Abstract [sv]

Stress och sömnbesvär är vanliga folkhälsoproblem som medför ökad psykisk ohälsa och stor belastning på primärvården. Detta skapar ett behov av korta resurseffektiva behandlingsmetoder. Studien syftar till att utvärdera primärvårdspatienters upplevelse av allians och kvalitet i två KBT-baserade gruppinsatser mot stress- och sömnproblem. Deltagarna var vuxna (17–83 år; majoritet kvinnor) patienter från primärvården. Studien hade en explorativ mixed-methods-design där kvalitativa innehållsanalyser av 64 fritextutvärderingar (n=52 i stresskolan, n=12 i sömnskolan) samt icke-parametrisk variansanalys med Friedmans test genomfördes av patienternas allians-skattningar av Session Rating Scale av 164 deltagare (n=114 i stresskolan, n=50 i sömnskolan). Analyserna av fritextsvaren visade att deltagarna upplevde stresskolan som meningsfull, där psykoedukation, normalisering, konkreta strategier och gruppformatet lyftes fram som värdefulla, medan det vanligaste förbättringsförslaget var fler grupptillfällen. Även deltagarna i sömnskolan upplevde innehållet som meningsfullt, där psykoedukation, normalisering, konkreta strategier och en liten gruppstorlek lyftes som värdefulla aspekter. Förbättringsförslagen var få men ett par deltagare nämnde mer tid för fördjupning som förslag. Allians skattades generellt högt för stresskolan, och Friedmans test visade statistiskt signifikanta förändringar över tid för samtliga dimensioner: relation, mål och ämne, arbetssätt/metod samt helhetsintryck. Även för sömnskolan skattades allians generellt högt, och för arbetssätt/metod och helhetsintryck ökade alliansen signifikant över tid, medan inga statistiskt signifikanta skillnader mellan de olika tillfällen framkom för relation, samt för mål och ämne. Sammanfattningsvis framstår interventionerna som relevanta och meningsfulla, men resultaten indikerar att ett ökat antal träffar kan vara centralt för att ytterligare stärka behandlingsupplevelse och klinisk nytta, särskilt i stresskolan.

Abstract [en]

Stress and sleep difficulties are common public health problems associated with increased mental ill-health and a substantial burden on primary care services. This creates a need for brief and resource-efficient treatment methods. The aim of this study was to evaluate primary care patients’ experiences of therapeutic alliance and quality in two CBT-based group interventions targeting stress and sleep problems. Participants were adult primary care patients (17–83 years old; majority women). The study employed an exploratory mixed-methods design, including qualitative content analyses of 64 free-text evaluations (n=52 in the stress management group, n=12 in the sleep school) and non-parametric variance analysis using Friedman’s test on participants’ alliance ratings from the Session Rating Scale among 164 participants (n=114 in the stress management group, n=50 in the sleep school). The analyses of the free-text responses showed that participants experienced the stress management group as meaningful, highlighting psychoeducation, normalization, concrete strategies, and the group format as valuable aspects, while the most common suggestion for improvement was to increase the number of group sessions. Participants in the sleep school also experienced the content as meaningful, emphasizing psychoeducation, normalization, concrete strategies, and the small group size as valuable features. Suggestions for improvement were few, although a couple of participants expressed a wish for more time to explore the topics in greater depth. Therapeutic alliance ratings were generally high for the stress management group, and Friedman’s test showed statistically significant changes over time for all subscales: relationship, goals and topics, approach/method, and overall impression. Alliance ratings were also generally high for the sleep school, and for approach/method and overall impression, alliance increased significantly Begränsad delning over time, whereas no statistically significant differences across sessions emerged for relationship or for goals and topics. In summary, the interventions appear relevant and meaningful, but the results indicate that increasing the number of sessions may be important for further strengthening the treatment experience and clinical usefulness, especially in the stress management group.
