    Abstract [sv]

    Bakgrund: Den palliativa vården är en helhetsvård som sjuksköterskan är högst involveradi. Sjuksköterskans roll innebär att vårda och vara närvarande, vilket kan generera storakänslomässiga utmaningar.Syfte: Att belysa sjuksköterskors upplevelser av att ge omvårdnad till patienter sombefinner sig i ett palliativt skede.Metod: Denna studie var utformad som en litteraturöversikt baserad på 16 originalartiklarhämtade från Cinahl och PubMed. Artiklarna lästes och huvudresultaten struktureradesunder rubriker som baserades på likheter.Resultat: I resultatet framkom att sjuksköterskor både har stärkande och utmanandeupplevelser av att vårda palliativa patienter. De upplevelser som var stärkande och gavkraft var känslan av meningsfullhet, skapande av nära relation till patient och närstående,att få bekräftelse och uppskattning. De som innebar utmaningar Att inte få vara delaktig ibeslut gällande patientens vård, att möta frustration och ilska, att vara närvarande och mötaexistentiella frågor och bristande resurser.Slutsats: Arbetet inom den palliativa vården genererar utveckling och en känsla avmeningsfullhet, men det innebär även stora utmaningar. Att som sjuksköterska arbeta medpalliativ vård är en utmaning som med de rätta förutsättningarna kan bidra till positivaerfarenheter. Känsla av sammanhang, KASAM, kan ge en förståelse för hur sjuksköterskansgrad av hälsa kan bibehållas.

    Bakgrund: Palliativ vård kan förklaras som lindrande vård och ges när kurativ behandling inte längre har någon effekt. Behandlingen inriktar sig på att ge livskvalité och smärtlindring. Syfte: Syftet är undersöka de komplementära metoderna musik och beröring som smärtlindrande effekt för personer i palliativ vård. Metod: Litteraturöversiktens sökningar utfördes via databaserna Pubmed och Cinahl. Artiklarna var av kvantitativ, kvalitativ samt mixad metod och kvalitetsgranskades genom granskningsmallar. Resultat: I de flesta av studierna upplevde deltagarna att de fick en smärtlindrande effekt efter deltagande i musikterapi eller beröring/massage. Diskussion: Att arbeta med komplementära metoder är ett sätt för sjuksköterskan att arbeta med smärtans olika delar. För att uppnå detta krävs det att sjuksköterskan ser helheten och hela människan. Konklusion: Musikterapi och beröring är ett sätt för sjuksköterskan att möta de utmaningar sjuksköterskan ställs inför i vården av en palliativ patient. Genom musik och beröring kan sjuksköterskan nå smärtan utifrån olika dimensioner.Mer forskning krävs för att öka det vetenskapliga beviset av den smärtlindrande effekten och göra komplementär medicin till en del i den palliativa vården. Brist på utbildning, engagemang, tid och förståelse har visat sig vara de bristande länkarna för att de komplementära metoderna används i vården.

    Abstract [en]

    In this article, the authors set out to study the time use of men and women in Sweden, comparing self-employed and employed individuals. Previous studies indicate that there are reasons to believe that both gendered time use and mechanisms related to time use might differ between the self-employed and employees. Employing time use data, the aim was to study whether there are differences in gendered time use between self-employed individuals and employees in Sweden, and furthermore, which mechanism relates to gendered time use among self-employed individuals and employees. The results show that self-employed men and women distribute their time in a more gender-traditional manner than employees. In addition, relative resources are found to be an important factor related to gendered time use among the self-employed. For employees, gender relations tend to be a mechanism related to gendered time use. The conclusion is that working conditions are important for gendered time use and should be considered in future studies.

    Abstract [en]

    In this study, 11Swedish occupational safety and health (OSH)i nspectors were interviewed about their views of and experiences interacting with micro-enterprises (1-9 employees). The qualitative content analysis found one  theme, “Falling outside thes ystem”, and three subthemes, “The inspector—shaped by specific standards”, “The bureaucrat and the micro-entrepreneur—two separate worlds”, and “System faults and system changes”. According to the inspectors, the Swedish OSH regulatory system, with inspectors on the frontline, neglects the specific needs, circumstance and characteristics of micro-enterprises.Therefore,we suggest revising the OSH regulatory system and following inspection methods and enforcement styles to better address the needs of micro enterprises.

    Abstract [en]

    The aim of this article is to elucidate how male and female managers of small-scale enterprises in Norway and Sweden relate to and experience the intersection between work and private life. A qualitative content analysis was adopted to explore interviews with 18 managers. The analysis resulted in three primary categories: conflict as a part of the deal, using management to construct balance, and management identity contributing to enrichment. A key theme that emerged was doing management. Both men and women reproduced masculine values in describing their management identities and in explaining how they enacted management. This clear identification was used to legitimate conflict, construct balance and explain the interaction between work and private life as enriching. How the managers enacted gender emerged primarily in how they related to family responsibilities and their feelings of guilt in relation to home and children.

