    Abstract [en]

    The psychometric properties of the Swedish version of the Adolescent Drug Abuse Diagnosis (ADAD) instrument were evaluated in two adolescent groups. The participants in the Normal group comprised 121 adolescents aged 15-17 years and the participants in the group of adolescents with antisocial problems comprised 1168 youths aged 10-21 years detained under the Swedish Care of Young Persons Act in special youth homes. The ADAD instrument produced good interrater reliability; the subscales showed moderate internal consistency and concept validity was satisfactory and comparable with American and Swiss versions. Finally, the ADAD subscales produced meaningful correlations. The interviewer rating, the adolescent’s rating and the composite scores are compared and discussed. The Swedish version of ADAD appears to be a psychometrically good instrument for assessing the severity of adolescent problems and their need for treatment. However, the composite scores need to be reconstructed to be useful in future research.

    Abstract [en]

    The concurrent and predictive validity of the Adolescent Drug Abuse Diagnosis (ADAD) in the Psychological status and problem area was examined for two adolescent groups: a non-clinical population of 121 Swedish adolescents (62 girls and 59 boys) aged 15 to17 years and a clinical population of 31 adolescents (14 girls and 17 boys) aged 12 to 19 years detained in special youth homes. Concurrent validity was demonstrated by significant correlations between the ADAD, Youth Self Report (YSR) and Beck Depression Inventory (BDI) scores. Findings regarding the predictive validity of ADAD show moderate associations with YSR, BDI and DICA problem ratings. The findings suggest that for clinical practice the ADAD Psychological status and problem area may be an appropriate assessment tool for measurement of current psychological problems. The utility obtained by making decisions using the test is substantial.

    Novice psychotherapists’ development in professional characteristics and work involvement styles in trainingManuscript (preprint) (Other academic)
    Abstract [en]

    Information about how psychotherapists develop their professional characteristics and work involvement styles during training is scant; in addition, awareness of the need to find answers to how psychotherapy training can best be organized is increasing. This study investigated novice therapists’ development of healing and stressful work involvement in baseline psychotherapy education in Sweden. Undergraduate students (n = 76) provided information longitudinally by responding to the Development of Psychotherapists Common Core Questionnaire (DPCCQ). The results demonstrated that therapists’ healing and stressful work involvement, current therapeutic skills, perceived difficulties, and constructive coping strategies changed positively and linearly. Technical expertise changed the most, having a large effect, and relational skills developed moderately. Surprisingly, in-session feelings of anxiety and boredom did not change. The process of positive and linear development of in-session feelings is important in psychotherapy education. The question becomes how the training should address trainees’ personal issues or countertransference that might affect in-session feelings.

    This study investigated changes in trainees’ self-rated experience as a therapist over the course of one practicum treatment case in basic psychotherapy education in Sweden. Undergraduate students (n = 76) provided longitudinal information on their healing involvement and stressful work involvement. The results of the Development of Psychotherapists Common Core Questionnaire (DPCCQ) demonstrated that trainees’ basic relational skills, technical skills, perceived difficulties, and constructive coping strategies changed linearly, with an increasing slope. Technical expertise changed the most, and relational skills developed moderately. In-session feelings of anxiety and boredom did not change. The individualized reliable change scores show that the process during training is different for different students. Most students did not change at all, and some students even changed negatively. Investigation of how pedagogic variables affect therapists’ development is necessary to support the professional growth of trainees in their involvement with different types of psychotherapy.

    Abstract [en]

    This study investigated the outcome of undergraduate trainee-led psychotherapy and how different self-image patterns explain symptom change. Pre- and post-treatment data from 235 Swedish outpatients were used. Clients were assessed with Symptom Checklist-90 (SCL-90) and the Structural Analysis of Social Behavior (SASB). Outcome effect size was moderate and in line with earlier studies on trainees. Clinical significant change showed that 42% of all clients were recovered or improved after 18 sessions in either training condition PDT or CBT. Regression analysis showed that a more negative self-image and higher levels of self-control before treatment predicted improvement in both psychiatric symptoms and personality factors. A negative self-image, when observed before treatment, can be understood as an increased motivation for change.

