Intersectorial cooperation is commonly regarded as a desirable development in order to achieve increased efficiency and quality, and to reduce redundancies. In this report the experiences are presented from a project where nurses, midwives, preschool teacher and social worker integrated their activities around families and children. The results show that although all involved personnel were confident that the integration was favorable, they all experienced tensions, due to flaws in administrative and managerial procedures, perverse economic incentives, differences in professional background and organizational culture and personal preferences. This study indicates that integrative policies need to be followed by determined implementation.
Barnens hälsa påverkas redan tidigt i livet av föräldrarnas livsvillkor. Låg social status, brist på vänner och stress tidigt i livet leder till hälsoskillnader. Familjecentraler med en öppen förskola och förebyggande socialt arbete är en tidig insats och en lågriskstrategi som riktar sig till alla. Den har goda förutsättningar för att stärka föräldrarnas tillit och gemenskap med andra och kan bidra till att minska hälsoskillnader genom att främja barnens hälsoutveckling. Syftena med utvärderingen av de 16 familjecentralerna var att undersöka i vilken mån besökarna är representativa för befolkningen i upptagningsområdet och av vilka anledningar föräldrarna besöker öppna förskolan på familjecentralen. Jämförelse mellan befintlig befolkningsdata i upptagningsområdena och föräldrabesökares bakgrundsdata visar att familjecentralen i huvudsak når en representativ del av befolkningen. Föräldrar använder sig av familjecentralen för att få råd, utbyte av erfarenheter, barnens skull och att umgås med andra. I synnerhet för invandrare har familjecentralen stor betydelse för att komma in i det svenska samhället.
Familjecentralens verksamhetsidé för öppna förskolan på familjecentraler – formulerat som en hypotes - har utvärderats steg för steg genom samla in data på sammanlagd sex familjecentraler för att pröva hållbarheten i hypotesen. Utvärderingen visar på ett föräldraskapande bland småbarnsföräldrar genom att de lär sig om barn och föräldraskap, får möjlighet att utöka sitt sociala kontaktnät och att bara vara i en trygg omgivning där socialt stöd finns till hands. De får tillsammans med andra känna sig ’good enough’ som en vanlig förälder med ett normalt barn. Professionell följsamhet utmärker personalens arbetssätt. De bedömer den ”mognadsgrad” som föräldern befinner sig i för att söka stöd och anpassar sitt förhållningssätt till föräldern genom fysiskt avstånd och djup i samtalet. På så vis kan pedagogiska, sociala och psykologiska mekanismer triggas igång och förklara de resultat som föräldrar upplever för dem själva och deras barn. Artikeln avslutas med en omformulerad hypotes om öppna förskolors verksamhet på familjecentraler.
På senare år har ett antal program som t.ex. Community Parent Education Program (COPE) initierats för att främja barns hälsa och psykosociala utveckling. COPE är ett manualbaserat program som ofta används som del i de föräldrautbildningar som ges i invandrartäta områden. COPE utvecklades i Kanada och vid användningen i Sverige översätts materialet. Program erbjuds på språk som t.ex. arabiska och somaliska och när dessa kurser ges översätts materialet ytterligare. I artikeln undersöks vad som händer när ett manualbaserat program översätts och implementeras i svensk kontext. Studien visar att de samtalsledare som leder programmen strävar efter manualtrogenhet, men att översättningar och kulturella anpassningar sker för att de utrikesfödda deltagarna ska förstå programmet. Många föräldrar är dock nöjda med och känner sig stärkta i sitt föräldraskap efter att ha genomgått COPE programmen.
I artikeln ”aggressiv musik och coping” diskuteras aggressiv musik (heavy metal, hårdrock, punkrock, hård rap och aggressiv popmusik) som copingstrategi vid den rollkonflikt som kan uppstå i samband med att någon drabbas av cancer. Detta eftersom den sjukroll som tenderar att tillskrivas individen i många fall strider mot den roll personen själv önskar inneha. Diskussionen förs utifrån två fallbeskrivningar hämtade från en studie som fokuserar på musikens roll för personer som drabbats av cancer. I analysen av de två fallbeskrivningarna framgår att aggressiv musik har fungerat som en copingmetod genom att hjälpa informanterna att hantera den ovan nämnda rollkonflikten.
