    Abstract [sv]

    Uppsatsen inleds med en beskrivning av Familjebaserad korttidsterapi såsom den bedrivs vid Barn- och ungdomspsykiatrimottagningarna i Falun resp Sundsvall/Härnösand, där författarna själva är verksamma. Modellen innehåller en särskild struktur med urvalsprocess utifrån särskilda kriterier och 1-2 samtal som är två timmar inkl paus. Ett salutogent synsätt genomsyrar denna familjeterapeutiska modell, där det också ingår reflektioner för terapeuter och med familj. Ramarna och de olika momenten i behandlingen syftar till att starta och optimera en förändringsprocess som familjerna själva kan arbeta vidare med.

     

    Studien syftar till att få en fördjupad beskrivning av modellen samt närmare undersöka hur de verksamma terapeuterna i Falun och Sundsvall/Härnösand tänker att de påverkas av att arbeta med denna modell och vad de tänker att de själva bidrar med som är verksamt. Som metod valdes en form av semistrukturerad intervju, s k fokusgruppintervju. Terapeuternas reflektioner utifrån intervjuerna, redovisas dels i processbeskrivande form, dels i tematiska summeringar och citat.

     

    Resultatet visar på att terapeuterna i båda grupperna beskriver att de i denna modell tycker sig vara på en högre medveten prestationsnivå och att de upplever mer kraftfullhet och positiva känslor än när de arbetar med annat behandlingsarbete. De beskriver att både den begränsade tiden, samt övriga ramar, förhållningssätt och metoder samverkar och påverkar den egna upplevelsen och att det de gör upplevs verksamt. Särskilt intressanta reflektioner framkommer kring betydelsen av arbetsteamet, som finns med i och under det direkta familjearbetet, samt de ”reflekterande teamen” som metod.

    Uppsatsens syfte var att via intervjuer med behandlingspersonal vid en barnpsykiatrisk klinik undersöka deras syn på behov av att erbjuda familjebehandling/familjeterapeutiska insatser samt hur man, om behov föreligger, ska gå tillväga för att stärka familjeperspektivet utifrån förändrad prioritering. Det visade sig att alla respondenter, både nya och mer erfarna, såg ett tydligt behov av att behandla hela familjer då barnet är en del av detta sammanhang och inte själva kan bära ansvar för förändring. Respondenterna såg både ett behov av familjekompetens hos alla behandlare, men även behov av familjeterapi för mer komplexa ärenden. Tre betydelsefulla åtgärder kunde identifieras för att förstärka familjeperspektivet. Dels framkom ett behov av utbildningsinsatser. En basutbildning för alla på kliniken i familjebehandling med syfte att förstärka och behålla kompetens, men framförallt för att skapa en gemensam grundsyn hos medarbetarna. Man ansåg det också som betydelsefullt och lärorikt att få arbeta tillsammans med erfarna behandlare. Även familjeterapeuterna behöver fortbildas, dels i nya evidensbaserade metoder men också handledarutbildas beroende på hur uppdraget till gruppen kommer att formuleras av ledningen. Specialistgrupp för att möjliggöra metodarbete är den andra åtgärden. Kompetens finns i metod hos klinikens medarbetare men praktiseras inte till fullo. De metoder som används systematiskt inom kliniken, är de som har stöd av specialistgrupp. Rätt använd kompetens ses också som betydelsefullt. Dels utifrån att det bidrar till att behålla och utveckla spetskompetens, men det skapar också ökad respekt för ledningen hos medarbetarna och ger arbetstillfredsställelse om klinikens resurser används på ett effektivt sätt.

    Abstract [en]

    A Ca/+-activated protease from rat liver cell sap was prepared. It was shown to act on rat liver pyruvate kinase that had been phosphorylated by the catalytic subunit of cyclic AMP-dependent protein kinase, the activity being optimum at neutral pH. The modified pyruvate kinase had the same Vmax as the phosphoenzyme but showed a lower affinity for the substrate phosphoenolpyruvate. The possibility that this proteolytic attack is the step that initiates further degradation in the cell is discussed.

    OBJECTIVES: The purpose of this study was to investigate sleep with polysomnography and self-ratings and the diurnal pattern of sleepiness and fatigue in a group suffering from severe occupational burnout. METHOD: Twelve white-collar workers on long-term sick leave (>3 months) and 12 healthy controls with high and low scores on the Shirom Melamed Burnout Questionnaire (SMBQ) were included. A 1-night polysomnographic recording (after habituation) was carried out at home, and sleepiness and mental fatigue were rated at different times of the day for weekdays and the weekend. Precipitating factors at the time of the illness at work and real life were considered, and different dimensions of occupational fatigue were described. A repeated-measures analysis of variance using two or three within group factors was used to analyze the data. RESULTS: The main polysomnographic findings were more arousals and sleep fragmentation, more wake time and stage-1 sleep, lower sleep efficiency, less slow wave sleep and rapid eye movement sleep, and a lower delta power density in non-rapid eye movement sleep in the burnout group. The burnout patients showed pronounced sleepiness and mental fatigue at most times of the day for weekdays without reduction during weekends. The precipitating factor was occupational stress (psychiatric interview), and work stress indicators were increased. CONCLUSIONS: Occupational burnout is characterized by impaired sleep. It is suggested that impaired sleep may play a role in the development of fatigue or exhaustion in burnout.

    Abstract [en]

    Objective: Previous work has demonstrated a link between restricted sleep and risk indicators for cardiovascular and metabolic disease, such as levels of cortisol, lipids, and glucose. The present study sought to identify relations between polysomnographic measures of disturbed sleep (frequency of arousals from sleep, total sleep time, and sleep efficiency) and a number of such indicators. A second purpose was to relate the number of arousals to mood, stress, work characteristics, and other possible predictors in daily life. Methods: Twenty-four people (10 men, 14 women; mean age 30 years), high vs. low on burnout, were recruited from a Swedish IT company. Polysomnographically recorded sleep was measured at home before a workday. Blood pressure, heart rate, morning blood sample, and saliva samples of cortisol were measured the subsequent working day. They were also recorded for diary ratings of sleep and stress, and a questionnaire with ratings of sleep, stress, work conditions, and mood was completed. Results: A stepwise regression analysis using sleep parameters as predictors brought out number of arousals as the best predictor of morning cortisol (serum and saliva), heart rate, systolic and diastolic blood pressure, total cholesterol, high-density lipoprotein (HDL)-, low-density lipoprotein (LDL)-cholesterol, and LDL/HDL-ratio. Work stress/unclear boundaries between work and leisure time was the best predictor of arousals among the stress variables. Conclusion: Consistent with sleep restriction experiments, sleep fragmentation was associated with elevated levels of metabolic and cardiovascular risk indicators of stress-related disorders. Number of arousals also seems to be related to workload/stress.

