    Abstract [en]

    Background: As dispersed ethnic populations in Swedish society expand, the healthcare system need to adapt rehabilitation services according to their needs. The experiences of trauma and forced resettlement have a continuing impact on health and musculoskeletal pain, as well as the intersecting structures that prerequisite the possibilities in the new country. To understand the specific needs of women from the Iraqi diaspora in Sweden, there is a need to elucidate the effects of pain on their everyday life. Aims: To elucidate everyday life with chronic pain from the perspective of women from the Iraqi diaspora in Sweden. Methods: Qualitative interview study according to Glaser's grounded theory. Results: The results from 11 interviews suggest that pain was associated with dependency on society as well as on family. It resulted in a struggle for sense of control, framed by faith in God, influenced by the healthcare system, and with support from family. The women's testimony of lack of continuity of care, resulting in recollection of lived traumas in every visit, is a vital sign of the unconscious power relations within health care and how representatives from health care, instead of being the ones who help the women forward, become the ones who hold them back. Conclusions: The results show the importance of challenging the normative assumptions embedded in health care and treatment for patients with chronic pain and of including the voice of "others". © 2013 the Nordic Societies of Public Health.

    Abstract [en]

    The purpose of this study was to develop an interview guide for use by primaryhealthcare professionals to support them in identifying the rehabilitation needs of forcedresettled women from the Middle East living with chronic pain. Previous findings togetherwith the existing literature were used as the basis for developing the interview guide in threesteps: item generation, cognitive interviews, and a pilot study. The study resulted in a16-item interview guide focusing on patients’ concerns and expectations, with considerationof pre-migration, migration, and post-migration factors that might affect their health. Withthe help of the guide, patients were also invited to identify difficulties in their daily activitiesand to take part in setting goals and planning their rehabilitation. The current interview guideprovides professional guidance to caretakers, taking a person-centered participative point ofdeparture when meeting and planning care, for and together, with representatives from dispersed ethnic populations in Sweden. It can be used together with the patient by all staffmembers working in primary healthcare, with the aim of contributing to continuity of careand multi-professional collaboration.

    Abstract [en]

    Meeting patients from other countries constitutes a challenge for health care. The purpose of this study was to increase knowledge about tacit understandings of treatment in practice by determining the perceptions of chronic pain and rehabilitation directed to resettled women from the Middle East, from a variety of health care professionals within primary care. Based on the results, we find a need to support and increase knowledge among health care professionals to involve the patient and consider her beliefs, expectations, background, current life situation, and spirituality, and to involve family in rehabilitation.

    Abstract [en]

    Purpose: To conduct a systematic review of existing methods to evaluate the individual aspects of welfare technology from the perspectives of independence, safety, activity, and participation. Furthermore, the study aimed to describe outcomes that have been the focus of previous research to evaluate individual aspects of welfare technology in older persons living in ordinary housing. Materials and methods: A systematic literature review in PubMed, CINAHL Plus, PsycINFO, Scopus, and Web of Science. Studies selected were those that explored the use of assistive and welfare technology devices from older persons’ perspectives, and which considered the concepts of independence, safety, activity and participation, and quality of life. Results: A broad spectrum of instruments was applied in the studies. For independence, three questionnaires were used in the identified studies. For safety, one instrument was used in two versions. To study activity and participation as well as quality of life, several scales were used. Additionally, several studies included qualitative approaches for evaluation, such as interviews, or posed one or more questions regarding the effects of welfare technology. Conclusions: The integration of digital assistive and welfare technology should be based on the needs of older persons, and those needs must be assessed using reliable and relevant instruments. The heterogeneity of the target group, i.e., older persons, together with the fact that assessments must give consideration to identifying goals, obstacles, and risks as well as users’ preferences, implies a person-centred approach.

    “Unga tjejer med invandrarbakgrund i mellanöstern och deras upplevelser i det svenska samhället” är en kvalitativ studie om hur invandrartjejer, med bakgrund i Mellanöstern, ser på sig själva i det svenska samhället, med ett särskilt fokus på deras sociala relationer, deras position i familjen och erfarenheter av hur de blir kategoriserade/stämplade som ”invandrartjejer”. Genom olika frågor kring: förhandlingar inom familjen, tjejernas sociala umgänge & kärleksrelationer, tjejernas position i det svenska samhället i förhållande till socialisation, självidentitet& identitetsskapandet, grupptillhörighet& etnisktillhörighet, etnicitet& etniskidentitet har vi ur ett sociologiskt perspektiv genomfört dessa i vår studie.  Anledningen till att vi just valt att studera detta område är att vi anser att det kan finnas en annan bild av dessa tjejer med invandrarbakgrund vilket media inte belyser. Samhället ser på att tjejer med invandrarbakgrund är förtryckta, strängt hållna och kontrollerade, men vi vill påvisa att det finns en annan bild av dessa tjejer. För att komma fram till resultatet så har vi använt oss av en kvalitativ metod med hermeneutisk ansats tematiskt strukturerade intervjuer. Vi valde att intervjua tio tjejer som är i åldern mellan 15-17 och har bakgrund i Mellanöstern.  Tjejernas erfarenheter och upplevelser har helt enkelt varit centrala för vår studie.

    Genom tiderna har det funnits ett särskilt band mellan människan och sällskapsdjuret. Forskning tyder på att denna relation bär med sig åtskilliga fördelar för människan. I den här studien undersöks vad hästen och hunden upplevs ha för inverkan på människan. I studien intervjuades 15 kvinnor som har djurrelaterade yrken. Nio beskrev hundens inverkan och sex beskrev hästens. Materialet komprimerades tills fem typer av inverkan framträdde: att människan (1) får en nära vän, (2) får välbefinnande, (3) får kraft, (4) utvecklar empati och (5) utvecklar ansvar. Skillnader som förekom var att hästberättelser fokuserade på gemenskap med andra människor i stallet och hundberättelser på sällskap från hunden. I berättelserna framkom även negativa teman som författarna har valt att bortse från och istället fokuserat på det positiva. Studien kan indikera att hundar och hästar kan komplettera varandra då de har olika syften i människans liv.