    Abstract [en]

    Purpose: Nurses working in forensic psychiatry often encounter offenders who have a severe mental illness, which may cause ethical challenges and influence nurses’ daily work. This study was conducted to illuminate the meaning of nurses’ lived experiences of encounters with patients with mental illnesses in forensic inpatient care. Methods: This qualitative study employed narrative interviews with 13 nurses. Interviews were audiotaped and transcribed verbatim and analysed following a phenomenological-hermeneutic approach. Results: Four key themes were revealed: “Being frustrated” (subthemes included “Fighting resignation” and “Being disappointed”), “Protecting oneself” (subthemes included “To shy away,” “Being on your guard,” and “Being disclosed”), “Being open-minded” (subthemes included “Being confirmed,” “Developing trust,” and “Developing compassion”), and “Striving for control” (subthemes included “Sensing mutual vulnerability” and “Regulating oneself”). Further, working in forensic psychiatry challenged nurses’ identity as healthcare professionals because of being in a stressful context. Conclusions: Dealing with aggressive patients with severe mental illnesses threatens nurses’ professional identity. Nurses must attempt to empathize with patients’ experiences and respond accordingly. Utilizing strategies rooted in compassion such as self-reflection, emotional regulation, and distancing themselves when necessary may enable nurses to more effectively respond to patients’ needs. © 2019, © 2019 The Author(s).     Abstract [sv]

    Bakgrund Forskning visar att personer med psykisk ohälsa riskerar att bemötas med negativa attityder från allmänhet och vårdpersonal vilket kan bidra till dessa personers isolering, minskad livskvalitet och ojämlik vård. Sjuksköterskor har ett professionellt ansvar att motverka denna stigmatisering. Syfte Var att belysa sjuksköterskors attityder till personer med psykisk ohälsa i hälso- och sjukvård. Metod En integrativ litteraturöversikt av kvantitativ design med deduktiv ansats. Resultat Sjuksköterskor hade både positiva och negativa attityder till personer med psykisk ohälsa av varierande karaktär; välvilliga, samhällsideologiska, auktoritära, stigmatiserande; vilka påverkades av sociodemografiska faktorer; ålder, kön, vårdkontext, position, utbildning, yrkeserfarenhet, personliga förbindelser till psykisk ohälsa. Diskussion De sociodemografiska faktorerna från resultatet som verifierades av tidigare forskning var personliga förbindelser till personer med psykisk ohälsa samt utbildning. Sjuksköterskor med positiva attityder visade en humanistisk människosyn och ett holistiskt förhållningssätt, vilket professionell omvårdnad bör grunda sig på. Vänlighet, förståelse, respekt och återhämtningsfokus utifrån personens berättelse är vad personcentrerad vård och tidvattenmodellen går ut på. En person med problem med livet kan hjälpas om fokus läggs på personen bakom ohälsan. Självklara idéer som att ”vara människa” och ”att hjälpa varandra” bör inte negligeras vid god omvårdnad. Slutsats Resultatet tolkas som negativt, för så länge det finns yrkesutövande sjuksköterskor med negativa attityder till personer med psykisk ohälsa återstår en fortsatt risk för dessa personers stigmatisering och diskriminering i hälso- och sjukvård. Med tidvattenmodellen är det möjligt att reducera negativa attityder och aktualisera återhämtning samt psykisk hälsa.

    Bakgrund: Att patienter med cancer lider av smärta är ett vanligt fenomen. Olika behandlingsalternativ finns men smärtan är fortfarande underbehandlad. Syfte: Att belysa upplevelsen av smärta hos patienter med cancer. Metod: Arbetet är en litteraturöversikt som inkluderade tretton vetenskapliga originalartiklar varav åtta kvantitativa, fyra kvalitativa och en mixad metod. Dessa artiklar söktes i databaserna Cinahl och PubMed. Resultat: Patienterna upplevde smärtan som hög med varierande karaktär. De hanterade sin smärta med både farmakologiska och icke-farmakologiska tillvägagångssätt. Patienterna upplevde påverkan på deras livskvalité och det vardagliga livet hämmades. Diskussion: Konsekvenser av smärtan resulterade i en negativ påverkan på patienternas livskvalité samt dagliga liv. Patienterna hanterade smärtan med både läkemedel och icke-farmakologiska strategier, med stöd och information från sjuksköterskan ökar chansen till god behandling. Att sjuksköterskan är medveten om smärtans omfattande påverkan och konsekvenser samt ger stöd och information gällande behandling skapar det en grund för samarbete med patienten och kan leda till god behandling. Slutsats: Smärta hos patienter med cancer är vanligt förekommande och sjuksköterskan bör besitta adekvat kunskap för att tillsammans med patienten bedriva god omvårdnad och lindra smärta på bästa vis.