    Nowadays, there are life-sustaining treatment that can prolong patients’ lives. This may make it difficult for medical personnel to set the limit for when the transition from curative to palliative care should be performed in institutional care. The purpose of the study was to describe the nurses’ experiences when a decision would be taken on when the patient went from curative to palliative care. In this study fifteen scientific articles were reviewed and analyzed. Four headlines emerged from the result: involvement, point in time, education/experience and feelings. One thing nurses had in common was that they thought that their opinion did not matter for the doctors about the transition from curative to palliative care, even though they spent more time with the patient and had a holistic point of view on the situation. The point in time was important in regards to when the decision was made and when the curative treatment was brought to an end. One difference between nurses was their experience and how secure they felt within their occupation. The nurses felt that it was difficult when there were not straight directives on how the treatment should be carried out. To be more secure in their jobs as nurses, there should be a higher level of education introduced throughout all of the health and medical care fields and there should be more research done about the patient’s experiences in their situation.

    Abstract [en]

    OBJECTIVE: Patient-physician communication and patient satisfaction are important elements of cancer care. Trust is considered to be crucial for the patient-physician relationship, yet little is to be found in the literature regarding what factors may influence trust.

    METHODS: We assessed predictors of trust in health-care professionals and in the medical care by administering two questionnaires, one at start of chemotherapy treatment and one at the time of the third chemotherapy cycle, to 304 parents of children with newly diagnosed cancer at the Children's Cancer Hospital in Cairo, Egypt.

    RESULTS: Parents' trust in the medical care at the time of the child's third chemotherapy cycle was significantly associated with the following at the start of treatment: having received at least moderate information about the disease (relative risk (RR) 13.2; 95% CI 7.8-22.3) and the treatment (RR 17.2; 95% CI 9.5-31.4), having the opportunity to communicate with the child's physicians (RR 21.3; 95% CI 11.7-38.8), being satisfied with the physicians conversation style (RR 30.6; 95% CI 14.4-64.9), having the emotional needs met (RR 22.2; 95% CI 11.8-41.9), and being met with care by the child's physicians (RR 32.0; 95% CI 15.2-67.7). After multivariable model selection, the strongest predictor of trust at the time of the third chemotherapy cycle was to be met with care at the start of treatment.

    CONCLUSION: Parents being met with care by the child's physicians at the beginning of the child's chemotherapy treatment develop an increased trust in the medical care.

    OBJECTIVE: Informing the child about his/her diagnosis and treatment plan is essential; research has shown that it is related to the patient's quality of life and adherence to medication.

    METHODS: For 7 months during 2008 (February to September), 2 study-specific questionnaires were constructed and administered to 304 parents of children diagnosed with cancer at the Children's Cancer Hospital Egypt.

    RESULTS: Among the 313 eligible parents of children diagnosed with cancer, 304 (97%) answered the first questionnaire and 281 (92%) answered the second questionnaire. We found that nearly three-quarters (72%) of the parents had their child's cancer diagnosis communicated by the physician. Among the 72%, the rate of the children present with the parent or parents during the disease disclosure conversation was 39% (n = 85/219). The majority of the children were in the age group 5-18 years (55%).

    CONCLUSIONS: Our findings indicate that cancer disclosure at the Children's Cancer Hospital is to a certain degree common; yet even when disclosure does take place, it is mainly in the absence of the child. Moreover, the information provided during the conversation may not be fully comprehended by the parent or the child because of the physician's misleading use of terms when disclosing the disease. Therefore, better practice should be developed for disease disclosure, and proper communication should be established between the patients and the provider; patient autonomy should also have an influence in the clinical practice.

    AIM: To investigate adherence to medical regimen and predictors for non-adherence among children with cancer in Egypt.

    METHODS: We administered two study specific questionnaires to 304 parents of children diagnosed with cancer at the Children's Cancer Hospital in Cairo, Egypt, one before the first chemotherapy treatment and the other before the third. The questionnaires were translated to colloquial Egyptian Arabic, and due, to the high illiteracy level in Egypt an interviewer read the questions in Arabic to each parent and registered the answers. Both questionnaires consisted of almost 90 questions each. In addition, a Case Report Form was filled in from the child's medical journal. The study period consisted of 7 mo (February until September 2008) and we had a participation rate of 97%. Descriptive statistics are presented and Fisher's exact test was used to check for possible differences between the adherent and non-adherent groups. A P-value below 0.05 was considered significant. Software used was SAS version 9.3 for Windows (SAS Institute Inc., Cary, NC, United States).

    RESULTS: Two hundred and eighty-one (90%) parents answered the second questionnaire, regarding their child's adherence behaviour. Approximately two thirds of the children admitted to their third chemotherapy treatment had received medical recommendations upon discharge from the first or second chemotherapy treatment (181/281, 64%). Sixty-eight percent (123/181) of the parents who were given medical recommendations reported that their child did not follow the recommendations. Two main predictors were found for non-adherence: child resistance (111/123, 90%) and inadequate information (100/123, 81%). In the adherent group, 20% of the parents (n = 12/58) reported trust in their child's doctor while 14 percent 8/58 reported trust in the other health-care professionals. Corresponding numbers for the non-adherent group are 8/123 (7%) for both their child's doctor and other health-care professionals. Almost all of the parents expressed a lack of optimism towards the treatment (116/121, 96%), yet they reported an intention to continue with the treatment for two main reasons, for the sake of their child's life (70%) (P = 0.005) and worry that their child would die if they discontinued the treatment (81%) (P < 0.0001).

