Samband mellan socioekonomi och kontinuitet: en tvärsnittsstudie i svensk primärvård

Sammanfattning

Bakgrund

Skillnader i hälsa, relaterade till socioekonomiska förutsättningar är väl dokumenterade. Kontinuitet i primärvården är associerad med bättre hälsoutfall, men kunskapen är begränsad om hur patientupplevd kontinuitet varierar mellan vårdcentraler med olika socioekonomisk sammansättning.

Syfte

Syftet med studien var att undersöka sambandet mellan den socioekonomiska sammansättningen hos patienter listade vid vårdcentraler och patientupplevd kontinuitet.

Metod

Studien hade en ekologisk tvärsnittsdesign på vårdcentralsnivå baserad på data från Nationell patientenkät och registerdata från 1 145 vårdcentraler. Socioekonomisk status mättes med medianinkomst, arbetslöshet och andel listade patienter med eftergymnasial utbildning. Patientupplevd kontinuitet analyserades med sju frågor om informationskontinuitet, relationell kontinuitet samt samordningskontinuitet. Sambanden analyserades med viktade linjära regressionsmodeller, ojusterade och justerade för ålder, kön och strukturella kovariater för vårdcentralerna.

Resultat

Efter justeringar och efter korrigering för multipla tester, kvarstod samband där vårdcentraler med en högre andel patienter med eftergymnasial utbildning hade högre patientupplevd kontinuitet i flera utfall: samstämmig kommunikation (β = 0,048; 95 % KI 0,024–0,072), samarbete (β = 0,023; 95 % KI 0,004–0,041), beslut om nästa steg i vården (β = 0,037; 95 % KI 0,015–0,060) samt möjligheten att träffa önskad läkare (β = 0,012; 95 % KI 0,003–0,021). Inga samband för medianinkomst eller arbetslöshet kvarstod efter korrigering för multipla tester.

Slutsats

Den socioekonomiska sammansättningen, avseende utbildningsnivå, hos patienter vid vårdcentraler var i viss utsträckning associerad med patientupplevd kontinuitet. Sambanden var svaga men återkommande, och varierade mellan olika kontinuitetsdimensioner, vilket motiverar vidare forskning.

Abstract

Background

Socioeconomic inequalities in health are well documented. Continuity of primary care is associated with better health outcomes, but little is known about how patient-perceived continuity varies across primary health care centers with different socioeconomic compositions.

Aim

This study aimed to examine the association between socioeconomic composition of patients listed at primary health care centers and patient-perceived continuity of care.

Methods

This ecological cross-sectional study used National Patient Survey and registry data from 1145 primary health care centers. Socioeconomic position was measured by income, unemployment rate, and post-secondary education among listed patients. Patient-perceived continuity was measured using seven questions covering relational, managerial and information-continuity. Associations were analysed using weighted linear regression, unadjusted and adjusted for age, sex and structural covariates. 

Results

After adjusting and correction for multiple testing, associations persisted between primary health care centers with a higher proportion of listed patients with post-secondary education and perceived continuity in consistent communication between care-providers (β = 0,048; 95 % CI 0,024–0,072), collaboration between care-providers (β = 0,023; 95 % CI 0,004–0,041), decisions regarding the next step in care (β = 0,037; 95 % KI 0,015–0,060) and access to preferred doctor (β = 0,012; 95 % CI 0,003–0,021). No associations persisted for median income or unemployment. 

Conclusion

Educational level of patients of primary health care centers was to some extent associated with patient-perceived continuity of care. The associations were weak but consistent and varied across dimensions of continuity, suggesting a need for further research.
