Kvinnosjukvårdens roll i arbetet mot våld: En kvalitativ studie om chefers uppfattningar om arbetet kring våld i nära relation inom kvinnosjukvården

Abstract [sv]

Bakgrund: Våld i nära relationer är ett folkhälsoproblem som påverkar hälsa och välbefinnande och medför fysiska, psykiska, sociala och ekonomiska konsekvenser för individen. Identifiering, hantering och stödjande insatser är en fråga om samhällsresurser. Hälso- och sjukvården ska tillhandahålla vård för god hälsa och förebygga ohälsa. Verksamheter inom kvinnosjukvården ska arbeta med riktlinjer och rutiner relaterat till arbetet mot våld i nära relationer.  

Syfte: Syftet med studien var att undersöka hur chefer för verksamheter inom kvinnosjukvården uppfattar på vilket sätt arbetet med våld i nära relationer är organiserat och genomförs.

Metod: I studien har en kvalitativ ansats använts med målstyrt kriteriestyrt urval, samt snöbollsurval. Datainsamlingen omfattade åtta semi-strukturerade intervjuer med chefer och samordnande professioner inom verksamheter för kvinnosjukvård inom sex olika regioner i Mellansverige. Analysmetoden har varit manifest innehållsanalys.  

Resultat: Resultatet visade att verksamheterna finner stöd av rutinmässiga dokument för att identifiera och hantera våldsutsatta. Dokumentation och kvalitetssäkring görs genom avvägningar av skyddade journaler, dokumentation och avvikelsehantering. För att upprätthålla arbetet krävs förutsättningar i form av kunskap, kontinuerliga utbildningsinsatser, samt organisatoriska förutsättningar gällande bemanning, arbetsbelastning och tidsresurser. Dessa faktorer bidrar till personalens engagemang och trygghet. Samtidigt lyfts vikten av fungerande samverkan mellan aktörer för att arbetet ska kunna koordineras.  

Slutsats: Kvinnosjukvården är en aktör kring arbetet med våld i nära relationer. Faktorer påverkar hur arbetet genomförs och organiseras. Insatser krävs på olika nivåer utifrån den socio-ekologiska modellen där nivåerna behöver samspela med varandra för att det hälsofrämjande och förebyggande arbetet ska ha effekt för hälsa och välmående.  

Abstract [en]

Background: Domestic violence is a public health issue that affects health and well-being and entails physical, psychological, social and economic consequences for individuals. Identification, management and supportive interventions are matters of societal resources. The health care system has a responsibility to provide care for good health and prevent further illness. Women’s healthcare has a responsibility to work according to guidelines and routines related to work against domestic violence. 

Aim: The aim of the study was to investigate how managers of women’s healthcare perceive the way in which work with domestic violence is organized and implemented. 

Method: The study used a qualitative approach with goal-directed, criterion-driven selection and snowball sampling. Data collection included eight semi-structured interviews with managers and coordinating professionals within women’s healthcare services in six different regions in central Sweden. The analysis method was manifest content analysis.

Results: The results showed that the services are supported by routine documents to identify and manage individuals exposed to violence. Documentation and quality assurance are addressed through considerations regarding protected medical records, documentation and incident management. To maintain routine work requires conditions such as knowledge, continuous training and organizational resourses related to staffing, workload and time resources. These factors contribute to staff engagement and security. At the same time, the importance of functioning collaboration between actors so that the work can be coordinated.

Conclusion: Women’s healthcare is an actor in the work with domestic violence. Factors affect how the work is carried out and organized. Interventions are required at different levels based on the social-ecological model where the levels need to interact with each other in order for the health promotion and prevention work to be effective for the health and well-being.
