    Abstract [en]

    Introduction

    Body awareness is a movement therapy used in Physical Therapy in Mental Health especially in Scandinavia. The method Basic Body Awareness Therapy has been scientifically investigated in particular for patients with Depression, Schizophrenia and Post Traumatic Stress Syndrome (PTSD).

    Methods

    Thirty-four Physical Therapists from 13 countries working with the Basic Body Awareness Therapy method in Mental Health Care were interviewed in six focus groups about what effects they have experienced in their work with patients. The Physical Therapists worked within the whole Mental Health spectra. Content analysis was used to analyze the informants’ experiences of the clinical effects of body awareness.

    Results

    Five categories emerged: To be in contact, Refocus and coping, Sense of Self, Relations to others and Daily life activities. The results are discussed in relation to previous research, existing theories of body awareness and cognitive neuroscience and findings of experimental psychology.

    Conclusion

    The informants experienced that Basic Body Awareness Therapy worked mainly by helping the patients to be in better contact with their “bodily self.” Stability, balance, improved grounding and the ability to relax were understood as the basis to establish an improved sense of self and leading to improved acceptance of oneself and one’s ability to relate to others.

    The ICF: International Classification of Functioning, Disability and Health (ICF) - A Swiss army knife?     Abstract [en]

    Background: The study describes the extent to which the concept of accessibility and accessibility issues has been reported in a national Scandinavian disability magazine. In this study particular attention is paid to how the compatibility between the various domains of the international classification - International Classification of Functioning, Disability and Health (ICF) - has been covered in the magazine over a decade. Furthermore, other types of descriptions that the magazine has made of accessibility for people with physical disabilities are considered. Methods: The study is based on a quantitative content analysis of a total of 326 articles from the Swedish disability magazine (SDM); 450 text pages published during the years 2000-2009. The magazine's coverage has been comprehensive. Results and conclusions: More than half of the studied texts were news items about various accessibility issues. Every third article can be characterised as a news article or feature article. The most frequently reported of the ICF domains consist of two perspectives: environmental accessibility and civil rights. Public opinion articles in the form of letters to editors and editorials focused on accessibility have a low frequency. Likewise, research reports are few. The study has included a review of illustrations and photographs. The illustrations are generally of high quality, reinforcing the disability message of the article.

    Effekter av McKenziemetoden vid behandling av ländryggssmärta klassificerad som derangement: En systematisk litteraturöversikt2021Independent thesis Basic level (professional degree), 10 credits / 15 HE creditsStudent thesis

    Bakgrund: Ländryggssmärta är ett vanligt förekommande problem hos den generella befolkningen och orsakar stora samhällskostnader. Fysioterapeuter kan behandla ländryggssmärta med olika interventioner och McKenziemetoden är en välkänd behandlingsmetod som bygger på att klassificera smärtan, derangement är den vanligaste huvudgruppen. Ett reducerbart derangement kännetecknas av att smärtan centraliseras eller minskar vid statiska positioner eller upprepade rörelser i individens riktningspreferens. Syfte: Att undersöka effekterna av McKenziemetoden på smärtintensitet och självskattad funktion vid behandling av ländryggssmärta klassificerad som derangement. Metod: Litteratursökningen gjordes i PubMed, Web of Science, Scopus, CINAHL with fulltext och SportDiscus. Randomiserade kontrollerade studier som undersökt effekten av McKenziemetoden vid ländryggssmärta klassificerad som derangement inkluderades. PEDro-skalan användes vid kvalitetsgranskning. Resultat: Totalt inkluderas sju RCT-studier, två studier uppfyllde optimal kvalitet enligt PEDro, fyra studier uppfyllde god kvalitet och en studie uppfyllde måttlig kvalitet. Två studier hade signifikant bättre effekt inom McKenziegruppen gällande både smärta och självskattad funktion. Fyra studier visade på signifikant skillnad mellan grupperna gällande smärta och fem studier visade på signifikant skillnad mellan grupperna gällande funktion, samtliga med bättre effekt för McKenziemetoden. McKenziemetoden hade bättre effekt både inom och mellan grupperna när behandlingen jämfördes med passiv elektrofysisk behandling och evidensbaserad vård. Konklusion: Det finns studier med god till optimal kvalitet som talar för att McKenziemetoden kan ha goda effekter på smärta och självskattad funktion vid ländryggssmärta klassificerad som derangement. Det krävs dock mer forskning inom området.

    Syfte: Att beskriva vilka avvikelsetyper i det motoriska utförandet som förekom mest hos spädbarn med cerebral pares (CP) vid fyra månaders ålder, samt att undersöka om dessa skiljde sig beroende på grad och typ av CP. Metod: Retrospektiv tvärsnittsstudie med kvantitativ ansats. Motoriska bedömningar med Structured Observation of Motor Performance in Infants (SOMP-I) av barn (n=66) hämtades från tre tidigare studier. Av dessa hade 50 hade en SOMP-I bedömning vid fyra månaders ålder och inkluderades i denna studie. Resultat: Vanligt förekommande avvikelsetyper i det motoriska utförandet var sidoskillnad, sparsamma rörelser, extension i armarna, knutna händer, extension i benen och plantarflekterade fötter. Förutom extension i benen i ryggläge och plantarflekterade fötter i stående, som var vanligare hos barn med svårare grad av CP, skilde sig avvikelsetyperna inte åt gällande grad eller typ av CP. Barn med unilateral spastisk CP visade inte fler sidoskillnader i det motoriska utförandet än barn med bilateral spastisk CP. Slutsats: Denna första studie som undersöker avvikelsetyper i det motoriska utförandet, visar att det finns avvikelser som är vanligt förekommande hos barn med CP, oavsett grad eller typ av CP. Dessa skulle kunna användas som tidiga indikatorer/röda flaggor på misstänkt CP, för att möjliggöra tidigare upptäckt och behandling av barn med CP, men ytterligare studier behövs för att undersöka detta vidare.

    Bakgrund: Hjärt- och kärlsjukdomar är den största folksjukdomen i Sverige. Patienter som har drabbats av hjärt- och kärlsjukdom erbjuds att delta i hjärtrehabilitering där träning ingår, vilket fysioterapeuter ansvarar för. Hjärtrehabiliteringen syftar till en beteendeförändring gällande främst fysisk aktivitet. I dagsläget finns begränsad forskning som ger en djupare förståelse för hur patienter upplever denna beteendeförändring.

    Syfte: Att studera hur patienter med hjärt- och kärlsjukdomar upplever en beteendeförändring gällande fysisk aktivitet i samband med genomförd hjärtrehabilitering.

    Metod: Studien hade en kvalitativ design med induktiv ansats. En semi-strukturerad guide användes vid intervjuer med nio patienter som deltagit i och slutfört hjärtrehabilitering. Materialet analyserades i en kvalitativ innehållsanalys.

    Resultat: Den kvalitativa innehållsanalysen utmynnade i sju kategorier. Kategorierna blev: Förhoppningar om vad hjärtrehabiliteringen skulle ge, gruppens betydelse, personalens betydelse, träningens påverkan på mående, hinder för träning under hjärtrehabilitering, ökad självsäkerhet för träning under hjärtrehabilitering och övergång till egenträning.