    Abstract [en]

    The present paper is a phenomenological-hermeneutic study that reflects on the experiences

    regarding motives and meaning of five foster parents concerning their specific being of foster

    parent for so-called unaccompanied children in Sweden. Scandinavian research has shown

    that this particular category of foster family plays a significant role in social child- and youth

    care. However, what is yet to be explored within current research are foster parents’ own

    experiences of their role. For this reason, the purpose of this study is to reflect on foster

    parents’ experiences regarding the role and its direct value are two-fold: On the one hand, the

    study of the meanings of being in the role of foster parent has important emancipatory

    implications. On the other hand, the study also suggests certain ideas regarding how the

    number of foster parents might be extended.

    Method: From a lifeworld perspective, and an ontological-phenomenological approach

    inspired by Heidegger, I have reflected on unstructured interviews by

    applying van Manen's phenomenological-hermeneutic method for thematic

    analysis.

    Findings: The study presents four considerable themes of motives, describing why

    people choose to assume the role of foster parenting. All of the four themes

    are about the informants’ perceptions of the role as a means of creative

    innovation of themselves and their surroundings.

    The study also presents six considerable themes of meaning describing the

    informants’ experiences of being a foster parent while meeting non-foster

    parents. All six themes are about the informants’ experiences of being

    portrayed and hidden while meeting with others.

    Consequences: The study shows the plausible with how foster parents’ emotional role taking

    and responsibility are negatively influenced by fellow men’s freedoms.

    There is a potential need to examine the possibility of a legislation that

    could conceivably support foster family of unaccompanied minors as a

    long-term form of cohabitation.

    Background: Dementia affected patients have increasing problems with memory , impaired language ability, which affects communication and behaviour is considerable. These difficulties may provide an outlet for aggression, agitation and anxiety, and are common in dementia, which is exhausting for patients and their relatives and carers. Behavioural changes are common condition in patients with dementia and affects quality of life and cause unnecessary suffering . Pharmacological treatment that is inserted to relieve these symptoms have many side effects. Printed out more knowledge about non- pharmacological nursing interventions that can facilitate care for patients , relatives and carers are great.

    Aim: To describe non- pharmacological nursing interventions in dementia care .

    Method: A general literature review of thirteen scientific papers compiled and described in a result.

    Results: Compilation resulted in two themes,  Music in dementia care and Stimulation in dementia care and four subthemes, Singing in the care, Listening to music, Activities and Consolation and fillip that showed a dominant positive effect on dementia affected patients and carers and there by create opportunities for caring in dementia care .

    Conclusion: The different nursing interventions may be useful in dementia care when they show a dominant positive effect on various behavioural changes, it seems memory stimulant and promotes communication between patients and carers which favours caring .

    Olika forskningsdiscipliner har i över 60 år studerat attribution. Dock finns ett kunskapsglapp då de flesta studier har gjorts på vuxna människor. Undersökningen koncentrerades därför till att studera attribution hos förskolebarn och söka reda på deras spontana reaktioner på sagofigurer och porträtt, samt deras bedömning av olika egenskaper hos dem. Deltagare var 16 förskolebarn, varav 9 flickor, vilka intervjuades i 45 minuter var. En innehållsanalys genomfördes vilken resulterade i olika teman och kategorier, därefter genomfördes en separat analys för barnens bedömning av tjocka respektive smala personer. Huvudresultatet visade att kategorier som var kopplade till utseendet var mer betydande än andra kategorier, samt att tendensen att föredra smala personer över tjocka påvisades. Resultaten diskuterades utifrån olika synvinklar på hur dessa resultat speglar samhället, och utifrån olika teorier om utveckling, samt vad detta kan tänkas ha för inverkan på både individen och samhället.

    OBJECTIVE: To examine the multivariate association between a model of self-perceived non-motor aspects and disability in cervical dystonia.DESIGN: A prospective and correlational design with two points of assessment. SUBJECTS: All 273 members with cervical dystonia from the Swedish Dystonia Patient Association were invited to participate. METHODS: Data were collected with one self-reported questionnaire. The questionnaire was sent by post on two separate occasions. Disability was the primary outcome variable measured by the Functional Disability Questionnaire. RESULTS: The questionnaire was completed by 180 individuals (66%) on both occasions. The multivariate association between the non-motor model and disability was statistically significant (adjusted R2 0.46, F(7, 149) = 19.76, p = 0.001). This indicated that 46% of the variance in disability was explained by the non-motor model. Self-efficacy appeared to be the most salient predictor of disability.CONCLUSION: The results of this study highlight the need for increasing awareness of self-perceived non-motor aspects among care providers treating patients with cervical dystonia. This presents opportunities for new rehabilitation possibilities that apply a behavioural medicine perspective.

    Background: In Sweden, approximately 440 children die in intrauterine fetal death annually. Losing a child before birth causes great sadness and despair to the parents. Midwives' have a significant role in supporting and nurturing parents in those situations, which is often experienced as a difficult and emotional task. Aim: To describe midwives' experiences associated to intrauterine fetal death. Method: Focus group interviews made with midwives working at a delivery ward in central Sweden, data were analyzed with  qualitative content analysis. Result: The data analysis resulted in ten subcategories,four categories and an overall theme. The categories were Presence, A meaningful task, Being both professional and a human being, and Support at work. The overall theme was To make meaning of the seemingly meaningless. The midwives needed time to be able to be fully present with the parents. According to the midwives it was a meaningful experience to be supportive and make a difference for the parents. To be professional while at the same time dealing with their own feelings appeared as a difficult task. They received the best support from their colleagues. Conclusion: Supporting the parents and creating a memorable moment with their stillborn baby was experienced as the most eager in these situations, and made the task meaningful for the midwives.