    Bakgrund: Barnhälsovårdssjuksköterskan möter dagligen många barn och familjer i sitt arbete. En betydande del av arbetsuppgifterna är att försäkra sig om barnens livsvillkor och att tidigt identifiera och initiera åtgärder vid omsorgssvikt, för att skydda barnet mot en ohälsosam utveckling. Detta ställer stora krav på barnhälsovårdssjuksköterskan i sitt arbete, vilket kan upplevas som komplext och svårt. Syfte: Syftet med denna studie var att belysa barnhälsovårdssjuksköterskans erfarenheter av att arbeta med barn och familjer i omsorgssvikt. Metod: Studien hade en kvalitativ design med induktiv ansats som analyserats med innehållsanalys. Sju barnhälsovårdsjuksköterskor intervjuades. Resultat: Analysen resulterade i tre huvudkategorier; Att upptäcka omsorgssvikt där barnhälsovårdssjuksköterskans kompetens och barnhälsovårdsprogrammet hade betydande delar för förmågan att identifiera eventuell omsorgssvikt. Att stötta föräldrar där arbetet för barnhälsovårdssjuksköterskan är förebyggande för att förhindra uppkomsten av omsorgssvikt men även samverka med andra instanser när väl omsorgssvikt har uppstått. Samt Att möta utmaningar i arbetet där barnhälsovårdssjuksköterskan upplever arbetet svårt och känslan av ensamhet i stora beslut. Slutsats: Barnhälsovårdssjuksköterskan har i sitt arbete en betydande roll i stärkandet av det sociala skyddsnätet för familjer med omsorgssvikt. Även betydelsen av kollegialt stöd i de tuffa beslut som rör familjer med omsorgssvikt är av stor vikt.

    Abstract [en]

    Background: Pediatric health nurses meet many children and families daily in her work. A meaningful part of the task assignments are to secure the life conditions, and to identify and initiate actions at child neglect to protect the child from an unhealthy development. This implies high demands on the pediatric health nurse in her work and can be seen as complex and hard. Purpose: To enlighten the child health nurses experiences of working with children and families in child neglect. Method: The study had a qualitative design with inductive approach that was analyzed with content analysis, where seven pediatric health care nurses were interviewed. Result: The analysis resulted in three main categories; To discover child neglect where the pediatric health care nurse competence and pediatric health care program had significant capacity to identify eventual child neglect; to support parents where the work for health care nurse is preventative to avoid the appearance of child neglect, but also interact with other instances when child neglect has occurred; and to face challenges at work where the health care nurses believe the work is hard and experience a feeling of loneliness in major decisions. Conclusion: Pediatric health care nurses have in their work a significant role in the strengthening of the social safety net for families with child neglect. Even the meaning of collegiate support in difficult decisions concerning families with child neglect are of major importance.

Syfte: Belysa olika behov av stöd en närstående till någon som lider av cancer önskar sig få eller faktiskt får. Metod: En litteraturöversikt med en induktiv ansats. Materialet har analyserats och resulterat i fem olika kategorier: Att bli sedd och involverad; Att bli informerad; Att ha någon nära; Att känna trygghet i sin tro på Gud; Att känna meningsfullhet. Resultat: I resultatet framkom betydelsen av att sjuksköterskor erbjuder stöd i olika former som en familj eller närstående önskar sig. Erbjudande av stöd till närstående bidrar till ökad livsvalitet hos patienten. Diskussion: Om närstående får det stöd som de har behov av så underlättar det för dem att vårda sin sjuka anhöriga då de lättare kan uppleva meningsfullhet, begriplighet och hanterbarhet. Det ger dem ambition till att ge en god vård till den sjuke i hemmet. Slutsats: Sjukvårdspersonalen får inte isolera de närstående från patienten då de alla är involverade i den uppstådda situationen. Som vårdpersonal är det viktigt att ha ett holistiskt synsätt och se till hela familjen. Om de närstående bilr omhändertagna så ökar även patientens välbefinnande.

    Bakgrund: Det finns flera sjukdomar och orsaker som kan resultera i en stomi. Upplevelserna efter att ha fått en stomi kan påverka en persons liv på många plan. Syfte: Belysa upplevelser hos vuxna personer med en stomi. Metod: En kvalitativ litteraturöversikt med induktiv ansats. En analysprocess i flera steg som resulterade i fyra kategorier. Resultat: Deltagarna hade svårt att acceptera sitt nya utseende. Klädstil och kost ändrades ofta. Rädsla för läckage och lukt gav påverkan på deras vardagliga och sociala aktiviteter. Deltagarna upplevde ofta brister i information, stöd eller utbildning pre- och postoperativt. De sökte istället detta på annat håll. Diskussion: Efter en långvarig och problematisk sjukdom upplevde personer i många fall en lättnad över att få en stomi, medan stomin i sin tur upplevdes skapa problem på andra områden i livet. Negativa upplevelser i självbild, självförtroende, det vardagliga och sociala livet samt i närhet till andra var vanligt. Sjuksköterskans viktiga roll var att hjälpa och vägleda dessa personer för att främja en god livskvalité. Slutsats: Sjuksköterskan bör ta sig tid till att prata om personens alla olika upplevelser för att kunna stötta dem i deras nya liv med en stomi.

    Aim: Having a known midwife at birth is valued by women across the world, however it is unusual for women with fear of childbirth to have access to this model of care. The aim of this study was to describe the prevalence and factors related to having access to a known midwife for women referred to counseling due to childbirth fear. We also wanted to explore if women's levels of childbirth fear changed over time. Methods: A pilot study of 70 women referred to counseling due to fear of birth in 3 Swedish hospitals, and where the counseling midwife, when possible, also assisted during labour and birth. Results: 34% of the women actually had a known midwife during labour and birth. Women who had a known midwife had significantly more counseling visits, they viewed the continuity of care as more important, were more satisfied with the counseling and 29% reported that their fear disappeared. Fear of birth decreased significantly over time for all women irrespective of whether they were cared for in labour by a known midwife or not. Conclusions: Although the women in the present study had limited access to a known midwife, the results indicate that having a known midwife whom the women met on several occasions made them more satisfied with the counseling and had a positive effect on their fear. Building a trustful midwife–woman relationship rather than counseling per se could be the key issue when it comes to fear of birth.