    Slutsats: Resultatet av denna studie visar att informanternas upplevelse av en beteendeförändring gällande fysisk aktivitet i samband med hjärtrehabilitering till största del omfattar en förändring av tankar och känslor så som minskad osäkerhet och rädsla inför fysisk aktivitet. Denna förändring har bidragit till att informanterna vågar anstränga sig efter en hjärthändelse.

    Introduktion: Det förefaller att det finns ett ökat behov av information om fysisk aktivitet och träning till gravida. Tillsammans med barnmorskor har fysioterapeuter goda möjligheter att påverka gravidas levnadsvanor och uppmuntra till en aktiv livsstil. Fysioterapeuters kontakt med gravida verkar dock variera och kan anses vara en underutnyttjad resurs i hälso- och sjukvården.

     

    Syfte: Studiens syfte var att undersöka fysioterapeuters syn på och upplevelse av att ge information om fysisk aktivitet samt träning till gravida på familje- och hälsocentraler i Umeå kommun.

     

    Metod: Semistrukturerade intervjuer utfördes och analyserades med kvalitativ innehållsanalys. Sju fysioterapeuter från sex familje- och hälsocentraler i Umeå kommun rekryterades. Fysioterapeuterna hade en spridning på 2½-30år i arbetslivserfarenhet.

     

    Resultat: Analysen resulterade 13 underkategorier som sammanställdes i fem kategorier; Fysisk aktivitet och smärta är fysioterapeuters arena, En stöttande, individanpassad och rådgivande roll, Ett arbete med många utmaningar, Hälso- och sjukvårdens struktur påverkar Fysioterapeuters arbete och Ambition att med små medel göra stor skillnad.

     

    Slutsats: Fysioterapeuterna upplever att möjligheten att samarbeta med barnmorskor, vid information till gravida om fysisk aktivitet och träning, påverkas av familje- och hälsocentralens storlek samt tidsmässiga-, kollegiala- och ekonomiska resurser. På familje- och hälsocentraler i Umeå kommun finns inget självklart samarbete mellan fysioterapeuter och barnmorskor. Fysioterapeuterna har inte en självklar roll i arbetet med gravida även om de upplever sig vara trygga i att informera om och har goda kunskaper om fysisk aktivitet samt träning under graviditet. Tydligare riktlinjer om fysisk aktivitet och träning under graviditet skulle kunna underlätta för hälso- och sjukvårdspersonal att informera gravida. Fysioterapeutiska insatser i form av träningsgrupper, informationstillfällen, samtalsstöd, uppföljningar och ett samarbete med barnmorskor i föräldragrupper skulle kunna förbättra gravidvården, både under och efter graviditet.

    Background: Injuries in sports are common, not least in football. Many studies focus on the physiological aspect of rehabilitation. However, it has been shown that social support is important for successful injury rehabilitation. Furthermore, there is a lack of research on the perceived support of football players and how they experience the support when returning to sport.

    Purpose: To examine football players’ experience of social support during rehabilitation and how the absence due to injury affects their perceived status position in the team.

    Method and design: Qualitative explorative interview study. Data was collected with semi-structured interviews with 2 male and 2 female football players in Uppsala. The interviews were analyzed through a qualitative content analysis.

    Result: The informants felt that they received different kinds of social support from different people. The support depended on the role and relationship the person in question had with the informant. Everyone felt that the support during a long-term rehabilitation naturally changed over time, from comforting and intense at first to a more decreased form as the informants became more accepting of the situation. Upon return, they felt that their status position in the team had changed as a result of the long absence. This was generally perceived as a natural process where they felt they needed to work their way back to earn the previously experienced status in the team.                                                                                                                                    

    Conclusion: Based on the results of the study, it can be considered important that football players receive different kinds of support from different key persons to keep up the motivation during a long-term rehabilitation. It is also perceived important that football players receive continued support when they return to sport, in order to make them feel welcome and to increase their confidence in their own ability to return to previous levels of performance.

    ABSTRACT

    Introduction: The Swedish Child Health Service (CHS) works to promote children’s health and aims to identify those with additional needs. Guidelines regarding a child’s motor development as they age are clear; however, the tools used to make that assessment are not standardized. When assessing a child, the parents input is integral. Despite this, the Swedish CHS does not have insight or knowledge into how parents are experiencing these assessments. 

     

    Aim: To investigate parents’ experience of the assessment process of motor development made by the Swedish CHS during the regular health visits. To determine what kind of support parents have received, or would like to have received, during the first 18 months of their child’s life.

     

    Method: Semi-structured interviews were conducted with 12 parents whose children had been referred to a pediatric physiotherapist. The data was analyzed using a systematic text condensation.

     

    Results: Parents described a lack of consensus regarding the motor development assessment process. They perceived the health visit to mainly concern the child’s growth as well as their wellbeing and that of the parents. The subject of motor development was not discussed to a point where parents felt satisfied, resulting in the parents often seeking further information through the internet, as well as with other family and friends. 

     

    Conclusion: Parents perceive a lack of information regarding the motor development of their child during health visits. They wish to have better support and information regarding motor skill development by the Swedish CHS.

    Under de senaste åren har forskning presenterats som påvisar en försämring i balans, knä- och fotledsstabilitet under kvinnors ägglossningsfas, vilket föreslås kan medföra en ökad skaderisk under deras idrottsutövande. Under ägglossningen är nivåerna av könshormonet östrogen som högst. Östrogenet påverkar enzymet lysyloxidas som hämmar kollagensyntesen i senor och ligament. Detta medför möjligen en ökad instabilitet i exempelvis knä- och fotled under ägglossningen. 

    Syftet med studien var att undersöka hur vanligt förekommande det är att idrottande fertila kvinnor anger en försämrad balans eller ökad skadebenägenhet under ägglossningsfasen. 

    Studien var en retrospektiv observationsstudie med kvantitativ metod. En enkät skickades ut till föreningsidrottande kvinnor som tränade minst tre gånger i veckan. De kvinnor som använde hormonella preventivmedel exkluderades från deltagande. Totalt deltog 17 kvinnor i studien.

    Resultatet visade att majoriteten av kvinnorna visste när de hade ägglossning men inte angav några förändringar i knä- och fotledsstabilitet, balans och ökad skadebenägenhet under denna fas. Dock angav nästan en fjärdedel helt eller delvis en försämrad balans under ägglossningen och strax över en fjärdedel angav en ökad skadebenägenhet under ägglossningen.

    Inga tydliga slutsatser kan dras men förändringar i balans och skadebenägenhet under ägglossningsfasen kan i vissa fall förekomma hos idrottande kvinnor och bör då tas i beaktning vid övningar där höga krav ställs på balans eller där ökad risk för skada finns. Fortsatt forskning rekommenderas.

    Background

    Urinary incontinence is one of the most common chronic health issues in society where few seek care meaning a decreased quality of life. Research regarding how exercise affects urinary incontinence varies. 

    Purpose

    On one occasion examine the prevalence of stress urinary incontinence (SUI) in physically active (PA) nulliparous in total and divided by activity, and the correlation between perceived level of discomfort of urinary incontinence (UI) and volume of moderate to high activity level amongst PA nulliparous with SUI.

    Method

    Quantitative observational study conducted via a web-based survey based on IPAQ-SF and PFIQ-7. PA women, age 16-55, who were born with a female gender, hadn’t given birth or been pregnant after week 13 and weren’t elite athletes were included. 