    CONCLUSION: Non-adherence to medical regimen is common among children diagnosed with cancer in Egypt, the main reasons being child resistance and inadequate information.

    According to the National Institute of Public Health about 350 000 Swedish children are living in homes where one or both adults has an alcohol addiction. Only one percent of these children is identified and offered support groups or similar activities, the rest goes by undetected through all of the educational years. The purpose of this study was to examine how the school nurse is working to identify children whose parents have an alcohol abuse and how they support these children. What support the school nurse wish to have in their work with these children, and how they keep themselves updated on the problems of these children. The basis of this study is based on a number of semi-structured questions. These questions were asked when interviewing ten school nurses. The material was analyzed by means of a qualitative content analysis. The results of the study demonstrate that the school nurse discovers children whose parents have an alcohol abuse by being watchful  and by gutfeeling in the meeting with the children and they are observent in the healthteam. They are giving support through personal contact with the children and by putting them in contact with

     other professions and buisnesses to provide them the support they need. The schoolnurse keeps her self updated of the problem through courses and lectures and by seeking information her self. The school nurse also cooperate with different professions round the school, and uses their own experiences from work and personal life.

    Företagshälsovårdens arbete med förebyggande arbetsmiljöåtgärder inom fysisk arbetsmiljö: Arbetssätt, metoder och effekter2014Report (Other academic)

    En av företagshälsovårdens viktigaste uppgifter är att arbeta med primärpreventiva tjänster för att förebygga arbetsrelaterade ohälsa hos sina kundföretag. Det finns generellt sett lite internationell forskning om företagshälsovård och dess förebyggande arbetsmiljöarbete. Denna litteraturstudie syftar till att undersöka hur företagshälsovården arbetar som utförare av arbetsmiljöinterventioner hos olika kundföretag med inriktning på primärprevention gällande fysisk arbetsmiljö. Sju artiklar inkluderades i studien. Resultatet visar att det finns få studier där det tydligt framgår att företagshälsovården är utförare av interventionen. De inkluderade studierna visar att när ett multidisciplinärt team varit involverat samt när interventioner har skett på flera nivåer, det vill säga på organisations-, grupp- och individnivå så har effekten varit positiv. Framgångsrika arbetssätt som testats av företagshälsovården för ett primärpreventivt arbete inom fysisk arbetsmiljö har bland annat inneburit systematiska riskbedömningar och teamarbete tillsammans med kundföretaget.

    Design: Data were taken from a sample of 1509 listeners aged between 18 and 91 years old. Participants completed computerized versions of the TMT and an adaptive speech-in-noise recognition test. All testing was conducted via the internet.

    Results: The results indicate that better performance on both the simple and complex subtests of the TMT are associated with better speech-in-noise recognition scores. Thirty-eight percent of the participants had scores on the speech-in-noise test that indicated the presence of a hearing loss.

    Conclusions: The findings suggest that the TMT may be a useful tool in the assessment, and possibly the treatment, of speech-recognition difficulties. The results indicate that the relation between speech-in-noise recognition and TMT performance relates both to the capacity of the TMT to index processing speed and to the more complex cognitive abilities also implicated in TMT performance.

    Abstract [en]

    There are more herbivorous waterfowl (swans and geese) close to humans, livestock and poultry than ever before. This creates widespread conflict with agriculture and other human interests, but also debate about the role of swans and geese as potential vectors of disease of relevance for human and animal health. Using a One Health perspective, we provide the first comprehensive review of the scientific literature about the most relevant viral, bacterial, and unicellular pathogens occurring in wild geese and swans. Research thus far suggests that these birds may play a role in transmission of avian influenza virus, Salmonella, Campylobacter, and antibiotic resistance. On the other hand, at present there is no evidence that geese and swans play a role in transmission of Newcastle disease, duck plague, West Nile virus, Vibrio, Yersinia, Clostridium, Chlamydophila, and Borrelia. Finally, based on present knowledge it is not possible to say if geese and swans play a role in transmission of Escherichia coli, Pasteurella, Helicobacter, Brachyspira, Cryptosporidium, Giardia, and Microsporidia. This is largely due to changes in classification and taxonomy, rapid development of identification methods and lack of knowledge about host specificity. Previous research tends to overrate the role of geese and swans as disease vectors; we do not find any evidence that they are significant transmitters to humans or livestock of any of the pathogens considered in this review. Nevertheless, it is wise to keep poultry and livestock separated from small volume waters used by many wild waterfowl, but there is no need to discourage livestock grazing in nature reserves or pastures where geese and swans are present. Under some circumstances it is warranted to discourage swans and geese from using wastewater ponds, drinking water reservoirs, and public beaches. Intensified screening of swans and geese for AIV, West Nile virus and anatid herpesvirus is warranted.

    Abstract [en]

    Background and objectives: In the past two decades, an increasing number of nationwide, Swedish Healthcare Quality Registries (QRs) focusing on specific disorders have been initiated, mostly by physicians. Here, we describe the purpose, organization, variables, coverage and completeness of 103 Swedish QRs. Methods: From March to September 2013, we examined the 2012 applications of 103 QRs to the Swedish Association of Local Authorities and Regions (SALAR) and also studied the annual reports from the same QRs. After initial data abstraction, the coordinator of each QR was contacted at least twice between June and October 2013 and asked to confirm the accuracy of the data retrieved from the applications and reports. Results: About 60% of the QRs covered 80% of their target population (completeness). Data recorded in Swedish QRs include aspects of disease management (diagnosis, clinical characteristics, treatment and lead times). In addition, some QRs retrieve data on self-reported quality of life (EQ5D, SF-36 and disease-specific measures), lifestyle (smoking) and general health status (World Health Organization performance status, body mass index and blood pressure). ConclusionDetailed clinical data available in Swedish QRs complement information from government-administered registries and provide an important source not only for assessment and development of quality of care but also for research.

    Studiens syfte var att undersöka familjeterapeuters erfarenheter av att arbeta med

    temat sexualitet i familjeterapi. För att försöka besvara detta genomfördes intervjuer

    med fem legitimerade familjeterapeuter som hade erfarenheter av att arbeta med par.