    Abstract [en]

    Background: Continuity models of midwifery care are rare in Sweden, despite its well-known positive effects. The aim was to describe pregnancy and birth outcome in women participating in a continuity of care project in a rural area of Sweden. Method: A register-based study of 266 women recruited to the project and a control group of 125 women from the same catchment area. Midwives provided antenatal care and were on-call 7 a.m. to 11 p.m. for birth. Data were collected from the antenatal and birth records. Crude and adjusted odds ratios with 95% confidence intervals were calculated between women in the project and the control group. Results: There were more primiparous women and highly educated women recruited to the project, and fewer foreign-born and single women, compared to the control group. Women in the project met more midwives and were less likely to have a pregnancy complication. During intrapartum care, women recruited to the project were less likely to need labour augmentation and less likely to have an instrumental vaginal birth and elective caesarean section. They had fewer second degree perineal tears and were more likely to fully breastfeed at discharge. No differences were found in neonatal outcome. The continuity of a known midwife at birth was quite low. Conclusion: This study shows that women self-recruited to a continuity of care project in a rural area of Sweden had a higher rate of normal births. There were few differences if having a known midwife or not. Long distances to hospital and lack of staff affected the level of continuity.

    Background: Evidence-based antenatal care is one cornerstone in Safe Motherhood and educated and confident midwives remain to be optimal caregivers in Africa. Confidence in antenatal midwifery skills is important and could differ depending on the provision of education among the training institutions across Africa. Objective: The aim of the study was to describe and compare midwifery students’ confidence in basic antenatal skills, in relation to age, sex, program type and level of program. Methods: A survey in seven sub-Saharan African countries was conducted. Enrolled midwifery students from selected midwifery institutions in each country presented selfreported data on confidence to provide antenatal care. Data were collected using a selfadministered questionnaire. The questionnaire consisted of 22 antenatal skills based on the competency framework from the International Confederation of Midwives. The skills were grouped into three domains; Identify fetal and maternal risk factors and educate parents; Manage and document emergent complications and Physical assessment and nutrition. Results: In total, 1407 midwifery students from seven Sub-Saharan countries responded. Almost one third (25-32%) of the students reported high levels of confidence in all three domains. Direct entry programs were associated with higher levels of confidence in all three domains, compared to post-nursing and double degree programs. Students enrolled at education with diploma level presented with high levels of confidence in two out of three domains. Conclusions: A significant proportion of student midwives rated themselves low on confidence to provide ANC. Midwifery students enrolled in direct entry programs reported higher levels of confidence in all domains. It is important that local governments develop education standards, based on recommendations from the International Confederation of midwives. Further research is needed for the evaluation of actual competence.

    Background: Although women with fear of birth often report negative birth experiences, few studies have focused on their experiences in the long term. The aim of this study was to compare birth experiences a year after childbirth in two groups of women receiving treatment for experiencing fear of birth during pregnancy. Methods: As part of the U-CARE: Pregnancy Trial, a prospective multicenter randomized controlled trial comparing the effects of internet-based cognitive behavioral therapy (iCBT) and standard care among pregnant women with fear of birth. Women were recruited at three Swedish hospitals following a screening procedure that assessed their fear of birth. Data were collected online with the Childbirth Experience Questionnaire (CEQ), one question about the overall birth experience, and questions about personal background, collected before randomization. Results: A total of 181 women responded to the follow-up questionnaire a year after childbirth. Approximately half of participants reported a less positive birth experience. Preferred mode of birth, actual mode of birth, marital status and psychiatric history were associated with the domains of the CEQ. However, no statistically significant differences emerged between the treatment groups. Conclusions: Being randomized to receive iCBT or counseling with midwives for fear of birth was not associated with perceptions of the birth experience assessed a year after birth. Most participants reported less-than-positive birth experiences and scored low on the domain of the CEQ reflecting Own capacity. In response, additional research remains necessary to identify the best model of care that might facilitate positive experiences with giving birth among women with fear of birth.

    Background: There is evidence that continuity of midwifery care is beneficial to women. Women with fear of childbirth in Sweden are offered counselling, but receiving care from a known midwife during labour is unusual, despite its effects in reducing interventions and increasing birth satisfaction. The aim of this study was to describe and compare birth outcome and experience of intrapartum care among women with fear of childbirth who received intrapartum care from a known midwife, versus those who did not. Methods: An experimental study of 70 women referred to counselling due to fear of birth during pregnancy wherein the counselling midwife, when possible, also assisted during labour and birth. Results: Having a known midwife during labour and birth had a positive impact on fearful women's birth experience and their perception of pain, but there was no difference in onset of labour or mode of birth. Women who received care from a known midwife experienced better care with regards to information, participation in decision making and perception of control. Conclusion: This study indicates that having access to a known midwife might have an impact on women's birth experience. This study was limited by its small sample size and further research would need to randomise fearful women to counselling or continuity of care to determine the contribution of each to reducing fear.