    Result

    215 women responded to the survey, 61 were excluded. Of 154 participants 37 women experienced SUI (prevalence 24%). Strength training (prevalence 18%) and running (prevalence 26%) were the most common activities. High-intensity training had a prevalence of 31–38%. Low intensity training had a prevalence of 19%. No correlation between experienced level of discomfort of UI and volume of moderate to high level of physical activity amongst women with SUI could be revealed. (r = -0,0489, p = 0,8).  

    Conclusion 

    SUI is common amongst exercising women without known risk factors. It is more common with SUI during high-intensity activities compared with low-intensity. No correlation was demonstrated between UI amongst women with SUI and activity level. Research regarding exercise as risk factors and treatment for UI is required.

    Quantitative observational study conducted via a web-based survey based on IPAQ-SF and PFIQ-7. PA women, age 16-55, who were born with a female gender, hadn’t given birth or been pregnant after week 13 and weren’t elite athletes were included. 

    Result

    215 women responded to the survey, 61 were excluded. Of 154 participants 37 women experienced SUI (prevalence 24%). Strength training (prevalence 18%) and running (prevalence 26%) were the most common activities. High-intensity training had a prevalence of 31–38%. Low intensity training had a prevalence of 19%. No correlation between experienced level of discomfort of UI and volume of moderate to high level of physical activity amongst women with SUI could be revealed. (r = -0,0489, p = 0,8).  

    Conclusion

    SUI is common amongst exercising women without known risk factors. It is more common with SUI during high-intensity activities compared with low-intensity. No correlation was demonstrated between UI amongst women with SUI and activity level. Research regarding exercise as risk factors and treatment for UI is required.

    Abstract [en]

    Objectives:

    The aims of this study were to evaluate the incidence of sagittal malalignment including kyphosis following cervical laminectomy without fusion as treatment for cervical spondylotic myelopathy and to assess any correlation between malalignment and clinical outcome.

    Study Design:

    Retrospective cohort study.

    Methods:

    In all, 60 patients were followed up with conventional radiography at an average of 8 years postoperatively. The cervical lordosis (C2-C7 Cobb angle), C2-C7 sagittal vertical axis (cSVA) and C7 slope were measured on both preoperative and postoperative images. Patients completed a questionnaire covering Neck Disability Index (NDI), visual analogue scale for neck pain, and general health (EQ-5D).

    Results:

    Mean C2-C7 Cobb angle was 8.6° (SD 9.0) preoperatively, 3.4° (10.7) postoperatively and 9.6° (14.5) at follow-up. Ultimately, 3 patients showed >20° cervical kyphosis. Mean cSVA was 16.3 mm (SD 10.2) preoperatively, 20.6 mm (11.8) postoperatively, and 31.6 mm (11.8) at follow-up. Mean C7 slope was 20.4° (SD 8.9) preoperatively, 18.4° (9.4) postoperatively, and 32.6° (10.2) at follow-up. The preoperative to follow-up increase in cSVA and C7 slope was statistically significant (both P < .0001), but not for cervical lordosis. The preoperative to follow-up change in cSVA correlated moderately with preoperative cSVA (r = 0.43, P = .002), as did the corresponding findings regarding C7 slope (r = 0.52, P = .0001). A comparison of radiographic measurements with clinical outcome showed no strong correlations.

    Conclusions:

    No preoperative to follow-up change in cervical lordosis was found in this group; 5.0% developed >20° kyphosis. No clear correlation between sagittal alignment and clinical outcome was shown.

    Abstract [en]

    Dysfunctional top-down pain modulation is a hallmark of fibromyalgia (FM) and physical exercise is a cornerstone in FM treatment. The aim of this study was to explore the effects of a 15-week intervention of strengthening exercises, twice per week, supervised by a physiotherapist, on exercise-induced hypoalgesia (EIH) and cerebral pain processing in FM patients and healthy controls (HC). FM patients (n = 59) and HC (n = 39) who completed the exercise intervention as part of a multicenter study were examined at baseline and following the intervention. Following the exercise intervention, FM patients reported a reduction of pain intensity, fibromyalgia severity and depression. Reduced EIH was seen in FM patients compared to HC at baseline and no improvement of EIH was seen following the 15-week resistance exercise intervention in either group. Furthermore, a subsample (Stockholm site: FM n = 18; HC n = 19) was also examined with functional magnetic resonance imaging (fMRI) during subjectively calibrated thumbnail pressure pain stimulations at baseline and following intervention. A significant main effect of exercise (post > pre) was observed both in FM patients and HC, in pain-related brain activation within left dorsolateral prefrontal cortex and caudate, as well as increased functional connectivity between caudate and occipital lobe bordering cerebellum (driven by the FM patients). In conclusion, the results indicate that 15-week resistance exercise affect pain-related processing within the cortico-striatal-occipital networks (involved in motor control and cognition), rather than directly influencing top-down descending pain inhibition. In alignment with this, exercise-induced hypoalgesia remained unaltered.

    BACKGROUND: Mortality rates among immigrant patients undergoing rehabilitation for musculoskeletal backache are unknown.

    OBJECTIVE: To study the association between marital status, severe psychosocial strain, receiving long-term time-limited sickness allowance (TLSA) and all-cause mortality (ACM) in a cohort of immigrants aged 20-45 years with long-standing backache in Sweden.

    METHODS: We studied 318 patients (92% foreign-born, 76% non-European) of known marital status on sick-leave for musculoskeletal backache. They were followed up for ACM until 2015. Socio-demographic data, TLSA and psychosocial strain, including major depression, severe psychosocial stressors and pessimistic thoughts, were analysed using multiple-imputation Cox regression.

    RESULTS: Over a mean (standard deviation) follow-up time of 15 (5.0) years, 11 (3.5%) participants died. At baseline, 34% were unmarried, 19% were receiving TLSA, and 71% had ⩾ 1 psychosocial strain component (38% depression; 47% severe stressors; 35% pessimistic thoughts). After concomitant risk factors were adjusted for, being unmarried and receiving TLSA were associated with higher mortality by factors of 6.2 (p= 0.005) and 5.8 (p= 0.006), respectively. Psychosocial strain was only significantly associated with higher mortality in the unadjusted analyses.

    CONCLUSIONS: Being unmarried and receiving TLSA were associated with significantly higher ACM in this highly marginalized group of immigrant patients.

    While the effects of lifestyle habits on hypertension or type 2 diabetes mellitus (T2DM) are well established, few lifestyle programmes in primary care directed towards patients with high cardiovascular risk have been evaluated.

    Aims To describe and elaborate on how participation in a 1-year lifestyle programme supported changes in lifestyle habits and altered the risk of cardiovascular disease (CVD) as well as explore patients’ and community health nurses’ (CHNs’) experiences of lifestyle change and counselling.

    Methods The 1-year lifestyle programme consisted of five appointments with a CHN for patients diagnosed with hypertension, T2DM or impaired glucose tolerance (n = 448). Focuse was on lifestyle habits that were related to patients’ diagnosis. Different behaviour change techniques were used to support lifestyle changes. Blood sampling and anthropometrical measurements were obtained at baseline and 1-year follow-up. The design of Studies I and II was observational and based on data that were consecutively collected between 2009 and 2014, whereas Studies III and IV had a qualitative design. Qualitative content analyses were performed based on data from individual interviews with patients (n = 16) and a focus group interview of CHNs (n = 3).