    Den salutogena modellen beskriver hälsa som ett kontinuum mellan ytterligheterna total ohälsa och total hälsa. En persons placering på kontinuumet kan förklaras av dennes ”känsla av sammanhang” (KASAM) - en skyddande resurs som hjälper individen att hantera livets stressorer. Mental ohälsa ökar bland unga vuxna i dagens samhälle och studenter är en utsatt grupp. Forskning visar att höga grader av KASAM bidrar till god självupplevd hälsa. I studien undersöktes, utifrån tre frågeställningar med tillhörande hypoteser, relationen mellan KASAM och mental ohälsa hos 273 högskolestudenter, varav 62 män. Deltagarna besvarade en enkät bestående av KASAM-formuläret och General Health Questionnaire. Data undersöktes med korrelations-, varians- och regressionsanalys.  Resultatet påvisade ett negativt samband mellan KASAM och upplevd mental ohälsa. Slutsatsen gav att KASAM tycks fungera skyddande mot mental ohälsa hos urvalets deltagare varpå diskussionen bland annat berör hur skolans miljö kan tänkas främja studenternas KASAM. Framtida forskning av organisationsstrukturers inverkan på KASAM föreslås.

    A social work perspective on transatlantic collaboration to prepare students for leadership and enhance service delivery2012Conference paper (Refereed)
    Abstract [en]

    The Transatlantic Consortium on Early Childhood Intervention was established in 2002 to address practice and policy pertaining to children with disabilities and their families. The goals of the Consortium were to contribute to the preparation of practitioners for leadership roles, to enhance service delivery in participating countries, and to enhance international understanding about early childhood intervention. The Consortium, which is now called, Global Education and Disability Studies (GEDS), involves universities in Europe (Jonkoping University and Stockholm University in Sweden; University of Porto in Portugal; Ludwig Maximilians University in Germany) and in the U.S. (University of North Carolina at Chapel Hill, Vanderbilt University, and the University of Colorado). It was initially established with funding from the European Union and the US Department of Education. The project has had three rounds of funding, and has facilitated study by hundreds of students from participating universities, who have spent Summer Institutes, extended stays, and whole semesters in partner countries taking courses; meeting with local practitioners, with consumers, and with family members; and examining their assumptions about disability. They have studied early childhood programs and policies, and carried out research on disability and early childhood intervention. The activities of the Consortium have involved faculty and students from the fields of Social Work, Education, and Psychology. The Consortium is especially important for social work as it focuses on policies and practice of relevance to vulnerable populations across the globe, and has engaged students and faculty members in close, direct interaction with clients, as well as social work practitioners, students, and faculty members. The presentation will address the activities and structure of the Consortium and its impact on students and their practice. Discussion will focus on how such a project can facilitate collaboration and give birth to new ideas.

    Abstract [sv]

    Syftet med studien är att undersöka arbetsförmedlares erfarenheter av Arbetsförmedlingens utvärderingsverksamhet och dess påverkan på kärnverksamheten. Inom ramen för en stor omorganisation som kallas förnyelseresan genomförs utvärderingar på både central och lokal nivå. På lokal nivå utvärderas främst så kallade växthus- och pilotkontor. På högre nivåer i organisationen används utvärderingar i första hand som beslutsunderlag. Utvärderingsverksamheten har lett till införandet av ny informationsteknik, organisatoriska förändringar samt metodutbildningar för arbetsförmedlare. Om arbetsförmedlarna gavs större inflytande över utvärderingskriterierna skulle utvärderingsverksamheten ytterligare bidra till utveckling av kärnverksamheten och stödet till de arbetssökande.

    Arbetsförmedlingen använder utvärdering huvudsakligen för kontroll och återrapportering till regeringen och inte för verksamhetsutveckling avseende utveckling av stödet till de arbetssökande. Det visar en analys av policydokument och kvalitativa intervjuer med ledningspersonal inom ett lokalt arbetsmarknadsområde. Arbetsförmedlingens utvärderingar genomförs centralt och kommuniceras nedåt i organisationen. New Public Management-modellen i organisationen tycks vara överordnad Arbetsförmedlingens funktion som människobehandlande organisation.

    Bakgrund: Antalet personer med demenssjukdom blir allt fler i samhället och merparten av människor som vårdas på en institution känner begränsad tillgång till vistelse i utomhusmiljö. Samtidigt anser anhöriga att deras närstående med demenssjukdom saknar stimulans för att vistas utomhus och behöver erbjudas mer tid i utomhusmiljö. Problem: Personer med demenssjukdom har önskemål om att vården behöver individanpassas och erbjudas efter enskild individs behov och identitet. Samtidigt har sjuksköterskor kunskapsbrist kring betydelsen av utomhusmiljö för personer med demenssjukdom. Syfte: Att ur ett patientperspektiv undersöka utomhusmiljöns betydelse för personer med demenssjukdom. Metod: En allmän litteraturöversikt enligt Friberg (2012) med beskrivande design. Resultat: Utomhusmiljön hade en inre och yttre påverkan på personer med demenssjukdom. Personer med demenssjukdom upplevde en känsla av välbefinnande, livskvalitet, frihet, oro, rädsla samt social interaktion med andra, vid utomhusvistelse. Slutsats: Kunskap om utomhusmiljöns inre och yttre påverkan kan ligga som grund för sjuksköterskan att tillgodose en god och säker vård. Ökad kunskap kring det undersökta fenomenet kan även utgöra ett stöd för att främja hälsa och välbefinnande samt lindra lidande hos personer med demenssjukdom.     Bakgrund: Fetma är en medicinsk diagnos och många av kroppens biologiska funktioner är i obalans med ökad risk för följdtillstånd. För att motverka ohälsan är viktminskning ofta det enda alternativet. Fetmakirurgi är en hållbar viktminskningsmetod, vanligast är gastric bypass, med snabba långsiktiga effekter på följdtillstånd. I riket, liksom i Västmanlands län opereras en övervägande majoritet kvinnor. Varför män i Västmanland opereras i lägre utsträckning än kvinnor är okänt.