    Frågeställningarna var; Förekommer temat sexualitet i de samtal familjeterapeuter

    har? Hur ser familjeterapeuter på sin roll när det gäller temat sexualitet i samtalen?

    Fem intervjuer genomfördes och analyserades med tematisk analys. Resultatet visar

    att temat sexualitet ofta kommer upp i familjeterapeuternas samtal främst när det

    gäller parsamtal där temat initierades av familjeterapeuterna oavsett vilket uppdrag

    som presenterades. Tema sexualitet kom även upp i individualsamtal men då mer

    sammankopplat med det problem man sökte för. Familjeterapeuterna ansåg att det är

    deras ansvar att ta upp temat sexualitet. För att våga detta krävdes att terapeuten var

    trygg med ämnet sexualitet. Faktorer som bidrog till denna trygghet var

    familjeterapeuternas inställning till och klargörande av sin egen sexualitet, kunskaper

    i ämnet sexualitet, handledning där sexualitet togs upp samt tillgång till forum där

    man kunde prata om sexualitet.

    Abstract

     

    The purpose of this work was to scientifically evaluate the sustainability of parts of  a company-specific survey within the Occupational Health Services (FHV). The service is called HALU which is an abbreviation of the “Work Health and Lifestyle” survey and provided by Feelgood AB. HALU is a questionnaire that is used frequently towards the customer and answered on an individual level with subsequent health interviews and a statement regarding the organization's performance in order to identify the health, lifestyle and working situation of the organization. In this work, questions regarding workloads were evaluated. There were six different workload questions, and the question in this paper were whether these issues had an internal consistency, validity and reliability, and if these issues’ correlation to questions about health during the past year. These analyzes were designed as cross-sectional studies The work also included a longitudinal study in which survey responses from the same individual on two occasions analyzed to see if the responses of workload and health has correlation at two different response times. The material used is a selection from Feelgood database from which includes total includes 130,422 HALU surveys. This sample includes 17,513 surveys from 201007. The study was performed in three steps: Step 1 was a reliability study in which the internal consistency analysis regarding workload questions in the questionnaire . A workload index was created (AI) Stage 2 was a validity study where the correlation coefficient analyzed for correlation between the AI which was calculated in step 1 and the in -depth study of health issues. Step 3 was also a validity study where the correlation coefficient was analyzed in terms of relationships between different survey occasions in the same individual, in order to see if the change in workload results in change in self-perceived health . The result showed that there was strong internal consistency regarding workload issues and that there was correlation between workload and health in a cross section . There were also correlation between survey responses occasions but this was low.

    Abstract [en]

    Dissatisfaction with medical information is a common problem among patients. There is also evidence that patients lack information that physicians believe they have given to the patient. The aims of this study were to 1) survey patients' subjective need for, and satisfaction with, the information that they received during their hospital stay 2) develop and evaluate systematic routines for giving information to the patients and also communication and collaboration between the medical and nursing staff concerning the satisfaction of the patients' need for information.

    The study was an intervention project and the research perspective was organizational psychology.

    Survey study. The patients experienced a considerable need for medical information, especially about the examination results and prognosis. The patients' need for information regarding prognosis was the least satisfied.

    Intervention 1. A general improvement of the information to the patients occurred when the systematic routines were established. The patients' subjective need for information was unchanged throughout two years. Their satisfaction with information, after an initial improvement, did not increase throughout these two years. There was low correlation between the patients' and their physicians' estimations concerning the patients' need for information on diagnosis, prognosis and examination results. Likewise, concerning the adequacy of that information.

    Intervention 2. Communication and collaboration between the medical and nursing staff included a system for assessment and solution of the patients' information problems. Problem-solving took place at a multidisciplinary team conference (MTC). Medical problems were better elucidated than the patients' psychological problems. After training of registered nurses (RN) as conference chairpersons, the patients need for information was better understood. The staff reported 42 information problems after training compared to two before. For half of the information problems decisions were discussed on steps to be taken in order to satisfy the patients' need for information. A year after the system for assessment and solution of information problems was established, the patients were more satisfied with information about examinations and their results and on information about medication (p< 0.05). Further, new norms for the patients' need for information were established and a change was initiated.

    The results are discussed with regard to how and why patients' shall have information, by whom and to whom information shall be given, when and where information shall be given and which content it shall have.

    Impaired speech perception occurs in several groups of children enrolled at Speech Language Pathology and Audiological clinics. There may be children with language impairment, cognitive impairments, hearing impairments and children with (Central)Auditory Processing Disorders (CAPD). At present no standardized speech perceptiontest in Swedish provides information about how children discriminate, identify and produce speech sound contrasts in words. It is therefore of great importance to develop diagnostic tools to obtain a reliable test procedure and enable differential diagnostics.The present study had two purposes, to prepare and standardize a new speech perceptiontest (the Listen-Say test) and to investigate how typically developing children in grade 1 and 2 perform in quiet and in babble noise. The study included 21 children. Results showed that the children discriminated significantly more correct consonant contrasts inquiet compared to babble noise for all consonant contrasts combined. The study alsoshowed differences in some of the contrasts that were examined, both between quiet andbabble noise and between grades, grade 2 discriminated more correct contrasts thangrade 1. Significantly shorter reaction times for correct recognition of target words inbabble noise compared to quiet was observed. It is necessary to develop the test additionally prior to use in clinical practice. Nevertheless, the first data collection highlights important aspects of speech perception of Swedish consonant contrasts inquiet and in babble noise.

    The quality of clinical biobank samples is crucial to their value for life sciences research. A number of factors related to the collection and storage of samples may affect the biomolecular composition. We have studied the effect of long-time freezer storage, chronological age at sampling, season and month of the year and on the abundance levels of 108 proteins in 380 plasma samples collected from 106 Swedish women. Storage time affected 18 proteins and explained 4.8–34.9% of the observed variance. Chronological age at sample collection after adjustment for storage-time affected 70 proteins and explained 1.1–33.5% of the variance. Seasonal variation had an effect on 15 proteins and month (number of sun hours) affected 36 proteins and explained up to 4.5% of the variance after adjustment for storage-time and age. The results show that freezer storage time and collection date (month and season) exerted similar effect sizes as age on the protein abundance levels. This implies that information on the sample handling history, in particular storage time, should be regarded as equally prominent covariates as age or gender and need to be included in epidemiological studies involving protein levels.