    Results Study I: Favourable changes in physical activity, dietary habits and smoking were detected after participation in the programme. Study II: Significant improvements were demonstrated for all cardiovascular risk factors and the estimated 10-year CVD risk after participation in the programme. Study III: Patients’ experiences of lifestyle changes indicated that increased knowledge of lifestyle habits, gaining trust in oneself and support from others were important elements in the adoption of lifestyle changes. Study IV: The informants expressed that counselling should be based on a partnership, include goal setting and repeated measurements, and incorporate long-term support after the completion of the lifestyle programme.

    Conclusion This thesis adds to the knowledge on how lifestyle counselling can be designed and implemented in primary care. The findings show that patients with a new diagnosis of hypertension or T2DM are at high risk for future CVD and a structured lifestyle programme can contribute to improved lifestyle habits and a reduced 10-year CVD risk.

    ObjectivesTo examine the effect of a high-intensity functional exercise intervention on cognitive function in older people with dementia and depression living in nursing homes. 

    Secondary aim was to assess the association between depressive symptoms and cognitive function.

    MethodsParticipants (n= 186) were included from the Umeå Dementia and Exercise (UMDEX) study who were randomized to a high-intensity functional exercise (HIFE) intervention group or a control activity group over four months. Differences between groups was examined for changes in Mini-Mental State Examination at follow-up and baseline (MMSE-diff) using Independent sample t-test. Association was examined between changes in the 15-item Geriatric Depression Scale between follow-up and baseline (GDS-diff) and MMSE-diff using Pearson’s correlation coefficient. Analysis for differences and associations were performed according to intervention group or presence of depression. 

    ResultsNo significant difference was found in MMSE-diff between depression or interventions. Significant correlation between changes in MMSE-diff and GDS-diff was found in the control group (correlation coefficient -0.224, p-value 0.046), but not in any other group.

    ConclusionsThere was no significant difference of effect on cognition (MMSE) when comparing type of activity or state of depression. Changes in depressive symptoms were associated with changes in cognition in the control activity group, but not in the exercise intervention. Older people with dementia and depression can benefit from exercise, however in this group of older people with dementia living in nursing homes exercise didn’t significantly affect cognition. Further research is needed to examine the effects of longer interventions and aerobic exercise in this group.

    Background: To be physical active is important. The risk to sustain a knee injury during physical activity is dependent on the choice of activity and the number of hours the activity is exercised. Activities and sports that includes acceleration, deceleration, pivoting, jump and landing brings more load to the knee joint. In evaluation of a knee injury it is today common to use different kinds of patient-reported outcomes, PRO. Tegner Activity Scale, TAS, is one of the most common questionnaire about sport participation today. Several of today common activities and sports are missing in TAS. Also some of the activities and sports with high incidence of knee injury is missing. Today there is no knee related activity scale that includes the activities and sports with the highest incidence of knee injury and the most common exerted activities and sports in Sweden. The need for a new knee related activity scale is big.

    Aim: The aim of this study was to develop a new content valid knee related activity scale in PRO form.

    Method: The study was conducted in two steps. First a new knee related activity scale with TAS as basis was made. TAS was then compared with statistics over participation in activities/sports from Riksidrotts Förbundet, RF, and from The Swedish National Knee Ligament Register. This to ensure that the most common activities and the activities with high risk for knee injury was in the new scale. Experts: trainers, actives and physiotherapists, with experience from each of the specific activities and sports were then asked to place their specific activity/sport in the new scale based on their belief of load to the knee joint.

    Results: The experts response was analysed, the activity scale was corrected and then the new scale, Knee Activity Scale, KAS, was completed. KAS contains 50 activities/sports compared with 22 in TAS. The division in competitive and recreational level from TAS was removed as well as the work-related activities. Four activities/sports was found on a higher level in KAS compered to TAS.

    Conclusion: A new knee related activity scale in PRO form was designed.

Det finns dock få studier som beskriver upplevelser av träningen. Syftet med denna studie var att beskriva upplevelser av sjukgymnastledd gruppträning hos individer med Parkinson och undersöka hur resultatet överensstämde med domänerna i Internationella klassifikationen av funktionstillstånd, funktionshinder och hälsa (ICF). En kvalitativ deskriptiv innehållsanalys tillämpades. Fem deltagare medverkade i studien. En intervjuguide sammanställdes, intervjuerna transkriberades och tolkades. Slutligen sammanställdes resultatet i en tabell med kategorier, subkategorier och koder. Ytterligare en tabell utformades för att redovisa resultatet enligt ICF:s domäner. Det framkom i studien att en sjukdomsspecifik grupp möjliggjorde en trygghet och en social samhörighet. Vidare placerades majoriteten av resultatet inom ICF:s domäner aktivitet/delaktighet samt omgivningsfaktorer, vilket belyste att de psykosociala faktorerna föreföll prägla deltagarnas uppfattningar i större omfattning än de fysiska.

    Background: Ménières disease is a complex neurological disease that varies greatly from individual to individual with symptoms such as dizziness and tinnitus. There are few studies that examines the differences regarding balance and dizziness related perception of disabilitiy between men and women with Ménière’s disease.

    Purpose: Investigate self-assessed perception of disability and balance of people with Ménières disease. The study also aimed to investigate whether there were any differences between men and women regarding the above mentioned variables.

    Method: Retrospective study with quantitative, correlative and comparative design. 41 subjects, 21 women, 20 men were included in the study.

    Results: No significant difference between men and women regarding scores on DHI-S or result on Romberg’s test could be shown in this study. Neither could any significant connection between self-assessed perception of disability and balance measured with DHI-S and Romberg’s respectively could be made.

    Conclusion: No relation between self-assessed disability and balance was detected between men and women. The study also did not find any differences between the sexes on how they perform on balance measurement or rate their disability. More studies are needed to further investigate and map this area.

    Aims

    The aim of this study was to conclude what effects, both positive and negative, physical activity can have for people in rehabilitation. The following questions were used: What background have the different pain states? What form of physical activity and the duration, frequency and intensity of use? How are the people in question affected by the physical activity on the pain state - positive / negative effects?

    Methods

    The study is based on five interviews with women in rehabilitation, in the form of physiotherapy. I thought, this way could help me to find out however people can be affected, positively or negatively, by physical activity at various physical pain. The answers from the interviews was then compiled and compared with previous researched and other material. Literature studies have also derived background information and for increased knowledge in the subject.

    Results

    The results showed that the interviewed women who go to rehabilitation, feel improved physical capacity and decreased pain during exercise, irrespective of the different painstates and painfactors for each interviewee. The current pain permits are one workrelated injury, accident injury, sports injury, overworked and congenital disabilities. All interviewees are in rehabilitation for an indefinite period of time - it depends on how their body continues to respond to exercise and how you feel effects of endurance training. Everyone feel, so far, that the exercise has positive effects, provided that you only train your own potential and that you are not training too hard, resulting in a backlash of negative effects. Interviewees also shared that they experience positive effects in maintenance training – if they train regularly on a comfortable level and not take long breaks in their training. Training paus demonstrated a perceived increased pain and less mobility, than in training periods.