    Syfte: Studiens syfte var att erhålla och analysera mäns värderingar och drivkrafter bakom fetmakirurgi. Målet var även att studera mäns subjektiva uppfattningar av västerländska kroppsideal och dess roll bakom beslutet till fetmakirurgi, samt deras förväntningar av det kirurgiska ingreppet.

    Metod: Informanterna rekryterades genom Överviktigas riksförbund i Västmanland. Urvalet bestod av fem män i åldern 40-49 år som samtliga intervjuades om subjektiva erfarenheter av och attityder till fetma och fetmakirurgi. Intervjuerna utfördes med hjälp av ett intervjuschema och en tematisk tankekarta baserade på Hälsouppfattningsmodellen.

    Resultat: Den övergripande drivkraften bakom beslutet var männens hälsa och välbefinnande. Att orka jobba och vara aktiv med familj och vänner beskrevs även som avgörande. Rådande kroppsideal eftersträvades inte. Enligt männen var fetmakirurgi resultatet av insiktsprocesser och ett tillräckligt socialt stöd; fler bör se fetmakirurgi som en hållbar viktminskningsmetod

    Syftet med studien var att undersöka socionomstudenters åsikter gällande mäns våld mot kvinnor i nära relationer samt deras åsikter om samhällets stödinsatser. Vidare ville vi ta reda på i vilken grad studenterna ansåg att utbildningen hade ökat deras kunskaper om mäns våld mot kvinnor i nära relationer. En enkätundersökning genomfördes och respondenterna bestod av 57 socionomstudenter som studerade sista året på en högskola i mellersta Sverige. Resultatet analyserades främst utifrån könsmakt som teoretiskt perspektiv samt med hjälp av tidigare forskning i ämnet. Resultatet visade att socionomstudenterna i många avseenden hade liknande åsikter om våldets orsaker och samhällets stödinsatser. Majoriteten av socionomstudenterna ansåg att våldet hade med patriarkala attityder att göra och ansåg i stor utsträckning att en kvinna omedelbart ska anmäla om hon blivit utsatt för våld. De stödinsatser som studenterna ansåg var mest centrala i arbetet med våldsutsatta kvinnor var socialtjänst och kvinnojour. I undersökningen framkom även att studenterna tyckte att utbildningen hade haft en inverkan på deras kunskaper i ämnet mäns våld mot kvinnor i nära relationer.

    Abstract [en]

    Background: Non-specific low back pain is one of the most common reasons for seeking help in primary care. For chronic, non-specific low back pain exercise rehabilitation is a first choice and focus in the last decade has been on motor control exercises. Psychosocial factors such as self-efficacy and fear of movement may be important to consider in the treatment as they play a role in how low back pain develops. Aim: To investigate how rated pain, self-efficacy and fear of movement as well as level of function changed in individual patients with chronic, nonspecific low back pain during and after an intervention group with motor control exercises. Method: The design was an A-B-A single-subject design (SSED). Pain level, self-efficacy and fear of movement were measured by self-assessment questionnaires once a week during phase A-B-A and level of function before and after the intervention. The intervention lasted for eight weeks, once a week. Data were presented visually and analysis was done to prove statistical (2-SD band method) and clinical significance. Results: The analysis showed that rated pain, self-efficacy, fear of movement and level of function changed in a positive direction for several of the patients. Statistical and clinical significance varied. Conclusion: Graded motor control exercises in a group under the supervision of a physiotherapist seem to have a positive effect for several of the participants with regards to level of pain, self-efficacy, fear of movement as well as level of function. Since SSED has a limited generalizability, results should be replicated in further studies.

    Breastfeed has health benefits for both mother and child. The midwife has disclosure requirements when it comes to breastfeeding benefits and shall mediate relevant knowledge about breastfeeding to the pregnant woman and her partner. Through WHO:s 10 steps breastfeeding can be promoted, supported and protected. The midwifes support is an important aspect under a womans pregnancy. Breastfeeding preparation must start at the maternity services so the woman can receive great possibilities to breastfeed. The aim was to describe midwives experiences giving breastfeeding preparation to primiparas in the maternity services. A qualitative interview method was used and ten midwives were interviewed at two different regions in Centralsweden. A semi-structured interview guide was used. The data collection was analyzed with a qualitative content analysis. The content analysis developed three main categories; Midwives experiences are that majority of the first parturients have a positive attitude towards breastfeeding, Midwives consider that breastfeeding are important and Midwifes strategies and challanges and also 10 belonging subcategories. Midwives considered breastfeeding preparation to primiparas as an important part of their work at the maternity services, and that breastfeeding knowledge was mediated from current evidens and to support an informed choice. An obstacle was to reach the pregnant women with information who fokused on the childbirth, possibilities were parenting preparation groups and individual conversation. The breastfeeding preparation was different between the regions. One region had an early focus on breastfeeding and the other one introduced breastfeeding later on during the pregnancy. Most of the midwives considered that breastfeeding education was irregular and the majority desired more time to offer good breastfeeding preparation for primiparas.

    The spouses of people suffering from dementia are commonly first-in-line caregivers. This canhave a considerable effect on their own lives, health and marriages. Several studies havefocused on spouses’experiences, but very few have focused in any depth on their descrip-tions of themselves as subjects. Therefore, the aim of this study is to describe how spousecaregivers can express themselves when living with and caring for their partners withdementia. The study has a qualitative approach with a discourse analysis design and usesanalytical tools such as rhetoric, subject positions and categorization. The results reveal threesubject positions: as an actor, as a parent and as a survivor. The results show that as spousesstruggle with external and internal clashes as subjects, they therefore need to develop copingstrategies. They also experience pronounced loneliness and a risk to their own health. There isthus a need to support these spouses as individuals in their differing and changing needs.