    Abstract [en]

    Objectives: This study investigated the relationships between client personality traits, and changes in those traits after cognitive behavioral or psychodynamic short-term therapy, and clients' perceived working alliances with their therapists and their clinical outcomes at a university training clinic in Sweden.

    Design: This was a longitudinal study, with the measures collected at pre- and post-therapy.

    Methods: The sample consisted of 138 clients with moderate psychological symptoms. Personality traits were measured using the Health-Relevant Personality Inventory, a health-relevant instrument that measures five factors.

    Results: The results showed that Antagonism, Impulsivity, Hedonic Capacity, and Negative Affectivity improved significantly during therapy, while Alexithymia did not. Pre-therapy personality traits were not related to perceived working alliances (as measured by the Working Alliance Inventory) or therapeutic outcomes (as measured by the Clinical Outcomes in Routine Evaluation Outcome Measure). Post-therapy personality traits negative affectivity, hedonic capacity and alexithymia were related to working alliance, and changes in personality traits were predictive of therapy outcome. The change in Hedonic Capacity and Negative Affectivity explained about 20% of the variance in post-therapy symptoms after controlling for pre-therapy symptoms.

    Conclusions: The results suggest that therapeutic foci on hedonism (extraversion) and negative affectivity (neuroticism) could be important for working alliance formation and symptom reduction in therapy. Future research should examine whether changes in clients' negative affectivity or hedonic capacity mediates the relation between perceived working alliance quality and clinical outcome in training and other psychotherapeutic contexts.

    This study examined how clients' self-image and perception of therapist's behavior are related to the therapeutic alliance, as well as how these variables change in the psychotherapy process in trainee-led psychotherapy. A total of 164 participants (M = 28.9 years) who attended two semesters of treatment at the Psychology Clinic of Umea University completed the Working Alliance Inventory (WAI) to evaluate their perception of their alliance with the therapist. They also completed two questionnaires based on the Structural Analysis of Social Behavior (SASB) model to give insight into their self-image patterns and to analyze their perceptions of the therapist's behavior. The results show that a positive self-image and positive perceptions of the therapist's actions increased significantly over the course of the therapy, with a corresponding decrease in negative patterns. The alliance scores show that the therapeutic alliance gains significance over time and that it is influenced more by the perception of the therapist's behavior than by the self-image. The self-image becomes relevant after the mid part of therapy, underlining the role of the therapist in co-creating the treatment relationship. The implications of these results are discussed, and so are directions for future research in other traineeled settings and samples.

    In this study, the Structural Analysis of Social Behavior (SASB) was  used to assess the self-image of psychotherapists enrolled in a psychotherapy supervisor training program in the initial and final phases of their training, as well as four months after the completion of their training. Their self-image was compared to that of experienced supervisors. The results show that the self-image of the supervisor trainees changed over the course of the training. By the end of the training, their self-image was as autonomous as that of experienced supervisors. Positive self-acceptance also developed. The relation between the outcome of psychotherapy, the style of trainees’ supervisor, and changes in the self-image of the trainees is also discussed.

    The aim of this study was to provide a standardization of the use of the Adolescent Drug Abuse Diagnosis (ADAD) among Swedish adolescents using interview self-reported and interviewer-rated problem severity. Data relating to nine life areas (medical status, school history and status, employment, social activities and peer relations, family background and relationships, psychological status and problems, legal involvement, alcohol use and drug use) were gathered from a randomized population-based sample of 847 adolescents aged 13–18 years. The results show that the effects of age, gender and geographic region were small but significant, with older adolescents, girls and adolescents in cities scoring higher for problem severity. Adolescents’ scorings were in the range “no problems” to “small problems” and the interviewer severity ratings were in the range “no real problems” to “no need for further help”. The conclusion is that individual score variations are much greater than can be attributed to age, gender or residential area. Consequently, the ADAD has the potential to serve as an instrument for assessing individual adolescents’ self-reported problems in Sweden.