    Att skriva journal utgör en stor del av logopeders administrativa arbete, och för att reglera och kvalitetssäkra journalers hantering och innehåll finns det lagar och föreskrifter som måste följas. Journalmallar utformas till viss del efter dessa lagar och föreskrifter, men varierar beroende på typ av verksamhet och journalsystem. Att effektivisera och standardisera journalföring är viktigt då journalen kan fungera som en kommunikationsväg för flera vårdgivare, samt som informationskälla för patient och anhöriga. Journalen bör därför vara enkel att följa och vara skriven med ett språk som i så stor grad som möjligt kan förstås av olika yrkesprofessioner inom vården. Att följa upp hur dessa mallar efterföljs, samt om förbättringar kan göras, är därför ett värdefullt arbete.

    Syftet med studien har varit att kartlägga hur sökorden i statusdelen används av fem yrkesverksamma barnlogopeder, och om de använde sökorden utifrån verksamhetens lokala mall. Ytterligare en aspekt författarna ansåg var av intresse att studera var språkbruk och förståelighet i journalerna, vilket gjordes i pilotform av författarna själva. Totalt analyserades 15 journaler.

    Resultatet i kartläggningen av användandet av sökord i journalernas statusdel visade bland annat att information fördes in under fel sökord i sju av de femton journalerna utifrån journalmallen. Författarna kom i studien bland annat fram till att mallen som journalen skrivs efter behöver vara väldefinierad och tydligt utformad för att minska tolkningsfriheten för att få så enhetliga, tydliga och lättförståeliga journaler som möjligt.

    Bakgrund: Jämlik vård är att alla ska bemötas, behandlas och vårdas på lika villkor oavsett enskilda faktorer. I hörselrehabiliteringen ingår det flera delar och varje del ska utgå från helhetsperspektivet och ske i samverkan med den enskilde. Audionomen ska inte påverkas av faktorer såsom kön eller ålder. Tidigare studier visar dock att ålder är en avgörande faktor i många fall, det råder en form av ålderism och åldersdiskriminering inom vården. Inga tidigare studier har hittats på hur den tekniska rehabiliteringen påverkas av ålder inom hörselvården varför detta låg i vårt intresse.

    Syfte: Syftet med studien är att undersöka hur jämlik den tekniska rehabiliteringen är inom hörselvården beroende på patienternas ålder.

    Metod: Studien är deskriptiv retrospektiv och är gjord genom journalgenomgång.

    Resultat: Resultatet visar inte några tydliga skillnader i den tekniska rehabiliteringen beroende på patientens ålder. Patienternas behov varierade mycket både inom grupperna och mellan grupperna. Skillnader kunde dock ses gällande patienter med samma behov och typ av hörapparat där patienter oavsett ålder kunde ha fått olika typer av hörapparater trots samma behov/mål.

    Slutsatser: Inga generella slutsatser kan dras huruvida den tekniska hörselrehabiliteringen är jämlik eller ej. Fler studier behövs inom ämnet där urvalet är större.

    I Utbildning i svenska för invandrare (SFI) ryms flera logopediskt intressanta områden. För att få en större inblick i SFI gjordes därför denna kvalitativa studie över lärares syn på undervisning inom SFI med hjälp av två fokusgruppintervjuer och grundad teori som forskningsansats. Denna studie har som huvudsyfte att spegla hur SFI-lärare ser på såväl sin egen undervisning som undervisning inom detta område i stort. Vidare frågar sig författarna hur lärare ser på de språkliga områdena: fonologi, grammatik, lexikon, pragmatik samt läs- och skrivförmåga. Uppsatsen genomsyras av tre huvudteman;

    samhälle, SFI-undervisning och individ, som uppstått under pågående studie. En fördjupad modell har dessutom gjorts över temat SFI-undervisning.. Det framkommer att SFI-undervisning är ett komplext område som både påverkar och påverkas av såväl individ som samhälle. Sammanfattningsvis framkommer att varje tema var för sig genomsyras av en central del; resurser, tid respektive individualisering. Samhället styr vilka resurser som finns att tillgå och hur SFI i stora drag organiseras. På SFI påverkar tidsaspekten till vilken grad undervisningen kan individualiseras för den enskilda eleven. Två områden som särskilt utmärker sig i intervjusamtal kring behov av resurser är uttalsvårigheter samt läs- och skrivsvårigheter och dyslexi.

    Slutligen beskrivs att det vore av intresse att utreda vilka personresurser som redan finns och vad de i nuläget arbetar med samt effekten av detta. Författarna föreslår därmed att vidare studier görs för att utröna om individualisering av undervisning kan underlättas med hjälp av ökade resurser eller samarbeten och i så fall hur dessa bör vara utformade. Vid en sådan studie skulle också frågan om logopeder är en yrkeskategori som skulle kunna komplettera eller utgöra en hjälp för SFI, kunna besvaras.

    Abstract [en]

    There are few studies regarding phonological skills related to working memory in young children. There is also a lack of tests of working memory for children in the younger ages. The aim of the current study was therefore to examine the relationship between visual working memory, phonological working memory and the ability to manage phonological representations, in three-year-olds. This was done by collecting comparison data of threeyear- old children’s results on the newly developed tablet-test ”KUBEN” (Cognitive development in young children), which measures the abilities visual and phonological working memory and phonological discrimination. In addition test for phonological production, also tested with tablet, and nonword repetition were used. A total of 91 Swedish three-year-old children with typical development participated in the study.

    For especially visual working memory, there was a lack of material with which to compare, but the result showed that children performed as expected regarding all tested abilities. The abilities improved with increasing age. On the contrary, neither gender nor experience of using tablets, affected the results of the children’s performance. The result of this study further describes relations between the tested abilities. There were correlations between all the abilities in this study that measures phonological representations (phonological discrimination, nonword repetition and phonological production). However, these phonological abilities did not correlate with the test of phonological working memory, which is a less expected finding. A possible explanation may be that it is difficult to test phonological working memory separately in children as young as three years old. Neither was there a correlation between tests of phonological and visual working memory, which indicates that two different abilities of working memory were tested. This confirms the faction within research arguing that visual and phonological working memory are separated abilities, already in young children.

