för uppskattad total intrakraniell volym. Tre z-poängsattakompositer (Minne, Exekutiv Funktion, Global Kognition) testades i en linjär mixadeffekt-modell med en Tid × GMV-interaktion.