    The aim of the study was to examine risk and vulnerability factors contributing to problems with alcohol use in adolescence. Data relating to seven life areas (medical status, school status, social relationships, family background and relationships, psychological functioning, legal involvement, and alcohol use) was gathered using the ADAD (Adolescent Drug Abuse Diagnosis) interview. A total of 1163 Swedish adolescents (809 boys and 354 girls) between the ages of 12 and 20 years old were interviewed. All had antisocial problems and were detained at special youth homes. It was found that for those aged between 12 and 18 years, the general risk factors for alcohol use were leisure and peer problems, problems associated with family background and relationships, and criminal behavior. These results suggest that drug abuse treatment planning should focus on altering the predisposing factors that exist in these domains. It was also found that the ADAD problem areas seem to be most useful as prognostic indicators of treatment outcome for adolescents with antisocial problems who are under 18 years of age.

    The aim of the study was to compare problem severity among Swedish adolescents, using self-reported and interviewer-rated data from 2000 and 2010, gathered with the Adolescent Drug Abuse Diagnosis (ADAD) interview. Data relating to family relationships, psychological status and problems were collected in two samples randomly selected from the adolescent population aged 15–17 years (121 adolescents in the year 2000 and 485 adolescents in the year 2010). The results show that the self-rated and interviewer-rated problem severity of adolescents in 2000 and in 2010 seems to be unchanged, with no increased polarisation for sex and socio-economic groups. There was a difference, however, was of girls reporting more severe problems in family relationships compared to boys. In 2010, compared to 2000, adolescents reported on fewer psychological problems (e.g. experiences of serious anxiety and tension, comprehension and concentration disorder, memory loss and, in addition, problems with relationships in and outside the family sphere – e.g. problems with getting along with siblings, and with trusting other people). In order to promote the mental health of adolescents, it is essential, during the next decade to reveal relationship problems, such as problems of insecurity with people outside the family.

    A model of the relation between self-concept and internalizing and externalizing problem behaviours in adolescence, with the self-concept influencing problem behaviours (S→IE), was assessed using a sample of 277 Swedish adolescents. The model was tested in a path analysis with data from Youth Self Report (YSR) and Structural Analysis of Social Behaviour (SASB) questionnaires. Consistent with the model, a positive self-concept was found to be the most important factor for adjustment and for protection against common problem behaviour. A negative self-concept combined with female gender were risk factors for internalized problems. Self-control had only a direct effect on externalizing behaviour for boys. Adolescents of 15, 16 years of age had a stronger relationship between a negative self-concept and externalizing problem behaviour than younger and older adolescents. Internalizing problem behaviours such as anxiety and depression predicted aggressive and delinquent behaviour. These findings highlight the importance of promoting of a positive self-concept in every adolescent in various psychosocial contexts.

    The relation between images of self and parents as defined in interpersonal theory and by the Structural Analysis of Social Behaviour (Benjamin), and internalizing and externalizing problems, as defined by the Youth Self-Report (Achenbach) was studied in a sample of 199 nonclinical adolescents (92 boys and 107 girls) aged between 13 and 17 years. Regression analyses showed that the pattern relating images of self and parents to mental health problems was different for boys and girls. Internalizing problems for girls were best predicted from aspects of their self-image while for boys parental behaviour played a larger role. A pattern of self-blame and lower self-affirmation together with parental blame was important for girls’ externalizing problems and a pattern of self-neglect and low self-autonomy and an uncertainty of parental love were important for boys externalizing problems. Results emphasize that boys and girls have different kinds of vulnerabilities as risk factors for mental health problems.

    This study examined whether self-esteem mediates the association problems in adolescents. A total of 204 Swedish adolescents aged between 12- and 16-years-old completed self-report measures; self-esteem was assessed with ‘I think I am’ (ITIA) and internalizing and externalizing problems with Youth Self-Report (YSR). Our results showed that internalizing problems in peer aggressors could be understood as problems with low self-esteem, while internalizing problems in peer victims could be understood both as problems with low selfesteem and as an effect of being victimized. Externalizing problems in peer aggressors and peer aggressor–victims could be understood as problems with being involved in peer aggression, while the same problems in peer victims could be understood as problems with low self-esteem. This article also discusses the importance of self-esteem in adolescents’ development of their own identities and for peer aggressive- related psychological psychopathology.