    Abstract [en]

    This study aims to investigate how social services in a county in central Sweden work preventively against honor-related violence and oppression. The study aims to investigate the obstacles, challenges and preventive opportunities that social workers face in the work. Based on a qualitative data collection method in which six social secretaries were interviewed, the study has shown that the majority of respondents have a critical view towards that the problem of honor is cultured and investigated solely from cultural features. The study also showed that the respondents felt that there was a lack of competence in the social service regarding honor matters and emphasized that there is a great need for excellence in education on the subject and also that there was a great need for aimed education in fields that focus on honour related violence and oppression. It was found that respondents disagreed on whether the social service´s work is preventing honor-related violence and oppression. Some of the socialworkers considered that the social service`s work is preventing and some disagreed. The respondents also emphasized the importance of increased cooperation between the social services and the school in preventing the work against the violence.

    Rollkongruensteorin innebär att kvinnor har svårare än män att nå ledarroller och kvinnliga ledares beteende bedöms mindre fördelaktigt än manliga ledares på grund av att ledarroller anses passa stereotypiskt manliga egenskaper bättre än stereotypiskt kvinnliga egenskaper. Studien undersökte om män och kvinnor uppskattade en chef och dennes meddelande med hård ton bättre eller sämre beroende på chefens kön och yrke som militär- eller förskolechef. Fyrtionio män och fyrtiofem kvinnor besvarade en av fyra versioner av enkäter innehållande frågor och påståenden med svarsalternativ. Variansanalyser visade att kvinnor tyckte att militärchefer hade bättre yrkeslämplighet och bättre ledaregenskaper än förskolechefer. Både kvinnor och män ansåg att ledaregenskaperna hos en manlig chef var bättre än hos en kvinnlig chef. Chefen ansågs ha mer relationsinriktade egenskaper om denne var militär eller man. Vidare ansåg kvinnor att kvinnliga chefer var yrkeslämpliga i större utsträckning än vad män ansåg. Resultatet både stöder och motsätter sig rollkongruensteorin.

    Abstract [en]

    Mental illness is one of the major public health issues in Europe and one of the biggest challenges Sweden facing is the growing mental illness among people younger than 25 years. Statistics shows that the self-reported mental illness has increased mainly among young women between 18-25 years. The underlying reasons for the increase in mental illness is relatively unknown, more knowledge is required in this field to develop preventive interventions and strategies. The aim of this study was to examine which thoughts young women have about the future and how those are perceived to affect their mental health.

    A qualitative approach with semi-structured interviews have been applied. The sample consisted of women who were working or studying and were born between the years 1993-1996. Altogether six individual interviews were carried out and data analysis consisted of a manifest content analysis. The result showed that young woman’s thoughts about the future include both individual thoughts and more general thoughts about Sweden and the world. An optimistic attitude towards their own future were evident while a pessimistic attitude towards Sweden and the future of the world emerged. Thoughts of the future in relation to their own future is a contributing factor to why young girls feeling depressed and anxious.

    Idag är psykisk ohälsa den vanligaste orsaken till långtidssjukskrivning i Sverige. Denna studie syftade till att undersöka vad det innebär för kvinnor att vara långtidssjukskrivna för psykisk ohälsa och återvända till arbetet. Datainsamlingen bestod av semistrukturerade intervjuer. Totalt intervjuades åtta kvinnor som varit sjukskrivna i minst två månader. Kvinnorna var bosatta i Uppsala län, Västmanlands län och Västra Götalands län. Dataanalysen genomfördes i linje med de riktlinjer IPA-metoden förespråkar. Resultatet påvisar att det förekommer en del svårigheter med att vara långtidssjukskriven för psykisk ohälsa. Mest framträdande var en känsla av orkeslöshet och isolering. Arbetsåtergången var utformad på olika sätt, den vanligaste rehabiliteringsåtgärden var arbetsanpassning. Studiens resultat tyder på att arbetsplatser saknar tydliga rutiner för arbetsåtergång vid långtidssjukskrivning för psykisk ohälsa. Vidare framgick det att delaktighet och inflytande, lyhörda chefer och den egna viljan var viktiga utgångspunkter för en god arbetsåtergång. Socialt stöd betraktades som en viktig förutsättning under hela sjukskrivningsförloppet.

    Abstract [en]

    Stress and stress related diseases is a problem in today’s society and those most affected by it are young people. The stress is also unequal as women are more affected than men. For students stress can be a factor that affects their performance in school negatively. The aim of the study was to investigate for perceived stress among students at Mälardalen University and analyze gender and study program differences. The study had a quantitative approach and was conducted thru a cross-sectional study. The study was answered by 72 questionnaires that were completed by students from four study programs at Mälardalen University. The data of the study was analyzed by chi-2-tests in the data program SPSS. The result of the study gave an account of the perceived stress among the students who reported to perceive some stress, but it was neither high nor low. The results showed gender- and study program-differences that were not statistically significant. However the differences among study programs could be traced to the gender differences as three out of four programs were male or female dominated. The conclusions of the study are that the students have some perceived stress and that there are some differences in perceived stress for gender and study programs that are not statistically significant.