    Conclusion

    Physical activity in rehabilitation showed positive effects as long workout was individualized and look at the prevailing pain. Regularity train in combination with a moderate level of frequency, duration and intensity revealed optimal positive effects.

    Bakgrund: Plantar fasciit är ett inflammatoriskt tillstånd i foten som uppstår till följd av mikrotrauman på senstråket som går under foten. Tillståndet oftast självläkande, men kan vara ihållande. Kan vara mycket smärtsamt och begränsande. Behandlingsmetoder som finns är stretching, tejpning, nattskena och fysioterapi. Stötvågsbehandling är behandlingsmetod som används vid stötvågsbehandling, anses vara en säker metod. Syfte: Kartlägga vilken effekt stötvågsbehandlingen hade på plantar fasciit avseende smärta och/eller funktion. Metod: För att besvara syftet utfördes en litteraturstudie. Sökningarna utfördes i två sökbaser, LTU:s sökbas och Pubmed. Resultat: 8 studier inkluderades och granskades. Stötvågsbehandling jämfördes med placebobehandling. Majoriteten avstudierna (fem av åtta) uppvisade goda resultat med statistiskt signifikans till fördel för stötvåg. Två av studierna uppvisade fördel till stötvåg, men det fanns ingen statistisk signifikans i studierna. Sista studien uppvisade ingen signifikant skillnad eller fördel till någon av grupperna. Stötvågsbehandling med medel- eller högeffekt uppvisade bättre resultat än lågeffekt i denna litteraturstudie. Radiell stötvåg hade mer tillförlitligt resultat än fokuserad stötvåg i denna sammanställning med. Konklusion: Stötvåg verkar kunna ha en positiv effekt på plantar fasciit avseende smärta och funktion. Stötvåg med medel- eller högeffekt är två alternativ för effekt. Radiell stötvåg kan även vara att föredra över fokuserad då den gav tillförlitligare resultat.

    Background: Physical activity (PA) provides health benefits for elderly. They are recommended to be physically active to a moderate level of exertion for 150-300 minutes a week. Träffpunkter 65+ in Uppsala municipality offers PA through group training, where the percentage of men participating is low. Value lies in finding what would motivate the men to participate, in order to attract inactive men.

    Purpose: To investigate older men's requests for and motivators to participate in group training at Träffpunkter 65+ and if there are differences between men who achieve recommendations for PA and those who do not.

    Method: A quantitative non-experimental cross-sectional study in the form of a survey was conducted and shared digitally through pensioner associations to men over 65 years.

    Results: Eighty-six men participated (median age 74.5 years, 50 respondents achieve PA). Group training with mobility, balance and cardiovascular fitness was requested, no differences were seen between the men who achieve and do not achieve PA. Motivating factors were: "offers greater physical challenge", "healthcare contact highlights benefits of training" and "training with a friend or relative". "Offering greater physical challenge" and "working towards a common goal" were significantly more motivating for men who achieve PA.

    Conclusion: Men preferred group training targeting body function. Motivators to participate included health benefits, group fellowship and physical challenge. Men who achieved PA were to a greater extent motivated by group training challenging physical capacity. Further studies on the target group are required.

    Abstract [en]

    This qualitative study investigated what factors influenced older adults to start and continue to exercise in a senior gym. Eight individuals, ages 65–81, were interviewed. The interviews were analyzed by manifest content analysis. Exercising at their own pace and finding peers, competent staff, and accessible machines were all positive factors for continuing to exercise in a senior gym. Initially, training was something to do to take control of life, physically or time-wise. Later, training became a health investment and a social activity. The found factors can be used as strategies for changing physical activity behavior in older adults.

    Unexpected pulling and pushing loads exerted by an object held with a precision grip evoke automatic and graded increases in the grip force (normal to the grip surfaces) that prevent escape of the object; unloading elicits a decrease in grip force. Anesthesia of the digital nerves has shown that these grip reactions depend on sensory signals from the digits. In the present study we assessed the capacity of tactile afferents from the digits to trigger and scale the evoked grip responses. Using tungsten microelectrodes inserted percutaneously into the median nerve of awake human subjects, unitary recordings were made from ten FA I and 13 FA II rapidly adapting afferents, and 12 SA I and 18 SA II slowly adapting afferents. While the subject held a manipulandum between a finger and the thumb, tangential load forces were applied to the receptor-bearing digit (index, middle, or ring finger or thumb) as trapezoidal load-force profiles with a plateau amplitude of 0.5-2.0 N and rates of loading and unloading at 2-8 N/s, or as "step-loads" of 0.5 N delivered at 32 N/s. Such load trials were delivered in both the distal (pulling) and proximal (pushing) direction. FA I afferents responded consistently to the load forces, being recruited during the loading and unloading phases. During the loading ramp the ensemble discharge of the FA I afferents reflected the first time-derivative of the load force (i.e., the load-force rate). These afferents were relatively insensitive to the subject's grip force responses. However, high static finger forces appeared to suppress excitation of these afferents during the unloading phase. The FA II afferents were largely insensitive to the load trials: only with the step-loads did some afferents respond. Both classes of SA afferents were sensitive to load force and grip force, and discharge rates were graded by the rate of loading. The firing of the SA I afferents appeared to be relatively more influenced by the subject's grip-force response than the discharge of the SA II afferents, which were more influenced by the load-force stimulus. The direction in which the tangential load force was applied to the skin influenced the firing of most afferents and in particular the SA II afferents. Individual afferents within each class (except for the FA IIs) responded to the loading ramp before the onset of the subject's grip response and may thus be responsible for initiating the automatic increase in grip force. However, nearly half of the FA I afferents recruited by the load trials responded to the loading phase early enough to trigger the subject's grip-force response, whereas only ca. one-fifth of the SA Is and SA IIs did so. These observations, together with the high density of FA I receptors in the digits, might place the FA I afferents in a unique position to convey the information required to initiate and scale the reactive grip-force responses to the imposed load forces.

    OBJECTIVES: To assess the physical and psychological effects across 11 weeks of music-exercise sessions, the participants' training experience, and attitudes towards physical activity. The effect of different music information was also investigated.

    METHODS: Overall, 146 sedentary volunteers were randomized into 4 exercise groups and each group received different music information. Physical capacity and psychological measures were obtained.

    RESULTS: Increased performance in oxygen uptake and flexibility and decreased blood pressure was found. Participants reported increased wellbeing and body-awareness, and an intention to remain physically active. No differences between groups were found.

    CONCLUSION: Music-exercise can be recommended to promote physical activity among sedentary individuals. The amount of musical information in synchronous music seems not to have any effects on self-selected intensity or physiological benefits.

    Bakgrund: Förkortad hamstringmuskulatur är vanligt bland fysiskt aktiva. Inadekvat hamstringmuskellängd och ökad styvhet i hamstringsmuskulatur är möjliga orsaker till skador i hamstring. Den senaste tiden har det blivit allt vanligare med användning av foamroller till syfte att behandla dessa förkortningar och styvhet. 

    Syfte: Syftet med denna studie är undersöka om användning av foamroller har någon direkt påverkan på töjbarheten och smärttolerans i hamstringsmuskulatur efter ett behandlingstillfälle.