    The present study evaluates the utility and issues around the composite scores in Adolescent Drug Abuse Diagnosis (ADAD). Participants were Swedish normal adolescents (121) and adolescents manifesting antisocial problems (1,168), ages 10 to 21. The results show that the interviewer severity ratings seem to be the most appropriate outcome when the objective is to differentiate between normal and antisocial adolescents. The composite scores appear to function as an indicator of current problems in all areas except for Medical and Alcohol sections. The critical items within the Medical and Alcohol composite scores are explored and discussed.

    The shape of alliance in psychotherapy and supervision using growth curve modeling was examined for clinically inexperienced trainee therapists, who were engaged in long-term cognitive behavioral - or psychodynamic individual psychotherapy at a Psychology Clinic in Sweden. Trainee therapists rated their view of the alliance with their clients and their supervisors on the Working Alliance Inventory at five time points. The alliance to the client show a consistent, positive development throughout the therapy. The alliance to the supervisors show initially a decrease, followed by an increase, and then again, the rate of increase in the supervisory alliance ratings slowed down indicating a more complex view of the supervisory alliance compared to the view of the alliance in psychotherapy.

    This study examines the self-concept and perceptions of early parental behavior in a group of 277 normal adolescents with respect to differences in age (12–18 years) and gender. A group of 30 antisocial adolescents of similar age (13–19 years) were included as a comparison group. The study was a cross-sectional questionnaire study and the Swedish translation of the Structural Analysis of Social Behavior (SASB) introject and mother/father questionnaires were used as measures of self- and parental concepts. Multivariate analyses of variance show that the normal adolescent self-concept and perception of early parental behavior are positive and that there are no age or gender differences. The early behavior of the mother is perceived more positively than that of the father. A positive self-concept is related to a positive perception of the parent's early behavior. The antisocial group of adolescents shows a more autonomous and negative self-concept and more negative perception of early parental behaviors. This was true especially in the group of antisocial girls. The conclusion from the study is that there is no evidence of a chaotic self-concept or tumultuous changes in the perceptions of early parental behavior during normal adolescence, but that an antisocial way of life might be reflected in a more negative and autonomous self-concept and in more negative perceptions of early parental behavior during adolescence.

    This study examined age and gender differences in the perceptions of early mother and father behavior in a group of normal adolescents. Two hundred and seventy-seven boys and girls (aged 12–18 years) completed EMBU (My memories of Upbringing), a questionnaire measuring perceptions of parental rearing behaviors, and questionnaires based on the Structural Analysis of Social behavior (SASB) model. A group of 30antisocial adolescents of similar age was included as a comparison group.The results show that the normal adolescents’ perception of earlyparental behavior is positive with no age or gender differences. The earlybehavior of the mother was perceived more positively than that of thefather. A more negative perception of early parental behaviors was foundin the antisocial adolescent group and especially in the group of antisocial girls. The results of EMBU were consistent with those of SASB with perceptions of more negative parental behavior of rejection and overprotection in the group of antisocial adolescents.

    Self-concept in adolescence.: A study of age and gender differences in groups of normal and antisocial adolescents2003Report (Other academic)

    This study examines the self-concept in a group of 277 normaladolescents with respect to differences in age (12-18 years) and genderand the interaction between age and gender. As a comparison, 27antisocial adolescents of similar age participated in the study. Theadolescents completed a questionnaire based on the SASB-model. Theresults showed that the normal adolescent self-concept is positive andthat there are no age or gender differences. A more autonomous andmore negative self-concept is found in the antisocial group ofadolescents. The antisocial girls seem to show more self-hate than allother groups. The conclusion from the study is that there is no evidenceof a chaotic self-concept during normal adolescence, but that anantisocial way of life might be reflected in a more negative and autonomous self-concept during adolescence.
