    Abstract [en]

    Poor nutritional status amongst elderly individuals with hip fractures is well documented. Studies have suggested that 30-50 % of patients admitted to orthopaedic departments suffer from protein-energy malnutrition (PEM). An 6 month intervention study. The study was conducted in Sweden between February 2005 and October 2006. Elderly patients with hip fractures (n=32). Evaluation of compliance with individual nutritional support and whether body weight and body fat (BF) could be maintained after six months. Evaluation of possible effects of nutritional supplements and dietary advice after hip fracture on BMI, BF, and Mini Nutritional Assessment (MNA). Overall compliance with supplement intake was 73%. After six months, BMI was unchanged. Womens BF had decreased (P andlt; 0.01), although the mean calorie intake with nutritional support was 34 calories per kg body weight/day. Three groups could be identified: one group with increased body weight and BF, one with loss of body weight and BF, and one with increased body weight together with increased TBW and loss of BF. Participants who consumed 0-1 supplements daily lost more weight than those who consumed 2 supplements daily. There was a positive difference (p=andlt; 0.001) for women between MNA values at baseline and after six months. In the present study compliance was satisfactory at the group level, and the energy and protein intake increased significantly. BMI was unchanged during the 6 months period. However, the women lost BF during the study period of with some had increasing total body water (TBW). MNA values for women changed in a positive direction.

    Abstract [en]

    Objectives: Both ambulatory and central blood pressures have been reported to correlate more closely than office blood pressure with left ventricular mass index (LVMI). The aim of this study was to test whether ambulatory systolic blood pressure (SBP) predicts LVMI independently of central SBP in patients with type 2 diabetes.

    Methods: We determined office, ambulatory and central blood pressures and performed echocardiography in 460 patients with type 2 diabetes, who participated in the CARDIPP (Cardiovascular Risk factors in Patients with Diabetes – a Prospective study in Primary care) study (ClinicalTrials.gov number NCT 01049737).

    Results: In separate multivariable regression models, both 24-hour ambulatory SBP, ambulatory day-time SBP, and ambulatory night-time SBP were significantly associated with LVMI, independently of central SBP, age, sex, BMI, ambulatory 24-hour heart rate, duration of known diabetes and presence or absence of any antihypertensive medications (r = 0.19, 0.17 and 0.18, respectively, P <0.01). All ambulatory SBP parameters, but not central SBP, were significantly associated with LVMI independently of office SBP.

    Conclusions: Ambulatory SBP predicted LVMI independently of central SBP in patients with type 2 diabetes. The use of ambulatory blood pressure measurements may be encouraged as a tool for refined risk stratification of patients with type 2 diabetes.

    Abstract [en]

    Purpose: To investigate district nurses (DN) opinions regarding facilitators and barriers in their work with health promotion. Methods: Qualitative descriptive study. Interviews with a purposeful sampling of 54 DNs. Data were analysed with content analysis. Results: The DNs expressed that nursing was the right profession to work with health promotion, and that it was an important task that was done by no one else. They experienced being stuck between disease-oriented and health promotion work and that priorities were wrong. The focus was not on health promotion and the time DNs could spend with health promotion was limited, and instead they had to perform elementary medical tasks. Conclusions: Tasks of a medical nature are given priority over health promotion. DNs lack support and no one asked or cared about the results from health promotion activities.

    OBJECTIVES: The relation between the use of different hormonal contraceptives and antidepressant therapy was investigated. METHODS: In a nationwide cross-sectional study among all women in Sweden aged 16-31, drug expenditure data on hormonal contraceptives and antidepressants were obtained from the Swedish Prescribed Drug Register. Odds ratios (ORs) for antidepressant use were calculated by logistic regression for progestin-only users versus non-users as well as for combined hormonal contraceptive (CHC) users versus non-users. RESULTS: In the study population (N = 917,993), 58.9% were hormonal contraceptive users, and 8.5% were antidepressant users. The age-pattern for antidepressant ORs differed between the two types of users of contraceptives; progestin-only users had significant ORs above 1 in all age groups whereas among CHC users the OR was above 1 solely in those aged 16-19. The largest difference between types of users was seen in the age group 16-19 in which women resorting to a progestin-only contraceptive had a 67% (95% confidence interval: 57- 78%) higher antidepressant use than women treated with a CHC. CONCLUSION: Progestin-only contraceptive users resorted to antidepressants more than users of CHCs. This phenomenon is particularly pronounced among teenagers. Therefore, special attention should be given to young women's mental history when prescribing hormonal contraceptives and vice versa: the contraceptive history should be taken into account when prescribing antidepressants.

    Abstract [en]

    Objective. The aim of this study was to investigate how distance to hospital and socioeconomic status (SES) influence the use of secondary health care (SHC) when taking comorbidity into account. Design and setting. A register-based study in Ostergotland County. Subjects. The adult population of Ostergotland County. Main outcome measures. Odds of SHC use in the population and rates of SHC use by patients were studied after taking into account comorbidity level assigned using the Adjusted Clinical Groups (ACG) Case-Mix System. The baseline for analysis of SES was individuals with the lowest education level (level 1) and the lowest income (1st quartile). Results. The study showed both positive and negative association between SES and use of SHC. The risk of incurring SHC costs was 12% higher for individuals with education level 1. Individuals with income in the 2nd quartile had a 4% higher risk of incurring SHC costs but a 17% lower risk of emergency department visits. Individuals with income in the 4th quartile had 9% lower risk of hospitalization. The risk of using SHC services for the population was not associated with distance to hospital. Patients living over 40 km from hospital and patients with higher SES had lower use of SHC services. Conclusions. It was found that distance to hospital and SES influence SHC use after adjusting for comorbidity level, age, and gender. These results suggest that GPs and health care managers should pay a higher degree of attention to this when planning primary care services in order to minimize the potentially redundant use of SHC.