    An interactive health technology solution for encouraging physical activity: a first model based on a user perspective2018Doctoral thesis, comprehensive summary (Other academic)
    Abstract [en]

    Globally, the level of physical inactivity is increasing. The overall aim of this thesis was to develop and test a first model of an interactive health technology solution (called App&Move) that should encourage physically inactive adults to be more physically active. App&Move was iteratively developed based on the user perspective, a so-called user-centered design. First, available technology was assessed; the validity and reliability of one smartphone pedometer application and one commonly used traditional pedometer were investigated. It was found that none of the investigated pedometers could measure correctly in all investigated situations. However, measurements by a smartphone appli-cation was identified to have high potential when aimed at monitoring physical activity in everyday situations. As the next step, a questionnaire was developed and distributed in central Sweden. The 107 respondents who answered the questionnaire were divided and analyzed in groups of users and non-users of physical activity self-monitoring technology. The results showed that users and non-users of such technology mainly had similar opinions about desirable functions of the technology. To gain further knowledge concerning how to design App&Move, the target group physically inactive non-users participated in focus group interviews. Important results were that the technology should focus on encouragement rather than measurements and that it preferably should be integrated into already existing technology, if possible already owned and worn by the person. A brainstorming workshop confirmed that the smartphone was a suitable platform, and a decision to develop a smartphone application was taken. A first draft of App&Move was developed, focusing on encouragement and measuring everyday activity and exercise in minutes per day. App&Move was based on available physical activity recommendations and strategies for successful behavior change. App&Move was positively received in a user workshop and thereafter iteratively refined and developed based on further user input. App&Move was usability tested in 23 physically inactive adults who used App&Move for four weeks and answered two questionnaires. Three usability aspects, effectiveness, efficiency and satisfaction, were assessed as follows: acceptable, high and medium, and slight increases in activity minutes were observed during the test period. To conclude, this thesis has investigated the user perspective of physical activity self-monitoring technology with a target group of physically inactive adults. Based on these findings, a behavior change application for smartphone, App&Move, was presented. The usability test indicated promising results with respect to usability and indicated an ability to encourage the users to physical activity to some extent.

    Abstract [en]

    Today, there are a variety of pedometers available based on different technologies. The aim of this study was to investigate step measurement accuracy regarding two different pedometer technologies, spring-suspended lever arm and accelerometers, in six walking environments, compared to a reference method. Descriptive data indicates that the spring-suspended lever arm pedometer had the largest SD and underestimated the steps in five out of six walking environments. The ANOVA showed that both pedometer technologies performed accurate measurements in one of the walking environments. However, none of the pedometer technologies performed accurate step measurements in all six walking environments.

    The aim of this study was to investigate the validity and measurement agreement of a smartphone pedometer application and a traditional pedometer, both compared to manual step count, in six environments. The study also examined the relative reliability of two positions of the smartphone pedometer application in the six environments. Twenty participants carried two smartphones (iPhone 4S) with the same pedometer application (Pedometer FREE GPSþ) in two device positions together with a traditional pedometer (Yamax LS2000) while walking in six environments. The pedometer application in the chest position (app chest) differed significantly from the manual step count when walking uphill (p¼0.003), the application in the trouser (pants) position (app pants) differed for downhill (p¼0.001) and the traditional pedometer (trad ped) differed upstairs (p¼0.007). The measurement agreement was high for app pants uphill, app chest downhill and trad ped downstairs. The relative reliability was fair for app chest, but poor for app pants. In conclusion, the pedometer application and traditional pedometer were valid for step self-monitoring in some environments, and the measurement agreement compared to manual step count was mainly medium to high. The relative reliability for the smartphone pedometer application was partly acceptable. Smartphones could be useful in some environments for physical activity self-monitoring.

    Background: Different kinds of physical activity (PA) self-monitoring technologies are used today to monitor and motivate PA behavior change. The user focus is essential in the development process of this technology, including potential future users such as representatives from the group of non-users. There is also a need to study whether there are differences between the groups of users and non-users. The aims of this study were to investigate possible differences between users and non-users regarding their opinions about PA self-monitoring technologies and to investigate differences in demographic variables between the groups. Materials and methods: Participants were randomly selected from seven municipalities in central Sweden. In total, 107 adults responded to the Physical Activity Products Questionnaire, which consisted of 22 questions. Results: Significant differences between the users and non-users were shown for six of the 20 measurement-related items: measures accurately (p = 0.007), measures with high precision (p = 0.024), measures distance (p = 0.020), measures speed (p = 0.003), shows minutes of activity (p = 0.004), and shows geographical position (p = 0.000). Significant differences between the users and non-users were also found for two of the 29 encouragement items: measures accurately (p = 0.001) and has long-term memory (p = 0.019). Significant differences between the groups were also shown for level of education (p = 0.030) and level of physical exercise (p = 0.037). Conclusion: With a few exceptions, the users and the non-users in this study had similar opinions about PA self-monitoring technologies. Because this study showed significant differences regarding level of education and level of physical exercise, these demographic variables seemed more relevant to investigate than differences in opinions about the PA self-monitoring technologies.

    The development and usability evaluation of an interactive health technology solution for the encouragement of physical activity in inactive adults - based on the user perspectiveManuscript (preprint) (Other academic)
    Abstract [en]

    Physical inactivity is increasing worldwide. Technology shows good evidence forhaving a good impact on health. Successful behaviour change regarding physical activity requires user involvement and the integration of relevant behaviour change techniques into the technology. The aim of this study was, based on the user perspective, to develop and later evaluate the usability of an interactive health technology solution that can encourage physically inactive adults to physical activity. A mixed methods design with a user-centred approach was used. The data collection included a brainstorming workshop, a user workshop, technical development and a usability evaluation (effectiveness, efficiency, satisfaction) involving 35 participants. From the brainstorming workshop, four themes were identified: focus, platform, characteristics and content. A schematic model of a first prototype was presented at the user workshop, and the feedback was positive. A behaviour change application was developed (App&Move), with a focus on encouraging functions. App&Move, which measures physical activity in minutes per day, has integrated behaviour change techniques, is based on available physical activity recommendations, relies on scientific evidence and is strongly based on the user perspective. The usability evaluation of App&Move showed that effectiveness was acceptable, efficiency was (with one exception) high, and satisfaction was medium. The results from this study contributes to the development of a first model of a behaviour change application that aims to encourage physically inactive adults to physical activity. In conclusion, the usability evaluation of App&Move showed promising usability, and the application seemed to encourage physical activity to some extent.