    Abstract [en]

    The purpose of this study was to develop a sensitive and specific method for quantifying dermal exposure to the resin acids 7-oxodehydroabietic acid (7-OXO), dehydroabietic acid (DHAA), abietic acid (AA), and pimaric acid (PA). In addition the method was evaluated in occupational settings during production of wood pellets. Tape-strips were spiked with the substances to evaluate the recovery of the acids from the tape. The removal efficiency of the tape was assessed by tape-stripping a specified area on a glass plate spiked with resin acids. The recovery of the acids from human skin in vivo was evaluated by applying acids in methanol onto the skin of volunteers. Occupational dermal exposure to the resin acids was assessed by tape-stripping the skin of workers involved in the production of wood pellets. The resin acids were analyzed by liquid chromatography mass spectrometry (LC-MS). The limit of detection was 15 pg (7-OXO), 150 pg (DHAA), 285 pg (AA) and 471 pg (PA) per injection. The recovery from spiked tapes was in general 100%. The removal efficiency of the tape was 48–101%. Recovery tests from human skin in vivo showed a mean recovery of 27%. Quantifiable amounts of resin acids were observed on four different skin areas with an increase in exposure during a work shift. This study shows that occupational dermal exposure to resin acids can be assessed by tape-stripping and quantified by LC-MS.

    Phonological awareness, decoding skills, letter knowledge and working memory capacity predict early reading skills. The aim of the present study was to examine how these different basic cognitive abilities together can explain reading comprehension in children during first year of school. Reading comprehension, decoding, phonological awareness, letter knowledge and working memory were studied in 36 children in first grade with normal hearing and had Swedish as their native language. The results show that decoding and phonological awareness together explain 62 % of the variance in reading comprehension among the participants. The conclusion from the present study is that decoding and phonological awareness predict reading comprehension during first grade.

    Mellan 200901 och 201212 genomgick 77 kvinnor värmeballongbehandling på grund av rikliga gynekologiska blödningar vid en gynekologisk klinik i Mellansverige. Postoperativt fick sju kvinnor stanna kvar över natten på grund av smärta och illamående, en kvinna fick stanna kvar då hon hade blödningsproblematik sedan tidigare och hennes blödningsbenägenhet kontrollerades, en kvinna fick problem med sitt blodsocker då hon var insulinbehandlad diabetiker och fick stanna för observation och tre kvinnor kunde inte tömma blåsan efter operationen. Sammanlagt åtta kvinnor fick endometrit efter värmeballongbehandlingen varav en fick läggas in för intravenös antibiotikabehandling. Fem av 77 kvinnor valde att hysterektomera sig inom ett år efter värmeballongbehandlingen, antigen på grund av fortsatta besvär med rikliga blödningar efter endometriedestruktionen eller på grund av kraftiga underlivssmärtor efter komplikationer som orsakades av värmeballongbehandlingen - enbart en kvinna valde på egen begäran hysterektomi trots ett normalt blödningsmönster efter värmeballongbehandlingen då hon önskade bli amenorreisk.

    Föräldrars mentaliseringsförmåga är avgörande för ett barns psykiska hälsa och utveckling.

    Utifrån detta utformades föräldragruppsbehandlingen Perspektiv med eklektisk utgångspunkt

    i framför allt Theraplay och Dialektisk beteendeterapi. Behandlingen gavs till föräldrar med

    barn mellan 0-12 år med allvarlig barnpsykiatrisk problematik med varierad symtombild och

    etiologi. En kvalitativ studie med fokusgruppsintervjuer genomfördes med syfte att undersöka

    föräldrars erfarenheter av behandlingen och eventuella upplevelser av effekt. Resultaten

    redovisas i teman som visar upplevelse av god effekt, vilket tolkas och diskuteras utifrån

    terapeutisk gruppeffekt, mentaliseringsutveckling och generalisering i vardagen vilka bygger

    på varandra i en cirkulär process. Resultaten stöder tidigare forskning som visar att

    föräldragruppsbehandling och mentaliseringsutveckling i grupp är effektivt. Studiens resultat

    motiverar fortsatt användning inom barn- och ungdomspsykiatrin och fortsatt

    metodutveckling.

    Background: Autism spectrum conditions (ASC) are defined by criteria comprising impairments in social interaction and communication.Altered visual perception is one possible and often discussed cause of difficulties in social interaction and social communication. Recently,Ashwin et al. suggested that enhanced ability in local visual processing in ASC was due to superior visual acuity, but that study has been thesubject of methodological criticism, placing the findings in doubt.

    Methods: The present study investigated visual acuity thresholds in 24 adults with Asperger’s syndrome and compared their results with 25control subjects with the 2 Meter 2000 Series Revised ETDRS Chart.

    Results: The distribution of visual acuities within the two groups was highly similar, and none of the participants had superior visual acuity.

    Conclusions: Superior visual acuity in individuals with Asperger’s syndrome could not be established, suggesting that differences in visualperception in ASC are not explained by this factor.Acontinued search for explanations of superior ability in local visual processing in personswith ASC is therefore warranted.

    Barns mentala ohälsa ökar kraftigt. Barnpsykoterapi med inslag av fri lek, här kallad lekterapi, har under många år varit den traditionella behandlingen av barn i behov av psykoterapi. För att ha en möjlighet att kunna jämföras vetenskapligt med andra dokumenterade behandlingsmetoder behöver evidensen för lekterapi som behandlingsmetod undersökas, stärkas liksom spridas. 

    Syftet med föreliggande uppsats är att undersöka kunskapsläget inom lekterapi som behandlingsmetod inom barnpsykoterapi. 

    Totalt 41 studier har granskats systematiskt enligt evidensbaserad medicinsk metod. Tjugoen av dem poängbedömdes medan övriga 20 delades upp i temagrupper för att visa ett ytterligare djup och bredd av de analyserade studierna. Tretton studier av de 21 poängbedömda fick minst 80% av totalpoängen, gradering I, och sju stycken graderingen II, d. v. s. 70-80% av max poängen. Enligt internationell praxis av gradering av evidensstyrka för slutsatser, visar två studier, eller fler, med ett högt bevisvärde eller god systematisk översikt på ett starkt vetenskapligt underlag som ger god evidens. Som ett bifynd visade sig 16 av de 41 studierna även ha en direkt anknytning till affektiv neurovetenskaplig forskning. 