    Metod: En experimentell studie utfördes där tio friska fysiskt aktiva män (ålder 25,4 ± 3 år, längd 183 ± 9,5 cm, vikt 78,5 ± 7,2 kg) med självrapporterad nedsatt rörlighet baksida lår rekryterades och genomgick två kortare behandlingar (3 x 30 sekunder), med foamroller och statisk stretch på deras dominanta ben (det ben dom sparkar boll med), vid två separata tillfällen. Mätningar av passiv rörlighet i knäled, styvhet i hamstringmuskulatur och smärttolerans på biceps femoris mest ömmande punkt utfördes före och direkt efter varje testtillfälle. Data sammanställdes och analyserades i IBM SPSS för skillnader före och efter gällande förändringar på rörlighet, styvhet och smärttolerans.

    Resultat: Små skillnader kunde observeras mellan behandlingarna.  Vid statisk stretch noterades en viss skillnad i styvhet på -0,76 Nm. I båda behandlingarna noterades en ytterst liten ökning i rörlighet på hamstringsmuskulaturen med större förändring i statisk stretch, +0,1 grader.  Smärttröskeln var lägre efter båda behandlingar, -125,5 kPa efter foamroller respektive -99,3 kPa efter statisk stretch. Dock visade inte resultatet på någon statistiskt signifikant skillnad i påverkan av passiv ledrörlighet, uppmät styvhet i hamstring eller smärttröskel.  

    Konklussion: Studien var liten och undersökte endast korttidseffekter, men resultatet talar för att det inte finns någon reell påverkan på biceps femoris styvhet eller smärttolerans med användning av foamroller.

    Att barn är aktiva inom olika idrotter i föreningar är främjande för deras hälsa. Om träning sker i samband med infektion kan dock negativa hälsoeffekter uppstå. Studier visar att träning vid infektion ej ska bedrivas.

    Syfte: Syftet med studien var att undersöka synen på träning vid infektion bland idrottande barn, deras målsmän samt tränare. Metod: Studien gjordes genom utskick av eget utformade enkäter via programmet Google Docs till idrottande barn, deras målsmän samt tränare i idrottsföreningen KFUM i Umeå. Resultat: Bortfallet i de tre grupperna idrottande barn, deras målsmän samt tränare var så pass stort att ingen statistisk analys av svaren kunde göras. Resultatet beskrivs därför endast deskriptivt. Konklusion: Att undersöka synen på träning vid infektion bland idrottande barn, deras målsmän samt tränare genom en enkätstudie har inneburit flera svårigheter. Enkätstudier som berör ämnen som kan anses vara känsliga kräver en väl genomförd planering av metod och upplägg för att kunna undersökas på bästa sätt. Om dessa metodologiska aspekter kan uppnås finns möjligheten att undersöka synen på träning vid infektion vidare.

    Results: The results show that physical activity neither was an issue in focus at any of the assisted living facilities, nor were recommendations on physical activity followed. Individuals that were able to exercise themselves could do so, whereas those in need of assistance had but limited possibilities to be physically active. There was a need for physical activity that the staff do not necessarily and sufficiently identify.

    Conclusion: The study illustrated that there were major variations in how older people engaged in physical activity and how physical activities were part of everyday life. Physiotherapists played no clear role at the facilities, especially with regard to preventive exercise. Older individuals were not involved in determining which activities should be made available to the residents.

    Introduktion: Träning lindrar fysiska och psykiska symtom under menstruation och PMS. Premenstruella symtom (PMS) är ett symtomkomplex som uppstår före menstruationsblödning. Menstruation är den månatliga blödningen som sker hos fertila kvinnor. Syfte: Syftet med studien var att undersöka om kvinnor förändrar träningsvolym under PMS och menstruation samt undersöka om träningsprestationen förändras under PMS och menstruation. Metod: En enkät utformades efter syftet och skickades via mail till 314 studenter på arbetsterapeutprogrammet och fysioterapeutprogrammet på Luleå tekniska universitetet. Biologiska kvinnor som menstruerar inkluderades i studien. Resultat: Totalt svarade 33 personer på enkäten. Samtliga var biologiska kvinnor som menstruerade. Av försökspersonerna angav 27% förändring av träningsvolym under PMS och 39% angav förändring av träningsvolym under menstruation. Ungefär hälften (52%) uppgav att de upplevde en negativ påverkan på träningsprestationen under menstruation och 39% upplevde en negativ påverkan på träningsprestationen under PMS. Konklusion: Förändring av träningsvolym och påverkan på träningsprestationen är väldigt individuellt. Fler studier behövs på både motionärer och elitidrottande kvinnor.

    Introduktion: Psykologisk stress är ett sympatiskt påslag som imiterar en flyktreaktion i kroppens nervsystem genom utsöndring av adrenalin, kortisol och noradrenalin i blodet– detta bidrar i sin tur med förhöjt blodtryck och muskelspänningar i kroppen som kan ligga till grund bakom ett flertal sjukdomar. Syfte: Syftet var att undersöka om det fanns signifikanta skillnader i stressnivåer mellan svenska kvinnliga konståkare och svenska kvinnor som inte utövade någon idrott, samt att undersöka om åldern 18–21 år, respektive 22–26 år kunde ge ett annorlunda resultat i den upplevda stressen mellan grupperna. Metod: Genom användning av verktyget "PSS-14" genomfördes en online enkätundersökning inom Sverige där den uppskattade stressen under den senaste månaden mättes hos kvinnor. Studien bestod av 100 svenska kvinnliga deltagare, 18–26 år. 51 deltagare var svenska kvinnliga konståkare och 49 deltagare var kvinnor som inte var medlemmar i en idrottsförening. Resultat: Medelvärdet i PSS–14 för totala andelen kvinnliga konståkarna var 27,92 poäng ± 8,57, och 27.04 poäng ± 7,68 för kvinnor som inte var medlemmar i en svensk idrottsförening. Inga signifikanta skillnader upptäcktes mellan gruppen konståkare och icke konståkare, åldrarna för undergrupperna visade sig även sakna en statistisk signifikans efter ett parametriskt t-test. Konklusion: Samtliga grupper indikerade ett medelvärde som var högre än tidigare studier efter ha besvarat enkäten PSS–14. Det upptäcktes dock inga signifikanta skillnader i stressnivåer mellan någon av grupperna. Däremot visades det att icke idrottande kvinnor 22–26 år indikerade en lägre upplevd stressnivå än de övriga grupperna, dock inte signifikant.