    Bakgrund: Arbetsrelaterad stress hos sjuksköterskor är ett växande problem idag. Det kan leda till ohälsa hos sjuksköterskor samt påverka patientsäkerheten. Sjuksköterskan skall sköta sin egen hälsa så förmågan att ge god vård inte äventyras. Arbetsgivaren är skyldig att verka för en god arbetsmiljö.

    Syfte: Att beskriva sjuksköterskors upplevelser av arbetsrelaterad stress på sjukhus.

    Metod: En litteraturöversikt med totalt 17 vetenskapliga artiklar med studier av kvalitativ, kvantitativ samt mixad metod ingick. Studierna sammanställdes och analyserades utifrån en innehållsanalys och en integrativ litteraturöversikt.

    Resultat: Analysen resulterade i kategorierna; För mycket att göra på för lite tid, vacklande hälsa, problem inom personalgruppen, patienters och anhörigas förväntningar och krav, avsaknad av stöd, uppskattning och delaktighet samt upplevd stresshantering. Det framkom att den arbetsrelaterade stressen kunde påverkas av ålder och erfarenhet, kultur, antal barn, anställningsform och inom vilken sektor sjuksköterskorna arbetade.

    Diskussion: Enligt Karasek och Theorells Krav-kontrollmodell placeras sjuksköterskan i den spända situationen som innebär risk för ohälsa. För att komma ur situationen bör sjuksköterskor få mer kontroll samt stöd vilket arbetsgivaren bör samverka för. Sjuksköterskor är skyldiga att arbeta för en god hälso- och sjukvård vilket hindras på grund av den arbetsrelaterade stressen.

    Slutsats: Arbetsgivare bör vara lyhörda och tillsammans med sjuksköterskorna verka för en god arbetsmiljö och säkerställa patientsäkerheten. Vidare forskning behövs för att ta reda på vad arbetsgivare tillsammans med sjuksköterskor kan göra för att förebygga arbetsrelaterad stress, förbättra arbetsmiljön samt patientsäkerheten.

    Acute mountain sickness (AMS) is a potentially life-threatening illness that may develop during exposure to hypoxia at high altitude (HA). Susceptibility to AMS is highly individual, and the ability to predict it is limited. Apneic diving also induces hypoxia, and we aimed to investigate whether protective physiological responses, i.e., the cardiovascular diving response and spleen contraction, induced during apnea at low-altitude could predict individual susceptibility to AMS. Eighteen participants (eight females) performed three static apneas in air, the first at a fixed limit of 60 s (A1) and two of maximal duration (A2-A3), spaced by 2 min, while SaO(2), heart rate (HR) and spleen volume were measured continuously. Tests were conducted in Kathmandu (1470 m) before a 14 day trek to mount Everest Base Camp (5360 m). During the trek, participants reported AMS symptoms daily using the Lake Louise Questionnaire (LLQ). The apnea-induced HR-reduction (diving bradycardia) was negatively correlated with the accumulated LLQ score in A1 (r(s) = -0.628, p= 0.005) and A3 (r(s) = -0.488, p = 0.040) and positively correlated with SaO(2) at 4410 m (A1: r = 0.655, p = 0.003; A2: r = 0.471, p = 0.049; A3: r = 0.635, p = 0.005). Baseline spleen volume correlated negatively with LLQ score (r(s) = -0.479, p = 0.044), but no correlation was found between apnea-induced spleen volume reduction with LLQ score (r(s) = 0.350, p = 0.155). The association between the diving bradycardia and spleen size with AMS symptoms suggests links between physiological responses to HA and apnea. Measuring individual responses to apnea at sea-level could provide means to predict AMS susceptibility prior to ascent.

    Background - Many errors and threats to patient safety are caused by deficiencies in the organization, such as inadequate procedures, inadequate planning and lack of information or communication. A specific danger to patient safety is the various gaps in the continuity of care. An example of the gap is the transfer of patients from the high-tech intensive care environment to the less frequent personnel ward environment, a process called ICU transitional care. ICU-nurses, ward-nurses, physicians, and other healthcare professionals, may provide this care. The effects of a poorly coordinated discharge can lead to readmission to the ICU and avoidable deaths.This study is a part of a research project financed by The Kamprad Family Foundation, with a purpose to gain new knowledge about how efficiency and quality in patient transfers within healthcare care can be improved. The purpose will be achieved through cooperation between the two research subjects Quality Management and Nursing science.Purpose - The purpose of this paper was to gain knowledge about how to improve patient transfers from intensive care to the general ward, by describing the intensive care co-workers generated suggestions.Methodology/approach – Six interdisciplinary focus groups interviews were conducted with co-workers at two hospitals in Sweden. The interviews were taped, verbatim transcribed and analysed with a mixed method. The QM values: ‘customer focus’, ‘leadership commitment’, ‘participation of everybody’, ‘continuous improvement’, ‘process focus’ and ‘base decisions on facts’ were used as an analytic framework.Findings - The co-workers suggested that the process should be defined and structured. The analyses showed a total of 94 suggestions that mainly were focused at following areas; planning, information, and communication, interest in the transfer process, enough resources, standardize and create routines, learning, follow up and evaluate.Practical implications - ICU transitional care is multifaceted and needs a holistic, system thinking solution. The findings indicate that the process could be safer, and more efficient by implementing the co-worker generated suggestions. The suggestions can be used to improve patients’ transfers in many contexts in healthcare, not only ICU transitional care.