    How can technology promote physical activity? Opinions from physically inactive non-users of self-monitoring technologiesManuscript (preprint) (Other academic)
    Abstract [en]

    BACKGROUND: To achieve a more physically active and healthier population, the opinions of potential future users of physical activity self-monitoring technologies need further investigation. Therefore, the aim of this study was to investigate the opinions of physically inactive non-users of physical activity self-monitoring technologies, regarding how and when technology possibly could measure and encourage physical activity. METHODS: Two focus group interviews were performed, consisting of 11 physically inactive persons who had no previous experience with regular use of physical activity self-monitoring technology. The focus groups were analyzed by qualitative content analysis. RESULTS: The transcripts of the two focus groups resulted in 17 subcategories and five categories, which formed two themes: measurement complexity and performance and basic qualities. CONCLUSION: This study showed that the physically inactive non-users of physical activity self-monitoring technologies had positive, innovative and mainly similar opinions about how technology could promote physical activity. Since the concept of encouragement permeates the results, it can be concluded that the physical activity technology for this particular target group should primarily be based on encouraging features and functions. Further, the study indicated that it is preferable to integrate physical activity self-monitoring technology into already existing technology that the user already owns.

    eHomecare – for safety and communication in everyday life: The perspectives of older users, relatives and care managers2017Doctoral thesis, comprehensive summary (Other academic)
    Abstract [en]

    The overall aim of this thesis was to examine how eHomecare affects the daily lives of older users and their relatives, with a focus on safety and communication. A further aim was to explore care managers’ perspectives, expectations and experiences of eHomecare and its implementation. Methods: Participants in study one and two were older people with granted eHomecare and eight relatives and, care managers in study three and four. Data were collected through four qualitative studies, using individual interviews in the first and second studies before the introduction and after six months’ use of eHomecare, by individual vignette-based interviews in the third study, and with focus-group interviews in the fourth study. Data were analysed using different qualitative content analyses. Results: The participants described safety as a part of everyday life. eHomecare was found to facilitate a ‘new safety’ for older people and a ‘re-established safety’ for relatives, yet its use raised concerns about ethical considerations and reduced human contact. Participants could attain feelings of togetherness and affection through communication, although this was also considered a vulnerable activity due to physical changes and loss of other people. Used correctly, eHomecare increased communication and thus closeness and participation for the participants. For older participants unable to use the technology as hoped, eHomecare led to disappointment. Care managers expressed that eHomecare can increase older peoples’ everyday life-quality if they receive the right tools at the right time. Care managers, however, have difficulties with eHomecare’s management process. While they struggle with their own attitudes, lack of time and high workloads, their decisions are also influenced by surrounding organisations and the older people’s relatives. Care managers’ own organisations, work situations, relevant stakeholders and society in general can hinder them in managing eHomecare as a new homecare service. Widespread information about eHomecare and opportunities for relevant stakeholders to participate in its implementation are good preconditions for fulfilling the mission of care managers. Conclusions: The findings describe eHomecare from the perspectives of its older users, their relatives and the care managers responsible for managing the service. Used correctly, eHomecare increases possibilities for communication and provides safety. However, care managers have a complex mission when managing the service and they express a need for support and knowledge. The findings can be used clinically to develop older peoples’ utilization of eHomecare and to develop support for the fulfilment of care managers’ mission.

    In Sweden, information and communication technology has been introduced into home care services for older people, i.e., eHomecare. In this study, we explore social workers’ (care managers, [CM]) expected orexperienced hindrances and preconditions affecting their role and missionin eHomecare implementation. Four focus group interviews were conducted with participants from four municipalities in central Sweden during thespring of 2015. One focus group consisted of CMs with experiences of eHomecare and the other three groups of CMs without eHomecare experience. The study found that several hindrances may occur when eHomecare is implemented as part of home care, which in turn affect CMs´mission. CMs need a society that is well-informed in general aboute Homecare and requires relevant stakeholders to participate in the implementation process. CMs need an organization that is clear about their mission and a work situation that gives them the right circumstances for professionally managing eHomecare. It is also understood that older people must be given the right circumstances to adopt the technology, thus facilitating the CMs’ mission. Knowledge and time are assumed to be preconditions for CMs when managing eHomecare.

    To implement information and communication technology (ICT) in elderlycare can be a challenge for healthcare organisations. In Sweden, caremanagers (CMs) are responsible for offering ICT as a new part ofhomecare, i.e. eHomecare. The aim of this study was to focus on CMs’perspectives and describe their perceptions of eHomecare. The studyhas a qualitative approach. Semi-structured interviews based onvignettes were conducted with 12 CMs in a medium-sized municipalityin central Sweden. Data were analysed using qualitative analysis. Thefindings showed that CMs are influenced by surrounding organisationsand relatives in their decision to grant eHomecare. There were alsodifficulties in CMs’ management of eHomecare. Furthermore, eHomecarewas perceived to improve the quality of everyday life for older. It isunderstood that the management of eHomecare is a challenging taskfor CMs and a complex mission. Interactions among CMs, relatives, careproviders, and other factors as for example CMs’ own attitudes have animpact on CMs’ decisions.

    The study's aim was to extend descriptions of how older patients with granted eHomecare and their relatives understand safety, and further to describe how they experience safety in everyday life. The study was conducted in Sweden. The participants were 12 older patients who had been provided with eHomecare and 8 relatives. Data were collected by semi-structured individual interviews. A conceptual framework of safety was used and a qualitative content analysis was conducted in a deductive and an inductive phase. The deductive results are presented in predefined categories: perceived sense of safety, disturbance and threats, re-establishing safety, and new safety. The inductive analysis resulted in two main categories: safety as part of everyday life and eHomecare as safety. The results show that eHomecare can promote safety for older patients and their relatives. Existing doubts and ethical concerns about the service can be minimized by providing adequate information.