    Introduktion: Fysisk aktivitet har stor betydelse för ett hälsosamt åldrande genom dess hälsofrämjande effekter. För äldre individer rekommenderas minst 150 minuters fysisk aktivitet per vecka. En inaktiv livsstil ökar risken för flertalet sjukdomar och funktionsnedsättande åkommor, vilket fysisk aktivitet minskar risken för. En kartläggning av äldres fysiska aktivitet är användbart för fysioterapeuters dagliga arbete samt utifrån ett folkhälsoperspektiv för att se den nuvarande statusen. Syfte: Syftet med studien var att kartlägga den fysiska aktivitetsnivån hos personer över 70 år i Luleå kommun. Metod: Data baserad på IPAQ-E från 45 individer som insamlats i en tidigare studie användes för att besvara denna studies syfte. Analys av data och beräkningar av deltagarnas totala fysiska aktivitet genomfördes i Microsoft Excel. Deltagarna kategoriserades i aktivitetsnivåerna låg, måttlig eller hög baserat på deras totala fysiska aktivitet. Resultat: 64% av deltagarna uppnådde rekommendationerna för fysisk aktivitet. 36% av deltagarna kategoriserades i låg aktivitetsnivå 22% i måttlig aktivitetsnivå och 42% i hög aktivitetsnivå, högre andel kvinnor än män uppnådde hög aktivitetsnivå. Deltagarna som uppnådde rekommendationerna var mer aktiva i samtliga ansträngningsnivåer i jämförelse med de som ej nådde upp till dessa. Ett svagt negativt samband framkom mellan ålder och total fysisk aktivitet. Konklusion: Majoriteten av deltagarna uppnådde rekommendationerna för fysisk aktivitet. Kartläggningen kan bidra med ökad kunskap och förståelse för fysioterapeuter. Kartläggningen bidrar även med kunskap ur ett folkhälsoperspektiv och kan utgöra ett verktyg för det fortsatta arbetet med äldres hälsa.

    Abstract [en]

    Stress-related exhaustion has been associated with selective and enduring cognitive impairments. However, little is known about how to address cognitive deficits in stress rehabilitation and how this influences stress recovery over time. The aim of this open-label, parallel randomized controlled trial (ClinicalTrials.gov: NCT03073772) was to investigate the long-term effects of 12 weeks cognitive or aerobic training on cognitive function, psychological health, and work ability for patients diagnosed with exhaustion disorder (ED). One-hundred-and-thirty-two patients (111 women) participating in multimodal stress rehabilitation were randomized to receive additional cognitive training (n = 44), additional aerobic training (n = 47), or no additional training (n = 41). Treatment effects were assessed before, immediately after and one-year post intervention. The primary outcome was global cognitive function. Secondary outcomes included domain-specific cognition, self-reported burnout, depression, anxiety, fatigue and work ability, aerobic capacity, and sick-leave levels. Intention-to-treat analysis revealed a small but lasting improvement in global cognitive functioning for the cognitive training group, paralleled by a large improvement on a trained updating task. The aerobic training group showed improvements in aerobic capacity and episodic memory immediately after training, but no long-term benefits. General improvements in psychological health and work ability were observed, with no difference between interventional groups. Our findings suggest that cognitive training may be a viable method to address cognitive impairments for patients with ED, whereas the effects of aerobic exercise on cognition may be more limited when performed during a restricted time period. The implications for clinical practice in supporting patients with ED to adhere to treatment are discussed

    Abstract [en]

    Background: Obstructive sleep apnea (OSA) is obstruction of airflow during sleep leading to respiratory arrest. OSA is most common with obesity and among men. Individuals with OSA have a higher depression rate than the rest of the population. Physical activity (PA) has proven to be an effective treatment for depression.

    Aim: To investigate whether change in PA correlates with change in self-assessed depressive symptoms and if there are differences regarding gender or intensity levels.

    Method: A retrospective, descriptive, correlative and comparative study. Data was obtained from a previous study. PA and self-assessed depressive symptoms were measured with SenseWear Pro3 Armband and MADRS-S at baseline and at 6 months follow-up.

    Results: There was a correlation between an increase of PA and a decrease of MADRS-S score, r = -0.27, p = 0.04. The correlation for men was stronger r = -0.35 (p = 0.02). Women had a correlation between an increase in PA and an increase of MADRS-S score, r = 0.7 not statistically significant, p = 0.16. There was no difference in MADRS-S score between the groups performing PA at moderate intensity 5 (5.5) median (interquartile), respectively high intensity 4.5 (9.25), p=0.68

    Conclusion: The results showed a correlation between an increase in PA and a reduction of MADRS-S score. Regarding differences between genders or intensity levels the low number of women and physically active at high intensity meant no conclusions could be drawn.

    Abstract [en]

    Patients with non-specific musculoskeletal disorders are considered to be a heterogeneous group with multifactorial problems. Appropriate and effective treatments for these patients have been hard to find. The purpose of this study was to identify subgroups of patients with different patterns of treatment outcome and to find possible predictors of outcome. A total of 78 patients, 64 females and 14 males, with non-specific musculoskeletal disorders were recruited consecutively to three different treatment approaches, Body Awareness Therapy, Feldenkrais, and conventional, individual physiotherapy in primary care. The outcome variables were calculated as effect-size values and consisted of dynamic balance; pain measurements according to pain drawing and MPI; psychological variables such as psychological distress and self-image; and health related factors with regard to quality of life, self-efficacy and sense of coherence. A cluster analysis revealed three subgroups of patients with significantly different treatment outcomes, which were systematically related to the treatment approach. With Body Awareness Therapy and Feldenkrais, the possibility to achieve positive treatment effects increased significantly in comparison to individual physiotherapy.

    Background: The use of digital video meetings in physiotherapy has increased in recent years, particularly  in conjunction with the covid-pandemic. The current evidence for physiotherapists' experiences and perceptions of using digital video meetings is limited. 

    Purpose: To investigate physiotherapists experiences and perceptions of meeting patients through digital video meetings. 

    Method: The study has a qualitative design. Data were collected through semi structured interviews of five physiotherapists in primary care. The data was processed using qualitative content analysis. 

    Results: Collected data resulted in the following main categories: restrictions in the use of digital video meetings, benefits of digital video meetings and development of digital video meetings. 

    Conclusion: In summary, physiotherapists generally considered digital video meetings to be a good tool available. Mainly by increasing the accessibility to the physiotherapist and by contributing to  a more flexible job. They also described limitations in their work with video meetings. For instance, organizational obstacles and the absence of physical contact with the patient. Furthermore, different ways of developing the video meetings were presented. For example, technical opportunities for improvement and opportunities for more education in the field.

    Bakgrund: Akupunktur har sedan 1984 varit godkänt av socialstyrelsen som behandling mot smärta och används idag flitigt av fysioterapeuter som behandlingsform mot flertal diagnoser där evidensen visat sig vara stark. 80% av befolkningen har någon gång i livet drabbats av ländryggssmärta. Syftet: Att med en litteratursammanställning jämföra effekten av smärtlindring mellan akupunktur och placebo nålar vid ländryggssmärta. Metod: Studien inkluderade 3 artiklar inom området ländryggssmärta, akupunktur och placebo nålar och deras effekt som smärtlindrande behandling. Artiklarna har kvalitetsgranskat enligt Fribergs kvalitetsanalys. Resultat: 2 av 3 studier visade på att akupunktur hade en bättre smärtlindrande effekt än placebo nålar vid ländryggssmärta. Akupunktur visade även på en längre smärtlindrande effekt efter avslutad behandling och vid tre månaders uppföljning. Konklusion: Mer forskning behövs trots att akupunktur visade på bättre smärtlindrande effekt då underlaget var för litet för att kunna dra en slutgiltig slutsats.

    Abstract [en]

    Musculoskeletal modeling based on Electromyography (EMG) has many applications in physiotherapy and biologically-inspired robotics. In this article, a novel methodology for the modeling of the dynamics of an antagonistic muscle pair that actuates the human ankle joint movements will be established. As it will be presented, the musculoskeletal model is based on a multi input single output (MISO) auto-regressive integrated moving average with exogenous input (ARIMAX) model, which takes the integrated EMG measurements as input and estimates the corresponding joint angles. Based on this methodology, a Pneumatic Artificial Muscle (PAM) robotic leg setup that mimics the flexion/extension movement of human ankle joint is controlled to replicate the human movement. The experimental results demonstrate the performance of EMG based joint angle estimation and control of the robotic leg with the proposed model.