    Rekommendationen är att använda barnpsykoterapi med fri lek för barn som ska behandlas vid mental ohälsa.

    Emergency department (ED) triage is used to identify patients' level of urgency and treat them based on their triage level. The global advancement of triage scales in the past two decades has generated considerable research on the validity and reliability of these scales. This systematic review aims to investigate the scientific evidence for published ED triage scales. The following questions are addressed:

    1. Does assessment of individual vital signs or chief complaints affect mortality during the hospital stay or within 30 days after arrival at the ED?

    2. What is the level of agreement between clinicians' triage decisions compared to each other or to a gold standard for each scale (reliability)?

    3. How valid is each triage scale in predicting hospitalization and hospital mortality?

    A systematic search of the international literature published from 1966 through March 31, 2009 explored the British Nursing Index, Business Source Premier, CINAHL, Cochrane Library, EMBASE, and PubMed. Inclusion was limited to controlled studies of adult patients (≥15 years) visiting EDs for somatic reasons. Outcome variables were death in ED or hospital and need for hospitalization (validity). Methodological quality and clinical relevance of each study were rated as high, medium, or low. The results from the studies that met the inclusion criteria and quality standards were synthesized applying the internationally developed GRADE system. Each conclusion was then assessed as having strong, moderately strong, limited, or insufficient scientific evidence. If studies were not available, this was also noted.

    We found ED triage scales to be supported, at best, by limited and often insufficient evidence.

    The ability of the individual vital signs included in the different scales to predict outcome is seldom, if at all, studied in the ED setting. The scientific evidence to assess interrater agreement (reliability) was limited for one triage scale and insufficient or lacking for all other scales. Two of the scales yielded limited scientific evidence, and one scale yielded insufficient evidence, on which to assess the risk of early death or hospitalization in patients assigned to the two lowest triage levels on a 5-level scale (validity).

    Objective

    The purpose of this study was to adapt and evaluate the psychometric properties of the ProFitMap-neck to Brazilian Portuguese.

    Methods

    The cross-cultural adaptation consisted of 5 stages, and 180 female patients with chronic neck pain participated in the study. A subsample (n = 30) answered the pretest, and another subsample (n = 100) answered the questionnaire a second time. Internal consistency, test-retest reliability, and construct validity (hypothesis testing and structural validity) were estimated. For construct validity, the scores of the questionnaire were correlated with the Neck Disability Index (NDI), and the Hospital Anxiety and Depression Scale (HADS), the Tampa Scale of Kinesiophobia (TSK), and the 36-item Short-Form Health Survey (SF-36).

    Results

    Internal consistency was determined by adequate Cronbach's α values (α > 0.70). Strong reliability was identified by high intraclass correlation coefficients (ICC > 0.75). Construct validity was identified by moderate and strong correlations of the Br-ProFitMap-neck with total NDI score (–0.56 < R < –0.71) and with the SF-36, HADS-anxiety, HADS-depression, and TSK (–0.32 < R < –0.82). Exploratory factor analysis revealed 2 factors for the Symptom scale: intensity index and the Function scale. Symptom scale–frequency index identified 1 factor. Structural validity was determined by percentage of cumulative variance >50%, Kaiser-Meyer-Olkin index > 0.50, eigenvalue > 1, and factor loadings > 0.2.

    Conclusion

    Br-ProFitMap-neck had adequate psychometric properties and can be used in clinical settings, as well as research, in patients with chronic neck pain.

    The majority of individuals appear to have insight into their own sleepiness, but there is some evidence that this does not hold true for all, for example treated patients with obstructive sleep apnea. Identification of sleep-related symptoms may help drivers determine their sleepiness, eye symptoms in particular show promise. Sixteen participants completed four motorway drives on two separate occasions. Drives were completed during daytime and night-time in both a driving simulator and on the real road.

    Ten eye symptoms were rated at the end of each drive, and compared with driving performance and subjective and objective sleep metrics recorded during driving. 'Eye strain', 'difficulty focusing', 'heavy eyelids' and 'difficulty keeping the eyes open' were identified as the four key sleep-related eye symptoms. Drives resulting in these eye symptoms were more likely to have high subjective sleepiness and more line crossings than drives where similar eye discomfort was not reported. Furthermore, drivers having unintentional line crossings were likely to have 'heavy eyelids' and 'difficulty keeping the eyes open'. Results suggest that drivers struggling to identify sleepiness could be assisted with the advice 'stop driving if you feel sleepy and/or have heavy eyelids or difficulty keeping your eyes open'.

    Background: ICD deactivation at end-of-life is technically uncomplicated. However, it may present a psychological challenge to healthcare professionals, patients, and next-of-kin. Objective: This study explored patients' experiences of complex issues of battery replacement and deactivation of the ICD. Methods: Semistructured interviews were administered to 37 medically stable ICD-recipients. Results: The ICD-recipients lived with an uncertain illness trajectory, but the majority had not reflected on battery replacement or elective ICD deactivation. Healthcare professionals had rarely discussed these issues with patients. However, this was consistent with the ICD-recipients' wishes. Many patients had misconceptions about the lifesaving capacity of the ICD and the majority stated that they would not choose to deactivate the ICD, even if they knew they were terminally ill, and it meant they would receive multiple shocks. Conclusion: The ICD-recipients tended not to think about end-of-life issues, which imply that many patients reach the final stages of life unaware of the option of ICD deactivation. © 2013 Elsevier Inc.

    Aim. To explore future reflections of spouses living with an implantable cardioverter defibrillator recipient with focus on the end-of-life phase in an anticipated palliative phase.

    Background. A history of or risk for life-threatening arrhythmias may require an implantable cardioverter defibrillator. Despite the life-saving capacity of the device, eventually life will come to an end. As discussion about preferences of shock therapy at end-of-life phase seldom takes place in advance, the implantable cardioverter defibrillator recipients may face defibrillating shocks in the final weeks of their lives, adding to stress and anxiety in patients and their families.