    Mer ambitiösa arbetsgivare i kommunal vård och omsorg har bättre hälsa bland medarbetarna2011Manuscript (preprint) (Other academic)
    Abstract [sv]

    Det be­hövs mer kunskap om hur arbetshälsan på­verkas av ledarskap och organisa­to­riska åtgärder och vilka åtgärder som är mest effektiva. Vid FALF-konferensen 2010 presenterades resultat från en studie om regionala skillnader i sjukfrånvaro hos kommunanställda baserat på personalens perspektiv (Åkerlind I, Ljungblad C, Granström F: Verksamhetsstyrning, arbetsmiljö och hälsa i kommunal vård och omsorg). Dessa resultat visade att skillnaderna mellan kommuner i personalens självskattade hälsa och sjukfrånvaro var relaterad till såväl utbud och deltagande i konkreta hälsofrämjande aktiviteter som till hur nöjd man var med ledarskapet och organisationsklimatet på arbetsplatsen. I det nu aktuella projektet belyses samma förhållanden utifrån vad arbetsgivaren i dessa kommuner rapporterar. Syftet har varit att belysa om arbetsgivare med ett mer aktivt hälso- och arbetsmiljöarbete har ett bättre hälsoläge bland personalen och vilka åtgärder som i så fall har betydelse.

     

    Ett frågeformulär (personalenkäten) med fokus bl.a. på ledarskap, organisationskultur, socialt klimat, hälsofrämjande åtgärder och självskattad hälsa, skickades ut till ett representativt urval av 15 000 anställda inom kommunal vård och omsorg om äldre och funktionshindrade i ett slumpmässigt urval av 60 av Sveriges 290 kommuner. En databas med aggregerade data från frågeformuläret samt registerdata om sjukfrånvaro hos de anställda i de 60 kommunerna konstruerades. Ett annat frågeformulär (arbetsgivarenkäten) skickades till den högsta chefen inom förvaltningen för vård och omsorg i samma kommuner. Denna enkät omfattade 1/ grundläggande fakta om organisationen, 2/ verksamhetsstyrning, 3/ hälso- och arbetsmiljöarbete samt 4/ företagshälsovård. Samband på kommunnivå mellan enkätdata och aggregerade data om personalens hälsa och sjukfrånvaro analyserades med hjälp av icke-parametrisk statistik.

     

    Resultaten från arbetsgivarenkäten visar att varken antalet chefer och anställda eller antalet anställda per chef var relaterat till hälsa och sjukfrånvaro. Inte heller kommunernas ambitionsnivå när det gäller kvalitetsarbete och verksamhetsstyrning i allmänhet hade något samband med utfallsmåtten. Däremot fanns tydliga samband mellan personalens självskattade hälsa och vissa aspekter av kommunens insatser beträffande hälso- och arbetsmiljöarbetet. När det gäller ledningens organisatoriska stöd för personalens hälsa var genomförandet av lokala hälsoprojekt särskilt viktigt. Även förekomsten av särskilt utsedda resurspersoner för hälsoarbetet (samordnare, hälsoutvecklare, hälsoombud, friskvårdsinspiratörer) hade betydelse. Det samma gäller utbudet av konkreta hälsofrämjande åtgärder som riktar sig till individer. Det fanns dessutom en god överensstämmelse mellan arbetsgivarnas och medarbetarnas rapportering om detta utbud, vilket ytterligare understryker dess betydelse. Däremot hade inte förekomsten av skriftlig policy för hälsofrämjande arbete eller olika former av chefsstöd i hälso- och arbetsmiljöarbetet någon betydelse (med undantag för konsultstöd i det systematiska arbetsmiljöarbetet). Alla kommuner var anslutna till företagshälsovård. Förekomsten av rutiner för behovsinventering och uppföljning av företagshälsovårdens insatser var relaterad till lägre sjukfrånvaro.

     

    Undersökningen visar alltså att en hel del av den stora variationen i personalens hälsa mellan olika kommuner kan relateras till arbetsgivarens sätt att organisera hälso- och arbetsmiljöarbete. Kommuner med ett mer aktivt hälso- och arbetsmiljöarbete har ett bättre hälsoläge bland personalen, vilket i sin tur är relaterat till lägre sjukfrånvaro. Såväl individorienterade insatser (brett utbud av hälsofrämjande aktiviteter) som organisatoriskt stöd (lokala hälsoprojekt och resurspersoner för kompetensstöd och samordning i hälsoarbetet) har betydelse. Ett mer genomtänkt och målinriktat sätt att använda företagshälsovården har dessutom ett direkt samband med lägre sjukfrånvaro. Undersökningen visar alltså att det finns starka skäl för arbetsgivarens att tillämpa en medveten och aktiv hälsofrämjande strategi. Resultaten är i linje med aktuella teorier om hälsofrämjande arbetsplatser, men dessa har tidigare inte har kunnat styrkas genom studier av skillnader mellan många olika arbetsgivare i samma bransch.

    A comparative study of the social care organizations in a representative sample of 60 out the 290 municipalities in Sweden demonstrates significant associations between the employer’s way of organizing the work and the employees’ health and sickness absence. Specific workplace health promotion measures, as well as leadership styles and social climate are of importance. External structural regional factors such as sociodemography and labour market, as well as sickness absence culture have influence too, however without reducing the association to the internal factors concerning workplace health promotion, leadership and social climate. Accordingly, there are good reasons for employers to engage in health promotion activities both by individual-directed measures and by organizational support. The same is true for a more general developmental and supportive leadership style.