    Background

    In Sweden, three to four out of every 1000 pregnancies end in stillbirth each year. The aim of this study was to investigate whether women who had experienced stillbirth perceived that they had received an explanation of the death and whether they believed that healthcare professionals were responsible for the death of the baby.

    Methods

    An online survey of 356 women in Sweden who had experienced a stillbirth from January 2010 to April 2014. A mixed-methods approach with qualitative content analysis was used to examine the women's responses.

    Results

    Nearly half of the women (48.6%) reported that they had not received any explanation as to why their babies had died. Of the women who reported that they had received an explanation, 84 (23.6%) had a specific explanation, and 99 (27.8%) had a vague explanation. In total, 73 (30.0%) of the 243 women who answered the question Do you believe that healthcare personnel were responsible for the stillbirth? stated Yes. The women reported that the healthcare staff had not acknowledged their intuition that the pregnancy was proceeding poorly. Furthermore, they perceived that the staff met them with nonchalance and arrogance. Additionally, the midwife had ignored or normalised the symptoms that could indicate that their pregnancy was proceeding poorly. Some women added that neglect and avoidance among the healthcare staff could have led to a lack of monitoring, which could have been crucial for the outcome of the pregnancy.

    Conclusions

    Half of the women surveyed reported that they had not received an explanation of their baby's death, and more than one-fourth held healthcare professionals responsible for the death.

    The focus of the current study was to explore nursing home staff's understanding and how they approach family caregivers' existential life situations when caring for individuals with dementia. A qualitative interview study was conducted with 20 nursing staff in two nursing homes in northern Sweden. Face-to-face interviews were analyzed using interpretive content analysis. One overarching theme with three themes and nine subthemes comprised the findings. The themes describe how nursing staff were "in between" when they cared for individuals with dementia and encountered family caregivers' existential life situations. Nurses acted as advocates and safeguarded family caregivers via dialogues and interactions. Nurses were supporters and by"being in between"they eased family caregivers'feelings of powerlessness in relation to dying and death."Being in between" may give nurses a deeper understanding of family caregivers'existential needs, thus increasing family caregivers' well-being.

    Purpose - The purpose of this paper is to present and analyse the result from using a questionnaire measuring the presence of a number of QM values, as well as the use of Appreciative Inquiry (AI) and the feeling of pride in a healthcare setting focusing on transitional care.

     

    Methodology/approach – Co-workers at two different hospital’s intensive care units answered a previously developed and tested questionnaire. A new factor was developed in order to measure customer focus in the healthcare context. The result was analysed using SPSS with the aim of finding areas to improve the patient transfer process from intensive care to general ward.

     

    Findings – The questionnaire can be used to create an understanding of the existing culture in an organization but it needs to be analysed qualitatively. The result from the examined organizations show a common culture in the two units even though they are located in different hospitals and cities. The result further shows some differences between professions but overall, the result points at a relatively high agreement to the QM values in the organization indicating the presence of a quality culture.    

     

    Practical implications – Using a QM value survey in the healthcare context could help developing the care from a different point of view then ordinary. It could help to shift focus from individual to group and system.

    Abstract [en]

    Many children suffer from headaches. Since stress may trigger headaches, effective techniques to cope with stress are needed. We investigated the effectiveness of two mind-body techniques, transcendental meditation (TM) or hypnotherapy (HT), and compared them with progressive muscle relaxation (PMR) exercises (active control group). Children (9–18 years) suffering from primary headaches more than two times per month received either TM (N = 42), HT (N = 45) or PMR (N = 44) for 3 months. Primary outcomes were frequency of headaches and ≥ 50% reduction in headaches at 3 and 9 months. Secondary outcomes were adequate relief, pain coping, anxiety and depressive symptoms, somatisation and safety of treatment. Groups were comparable at baseline. Headache frequency was significantly reduced in all groups from 18.9 days per month to 12.5 and 10.5 at respectively 3 and 9 months (p < 0.001), with no significant differences between the groups. Clinically relevant headache reduction (≥ 50%) was observed in 41% and 47% of children at 3 and 9 months respectively, with no significant differences between the groups. No differences were observed in secondary outcome measures between the intervention groups. No adverse events were reported. Conclusion: All three techniques reduced primary headache in children and appeared to be safe. Trial registration: NTR 2955, 28 June 2011 (www.trialregister.nl)

    What is Known:

    • Stress may be an important trigger for both tension type headache and migraine in children.

    • Good data are lacking on the effect of transcendental meditation, hypnotherapy or progressive muscle relaxation as possible stress-reducing therapies in children with primary headaches.

    What is New:

    • Three non-pharmacological techniques, i.e., transcendental meditation, hypnotherapy and progressive muscle relaxation exercises, all result in a clinically significant reduction of headaches and use of pain medication.

    • No large differences between the three techniques were found, suggesting that children can choose either one of the three techniques based on personal preferences.

    Abstract [en]

    Introduction: Little is known about integration of Complementary and Alternative Medicine (CAM) in the Dutch healthcare system. The aim of the present study was to investigate how Integrative Medicine (IM) is practiced and how CAM is integrated with conventional medicine in curative and long-term healthcare centres in the Netherlands.