    Electromyography (EMG) signals represent the electrical activation of skeletal muscles and contain valuable information about muscular activity. Estimation of the joint movements by using surface EMG signals has great importance as a bio-inspired approach for the control of robotic limbs and prosthetics. However interpreting surface EMG measurements is challenging due to the nonlinearity and user dependency of the muscle dynamics. Hence it requires complex computational methods to map the EMG signals and corresponding limb motions. To solve this challenge we here propose to use an integrated EMG signal to identify the EMG-joint angle relation instead of using common EMG processing techniques. Then we estimate the joint angles for elbow flexion-extension movement by using an auto-regressive integrated moving average with exogenous input (ARIMAX) model, which takes integrated EMG measurements as input. The experiments showed that the suggested approach results in a 21.85% average increase in the estimation performance of the elbow joint angle compared to the standard EMG processing and identification.

    Study designPsychometric study.ObjectivesTo i) describe the translation process and ii) explore the data completeness, targeting, reliability and aspects of validity of the Swedish version of Moorong Self-Efficacy Scale (s-MSES).SettingsCommunity rehabilitation program.MethodsNinety-two program participants and 42 peer mentors with spinal cord injury (SCI) in Active Rehabilitation training programs (enrolled in the International Project for the Evaluation of activE Rehabilitation (Inter-PEER)) were included. The s-MSES was completed online, once for program participants and twice for peer mentors. The translation process was based on guidelines and involved researchers, clinicians and consumers.ResultsMinor linguistic adaptations were made. Ninety-one percent obtained a total score. As expected, peer mentors exhibited ceiling effects in all subscales. Cronbach ' s alpha for the total scale was 0.92 (subscales 0.74-0.83). The intraclass correlation coefficient was excellent for the total and subscale scores (0.78-0.91). The s-MSES exhibited sensitivity to changes and there were no systematic changes between evaluation points. The s-MSES correlated significantly and positively with life satisfaction and resilience, and negatively with depression/anxiety.ConclusionThe s-MSES was translated through a rigorous, consumer-involved process ensuring accurate linguistic translation and cultural adaptation. Our results support the data completeness, targeting, reliability and aspects of validity of the s-MSES. The s-MSES can thus be considered suitable to assess self-efficacy in persons with SCI in community rehabilitation settings.     Abstract [en]

    Background Work disability is common among women with fibromyalgia (FM). The aim of the study was to investigate what health problems and work-related difficulties lead to hinders for continued work among women with FM. Methods A qualitative interview study. Twenty-seven gainfully employed women with FM participated in five focus group interviews. Their median age was 52 years, ranging from 33 to 62. The transcribed interviews were analyzed by content analysis. Results Health problems and work-related demands were identified. Limited physical capacity, increased stress and an increased need of rest were the major health problems, while physical, psychosocial and work organizational demands were the main work-related problems. Personal factors and factors related to family influenced the strategies used to manage the imbalance between the health problems and work-related demands. Conclusions Limited physical capacity and an increased need of rest made it difficult for these women to manage the physical, psychosocial and organizational work demands. Adjustment of the work tasks and work environment were the main factors influencing whether the women with FM could work or not.

    PURPOSE: To study how patients with fibromyalgia (FM) experienced physiotherapy group treatment comprising pool exercise and education. METHODS: Nineteen patients with FM were interviewed and the transcribed interviews were analysed using a phenomenological life-world approach. The participants' mean age was 45 years and their mean symptom duration was 10 years. RESULTS: Physiotherapy group treatment was experienced as an embodied learning process, comprising the themes: positive experience of body, sharing experiences of living with FM and creating new patterns of acting. The positive experience of body comprised the following categories: experiencing relaxation, experiencing physical capacity, acknowledging limitations and changing the pattern of activity. Sharing experiences comprised the following categories: not being alone and sharing joy. Three patterns of participation were identified: reciprocity, distancing and remaining outside. Creating new patterns comprised the following categories; calming down, creating a new relationship to self, creating a new relationship to social roles and creating new patterns for managing pain. CONCLUSIONS: The study indicates that physiotherapy group treatment was experienced as an embodied learning process. Positive experiences of body were intertwined with a new relationship to self and objects in the world. Interactions between the co-participants promoted the process of creating new patterns of thinking and acting in the social world.

    Abstract [en]

    Fibromyalgia and related syndromes are characterized by chronic pain and fatigue. This chapter identifies the types of exercise that are effective for these patients and provides recommendations for exercise prescriptions. Based on a systematic review of randomized controlled studies of exercise, we suggest that low-intensity aerobic exercise, such as walking, can improve function and symptoms. Aerobic exercise performed twice a week at moderate intensity can improve aerobic capacity and reduce tenderness. Pool exercise can improve function, distress and symptoms. Strength training at adequate load can improve strength without exacerbation of symptoms. Most patients tolerate low-intensity exercise. High-intensity exercise should be undertaken with caution. Due to the large variability of functioning and symptom severity in patient populations, exercise prescriptions should be individualized and should include a long-term plan to maximize functioning and wellbeing. Studies with larger populations, allowing subgroup analyses regarding benefits and adverse effects of programmes, are needed.

    Background Middle and distal insets of gastroepiploic vascularized lymph node transfer (GE-VLNT) for extremity lymphedema have been described. However, there has been no prior comparison of surgical or patient-reported outcomes between these techniques. We analyzed the outcomes between both insets in patients with extremity lymphedema. Methods Retrospective review of patients with extremity-lymphedema who underwent GE-VLNT. Two groups were analyzed: middle and distal recipient inset. We analyzed 6-month surgical and patient-reported outcomes using the Lymphedema Life Impact Scale-v2 (LLISv2) and scar satisfaction utilizing the Patient Scar Assessment Questionnaire (PSAQ). Results Between 2017 and 2019, 26 patients with stage II unilateral extremity lymphedema underwent laparoscopically-harvested GE-VLNT (13 distal and 13 middle inset). There were no differences in patient demographics between groups. Mean hospital stay for patients with upper extremity lymphedema was 1.3 vs. 4.0 days (P<0.05), and for lower extremity lymphedema was 1.0 vs. 4.5 days (P<0.05), middle vs. distal inset, respectively. Mean return to daily activities for patients with upper extremity lymphedema was 13.4 vs. 33.4 days (P<0.05), and for lower extremity lymphedema was 16.0 vs. 29.5 days (P<0.05), middle vs. distal inset, respectively. Both middle and distal inset showed significant mean excess volume reduction at 6 months postoperatively for both upper and lower extremity lymphedema (upper extremity: middle inset 23.3%, distal inset: 22.0%; lower extremity: middle inset 23.3% and distal inset 13.3%). LLISv2 scores showed improved functional outcomes postoperatively in both upper and lower extremity lymphedema with both insets. Scar satisfaction with appearance and symptoms was higher with middle inset (P<0.05). Conclusions: GE-VLNT is an effective surgical treatment for extremity lymphedema. The middle placement showed shorter hospital stay, early return to work and higher patient satisfaction.