    Design. Qualitative study with in-depth interviews analysed with a content analysis.

    Methods. Interviews were performed with 18 spouses of medically stable implantable cardioverter defibrillator recipients during 2011–2012.

    Results. The spouses described how they dealt with changes in life and an uncertain future following the implantable cardioverter defibrillator implantation. Six subcategories conceptualized the spouses' concerns: Aspiring for involvement; Managing an altered relationship; Being attentive to warning signs; Worries for deterioration in the partner's health; Waiting for the defibrillating shock; and Death is veiled in silence.

    Conclusion. Despite the partner's serious state of health; terminal illness or death and the role of the device was seldom discussed with healthcare professionals or the implantable cardioverter defibrillator recipient. Open and honest communication was requested as important to support coping with an unpredictable life situation and to reduce worries and uncertainty about the future and end-of-life. © 2013 John Wiley & Sons Ltd.

    Övervägande delen av forskning kring socialtjänstens placeringar av barn och ungdom riktar sig mot barnens situation vid dessa insatser eller föräldrarnas upplevelser av skiftande familjebehandlingar. Det har heller inte påträffats forskning riktad mot föräldrars upplevelser av barn och unga placerade i HVB-hem typ behandlingshem.

     I aktuell studie delger sex föräldrar med barn, placerade enligt LVU, individuellt sina synpunkter och tankar kring samarbete med socialtjänsten och olika vårdgivare såsom familjehem och HVB - hem.

    Denna kvalitativa studie, med en tematisk analys på narrativ bas, visar att föräldrarna är i stora drag nöjda med det samarbete som pågår gällande sina barn och ungdomar placerade p.g.a. eget beteende. Föräldrarna har dock därmed inte avsagt sig föräldraskapet utan önskar fortsatt information och delaktighet beträffande sina barn. Studien tar också fäste på anknytningsteorin både ur barnet och förälderns synvinkel gällande fortsatt relation. Fokus ligger också på den förändrade föräldrarollen som informanterna önskar få hjälp att hantera. Det saknas stöd för föräldrarnas egen situation då kris samt skam- och skuldkänslor uppstår i samband med barnens placering.

    Denna studie, visar stora likheter med utfallet av den studie Allmänna Barnhuset ansvarat för, gällande föräldrars röster om hur det är att ha sina barn placerade i fosterhem. (Höijer, 2007)

    Abstract [en]

    Background: Reorganization and downsizing can disrupt a competent staff and conflicts can arise between what the employee is being asked to do and their knowledge and competences. Reduced job satisfaction among nursing home staff with increased workload and strain can occur. Aim and Objectives: The aim was to investigate the organizational climate and prevalence of burnout symptoms among caregivers over time in three Swedish nursing homes (NH I-III) undergoing organizational changes, while education and clinical supervision were provided. Design: The study design combines qualitative and quantitative methods in a longitudinal two-year follow-up project in NH I-III. Methods: Support through education and clinical supervision was provided for caregivers only at NH I and NH II. At NH I-III caregiver self-assessments and interviews were completed and analysed three different times. Results: NH I revealed improvement and increased innovation over time, while NH II showed a decline with no ability to implement new knowledge. NH III retained a more status quo. Conclusions: Organizational changes and cutbacks, occurring at different times, appeared to cause major stress and frustration among the three personnel groups. They felt guilty about not meeting their perceived obligations, seemed to have lost pride in their work but kept struggling. The changes seemed to over-shadow attempts to improve working conditions through education and clinical supervision initially. Implications for practice: It will be important to learn from reorganizations and the consequences they will have for the staff and quality of care. Important topics for future research are to study financial cutbacks and changes in organizational processes in care of older people to be able to develop a more person centered care for older people.

    Mental health inpatient milieus have repeatedly been found to be associated with passivity, social disengagement, and low levels of interaction with staff. However, little is known about patients' experiences related to different ward activities. In the present study, we aimed to study the reports of activities and associated experiences of patients admitted to acute psychiatric inpatient wards. Disengaged, inactive, and solitary activities were hypothesized to be associated with less reward and more distress than their counterparts. We also aimed to investigate if such activities predicted distress, and if they were associated with clinical severity. Participants (n = 102) recorded their activities along with concurrent ratings of reward and distress in a structured 1-day diary, and nurses provided clinical severity ratings. On average, 3.74 of the 11 hours assessed (34%) were spent doing nothing, only 0.88 hours (8%) were spent with staff, and most of the time was spent in solitude. Doing nothing, being alone, and passivity were associated with the greatest levels of distress and lowest levels of reward, whereas informal socializing demonstrated the opposite pattern. Distress was not predicted by activity or reward when adjusting for baseline distress. Clinical severity was not associated with the amount of time spent alone or the experience of reward during activity. In conclusion, the risk for passivity and social disengagement during admission prevails. This activity pattern could have detrimental emotional consequences and warrants action, but more studies are needed to determine if activity actually precedes emotional experience.

    Bakgrund: När en familjemedlem blir svårt sjuk har det en stor påverkan på hela familjen. Närstående har och känner ofta ångest, depression och maktlöshet. Familjemedlemmens sjukdom kan också leda till att rollerna i familjen ändras, och en förändring i den ekonomiska situvationen och en ökad påfrestning för familjen. Syfte: Syftet med studien var att belysa behov som närstående till patienter inom palliativ vård kan ha. Metod: En litteraturstudie genomfördes med sökningar i Pubmed och Cinahl. Innehållsanalys gjordes av de 11 inkluderade artiklarnas huvudresultat. Tydliga framträdande fenomen i texten kondenserades, kodades och katogoriserades i underkategorier och kategorier. Resultat: I studiens resultat framkom att närstående till patienter inom palliativ vård har ett stort behov av kommunikation, information och stöd. Diskussion: Att ha en fungerande kommunikation fanns var något som var viktigt för de närstående, då det gav dem en känsla av trygghet. Det var även viktigt att rätt infomation gavs till de närstående, att den gavs vid rätt tillfällen och att de närstående kunde förstå informationen.För att stödja närstående spelar vårdpersonal en viktig roll, då de kan bidra med uppmuntran och hopp i en svår situation.