    Methods: A mixed methodology was applied, including a systematic internet search, an online survey and focus groups among healthcare professionals. Results: Most hospitals (92%) offered some form of CAM to treat anxiety (72%), restlessness (68%), pain (65%), sleeping problems (50%), stress (48%) and fatigue (36%). Mostly offered modalities were relaxation exercises (88%), art therapy (44%) and mindfulness (42%). Similar percentages of CAM use were found in other healthcare centres. Most CAM modalities were implemented without organizational vision, guidelines or protocols. Only 5% of implemented CAM was labelled as IM. Focus groups with health care professionals revealed the need to increase knowledge and to develop a vision for integration of CAM with conventional medicine, as well as to support effectiveness research in this area.

    Conclusions: This study is the first in the Netherlands to quantitatively map the extent of CAM integration in conventional health care. The number of CAM interventions offered at conventional health care settings was surprisingly high. However, integration was fragmented. Guidelines, education and research programs are needed to further integration of CAM, preferably organised in a Dutch Consortium for IM.

    Bakgrund: Urininkontinens påverkar individens hälsa och välmående. Sjuksköterskansbetydelse för att identifiera patienter med urininkontinens, och påbörja rättutredning/behandling, är avgörande. Dorothea Orems egenvårdsteori ger en grund tillförståelsen av egenvårdsbehov, för att uppnå hälsa. Begreppet hälsa är en definition somspelar en central roll genom litteraturstudien för att förstå hur hälsa påverkar individen, isin helhet. Syftet: Litteraturöversiktens syfte är att söka belysa upplevelser av att leva medurininkontinens och bäckenbottenträningens inverkan som egenvårdsbehandling. Metod:En litteraturöversikt har genomförts och induktiv design har nyttjats. Resultat: Individermed urininkontinens lider ofta av skam, ångest och rädsla, specifikt vid kontakt medvården, delvis beroende på normer och värderingar i samhället, angående urininkontinens.Bäckenbottenträning är en effektiv egenvårdsbehandling för patienter med urininkontinens.Bäckenbottenträning stärker muskulaturen hos individen och höjer därmed livskvalitetenrent psykiskt, då tillståndet avseendevärt förbättras. Sjuksköterskans spelar en central roll iatt hjälpa patienterna med bäckenbottenträningen. Problematiken ochdokumentationsbristen kring urininkontinens bör uppmärksammas för att en patientsäkervård ska kunna bedrivas. Slutsats: Genom att förbättra och öka informationen till samhälletoch påverka normer/värderingar, kan en säkrare vård initieras. Bäckenbottenträningen sombehandlingsmetod behöver växa som behandlingsmöjlighet för patienterna, dåbäckenbottenträningen visats vara en effektiv, säker och avgörande behandlingsmetod.

    Introduction

    Living with whiplash‐associated disorders (WAD) means living every day under the influence of pain and limitations. As the incidence of WAD and the related intensity of pain are somewhat higher among women than men, the aim of the present study was to describe women's experiences of living with WAD.

    Methods

    A purposive sample of seven women participated in individual in‐depth qualitative interviews, the transcripts of which were subjected to qualitative content analysis.

    Results

    The results of the analysis suggested six themes of women's experiences with WAD: living with unpredictable pain; trying to manage the pain; living with limitations; being unable to work as before; needing support and understanding; and learning to live with limitations. The findings showed that unpredictable pain limited women's strength to engage in activities of daily life and be as active as before. Support and understanding were important for their ability to manage changes in their daily lives.

    Conclusions

    Pain considerably affects the daily lives of women with WAD, particularly by limiting their ability to perform activities and to enjoy their professional and social lives. As women with WAD need support with managing their daily lives, nurses and other healthcare personnel should adopt a person‐centred approach, in order to support such women according to their individual needs and circumstances.

    Bakgrund: HIV är en immunsupprimerande sjukdom som kan drabba människor världen över. Sjukdomen är starkt förknippad med fördomar och diskriminering. HIV-drabbade individer är i behov av vård resten av livet. Det är därför viktigt att belysa deras upplevelse av bemötande och dess betydelse inom hälso- och sjukvården.

    Syfte: Syftet var att beskriva HIV/AIDS drabbade individers upplevelse av bemötandet och dess betydelse inom hälso- och sjukvården.

    Metod: En litteraturöversikt med induktiv design. De 18 artiklar som har används i resultatet är utifrån kvalitativ, kvantitativ och mixad metod.

    Resultat: Analysen resulterade i fem kategorier; ensidig kommunikation och bristfällig information, försummad sekretess, stigmatiserande beteenden, betydande faktorer för god vård och sambandet mellan tillfredställande vård och följsamhet. Resultatet visade på att hälso- och sjukvårdspersonalens förhållningssätt till HIV/AIDS hade en stor betydelse i hur bemötandet upplevdes samt en inverkan på den fortsatta vården.

    Diskussion: Då individerna inte bemöttes utifrån personcentrerad omvårdnad inom den allmänna vården blev kontrollförlust, ångest och dålig självkänsla en central del i deras upplevelse av hälso- och sjukvården. I jämförelse med kliniker med inriktning på HIV/AIDS beskrevs hälso- och sjukvårdspersonalen som kompetenta, icke-dömande och inbjudande. Slutsats: Hälso- och sjukvårdspersonalen bör arbeta utifrån personcentrerad omvårdnad för att skapa de bästa förutsättningarna för HIV-drabbade. Bemötandet har en stor betydelse för HIV- drabbades vård.

    Bakgrund: Smärta hos barn har länge ignorerats inom vården och behovet av smärtlindringtill barn har inte uppmärksammats förrän i slutet av 1980-talet. Inom vården utsätts barn oftaför så kallad akut smärta genom provtagningar och andra undersökningar. Upplevelsen avsmärta är individuell och omfattar barns fysiska, psykiska och kognitiva delar. Barn har rätttill en adekvat smärtlindring. Syfte: Att belysa olika smärtlindringsmetoders effekt vidvenpunktion hos barn i ålder 2 till 12 år. Metod: En systematisk litteraturöversikt där 15randomiserade studier inkluderades med 1574 deltagare. Litteratursökningen gjordes idatabasen PubMed. Resultat: I resultatet framkom det att EMLA® och lustgasbehandlingvar effektiva farmakologiska smärtlindringsmetoder. Av icke-farmakologiskasmärtlindringsmetoder var akupressur, indisk musik, distraktionskort och kalejdoskopeffektiva vid venpunktion. Slutsats: Det går att minska smärta hos barn vid venpunktion påolika sätt. Det framkom att både farmakologiska och icke-farmakologiska metoder äreffektiva smärtlindringsmetoder.

    Abstract [en]

    Purpose

    Self-management ability is commonly assessed in chronic disease research and clinical practice. The purpose of this study was to assess the structural and convergent validity of three commonly used self-management outcome measures in a sample of persons with neurological conditions.

    Methods

    We used data from a Canadian survey of persons with neurological conditions, which included three commonly used self-management measures: the Partners in Health Scale (PIH), the Patient Activation Measure (PAM), and the Self-Efficacy for Managing a Chronic Disease Scale (SEMCD). Confirmatory factor analysis was used to assess the structural and convergent validity of the three measures.

    Results

    When treated as single-factor constructs, none of the measurement models provided a good fit to the data. A four-domain version of the PIH was the best fitting model. Confirmatory factor analysis suggests that the three tools measure different, but correlated constructs.

    Conclusions

    While the PAM, PIH and SEMCD scales are all used as measures of patient self-management, our study indicates that they measure different, but correlated latent variables. None, when treated as single, uni-dimensional construct, provides an acceptable fit to our data. This is probably because self-management is multi-dimensional, as is consistently shown by qualitative evidence. While these measures may provide reliable summative measures, multi-dimensional scales are needed for clinical use and more detailed research on self-management.

    Longitudinal qualitative research in nursing is rare but becoming more common. Data collection and analysis over time provide some intriguing possibilities to better understand processes, development, and change in illness experience, healthcare organizations, and self-management. This paper aims to present a process for analyzing qualitative longitudinal data material, namely the Pattern-Oriented Longitudinal Analysis approach (POLA). We developed this approach after synthesizing experiences from two longitudinal qualitative projects and comparing our procedures and reflections with the relevant literature. Using the POLA approach, researchers can describe complexity and variation in changes over time. During the analysis process, emphasis is put on visualizing and identifying change at both an individual and a group level. Ontological and epistemological assumptions for the approach are also described. The benefit of this approach is the possibility to describe complexity and diversity in processes over time, which is important for the development of nursing knowledge. The analysis approach can be further used and developed by researchers seeking to understand variance or contextual features in processes and changes over time.

    Inledning: Endometrios är en kronisk östrogenberoende inflammationssjukdom som drabbar en av tio kvinnor. Sjukdomen kan ge kraftiga smärtor som är svåra att behandla och som ger en stor påverkan på kvinnornas liv. Syfte: Syftet var att beskriva kvinnors upplevelser och erfarenheter av att leva med smärta vid endometrios. Metod: Litteraturöversikt där 14 kvalitativa artiklar granskats och analyserats med kvalitativ innehållsanalys. Resultat: Omfattade fyra kategorier, Smärtans påverkan i det dagliga livet, smärta i kropp och själ, smärtans påverkan på relationer och självhantering och egenvård följt av nio subkategorier. Resultatet visade att smärta vid endometrios påverkade kvinnornas dagliga sysselsättning och upplevdes som både psykisk och fysisk påfrestande. Sjukdomen påverkade relationen till både familj och vänner negativt och brister i vården upplevdes. Kvinnorna använde sig av olika strategier och metoder för att kunna hantera sjukdomen och dess symtomen. Diskussion: Hur stöd från partner och familj, men även från vården, kan vara positiv vid hantering av endometrios. Att endometrios kan påverka det psykiska välbefinnandet och hur brister i vården kan påverka kvinnorna. Hur egenvård och självhantering har en betydelse för kvinnor då det kan öka livskvalitén och välbefinnandet samt vårdens betydelse vid egenvård. Slutsats: Endometrios påverkar hela kvinnans liv och att få möjlighet till stöd och hjälp samt egenvård och självhantering kan ha en stor betydelse för kvinnan. Hälso- och sjukvården bör få ökad kunskap och sjukdomen i stort behöver uppmärksammas för en ökad förståelse för drabbade kvinnor. Mer kvalitativ forskning ur både patient- och sjuksköterskeperspektiv behövs för att uppmärksamma sjukdomen och förbättra vården.