Emellertid visade analysen på att ålder kan vara en aspekt av betydelse beträffande vid vilken tidpunkt som ett motstånd gentemot sjukrollen växer fram och en konflikt mellan rollerna uppstår. Det är viktigt att se att en copingstrategi är en mötesplats mellan individ och situation. Den är komplex, mångbottnad och högst kontextuell. Diskussionen i denna artikel, vilken förs utifrån detta synsätt, visar på att aggressiv musik, som ofta förknippas med

In light of four welfare principles are family centres analyzed as they emerge in Sociomedical Journals special issue on family centers. The principles are: 1. welfare as a social compensatory 2. welfare as a social guarantee 3. welfare as social services, and 4. welfare as social investment. The question is whether and how they compete for attention and what impact it may have. The results shows that three of the four welfare principles appear as complementary in the vision of family centres while they appear as being in conflict when the texts are dealing with professionals everyday work. The organizational form, the form of family centres, seems to help the professionals in their dealing with conflicts between different principles in some types of work situations.
To find out the patients’ perceptions of the care and welfare services is important in order to develop the quality of care. The purpose of this study was to investigate the patients’ perspectives on care on equal terms and on their ideas about methods for a more equitable care. To achieve the purpose, qualitative methods were used in form of conducting interviews with patients’ and representatives of patient organizations. The results show that patients’ perceive that in order to achieve a more equitable care continuity, time during consultations, availability, communication and encounter should evolve and become better.
Equity in health and providing care on equal terms is one of the most important aspects of a welfare society. Several reports indicate that the health care services are not equal. The purpose of this study was to study health care providers’ perceptions on equitable care and their ideas about methods to achieve a more equitable care. An web survey was sent to health care providers whose unit were involved in the project "Care on equal terms" before and after the project period. The results show that health care providers perceived that the factors that influence clients’/ patients’ access to care on equal terms consists of: structural / organizational factors in the health care organization or in the society and clients’/ patients’ social situation.
The purpose of this article is to describe how the qualitative target “men’s violenceagainst women and violence in close in close relationships” has beenimplemented and integrated into the nursing program at Linnaeus University.Another purpose is to contribute with ideas and didactic methods within thefield of knowledge. The introductory part provides a brief background to theGovernment’s decision to add a new qualitative target in the Higher EducationOrdinance for seven vocational educations in 2018. Thereafter it ispresented how the qualitative target is implemented and integrated in threeof the nursing program’s courses. The description focuses on the work ofsystematically designing teaching and learning so that the courses’ examinationsare linked to learning outcomes, type of instructions and progression.
Barn till allvarligt somatiskt sjuka föräldrar löper en förhöjd risk för egen psykisk ohälsa. Här presenteras resultat från utvärderingen av implementering av rutiner och arbetssätt som syftade till att förbättra arbetet med att identifiera, informera samt stödja minderåriga barn till patienter på en neurologisk klinik. Fyra delstudier genomfördes. Resultaten indikerar att trots betydande insatser för att säkerställa att kliniken lever upp till sina lagstadgade uppgifter, fanns en stor förbättringspotential. Hela 80% av patienter med minderåriga barn hade inte fått information om sina barns rätt till stöd och 40% hade inte tillräckligt stöd för att utöva sin föräldraroll. Samtidigt noterades en tydlig ökning i andelen personal på kliniken som hade kunskap om sin skyldighet att erbjuda information, råd och stöd.
Research indicates an intersection between sport and patriarchal ideals andthe risk, to some individuals, that these will make them violent towards womenand other men. Against this background, the purpose of this article isto investigate the relationship between masculinity ideal, violence and sportand the possibilities for intervention. Theoretically, the article is inspired bythe concept of ‘the dominant hegemony’s masculinity ideal’. The argumentrevolves around the need to develop an understanding of violence in sportswith physical contact that is permeated by a critical gender perspective thatalso characterises the preventative initiatives. Based on previous researchand recent work within the Swedish sports movement, the article closes withoutlining and discussing a gender-conscious, violence-preventive model forthe sports movement.
This article problematizes the relationship between teachers as an occupational group and the security- and crisis management field, and argue that this relationship can be expected to influence the former’s working environment. Drawing on a literature study, the analysis finds two major potential challenges: a) work environment problems related to the new teacher’s role and b) gender aspects highlighted through this connection. This occupational group may be faced with new tasks that have traditionally been managed by rescue services or police. In this context notions of gender are actualized. Female dominated occupational groups risk being subordinated. Municipal resource allocation processes has so far excluded teachers in relation to security and crisis-management tasks, which can increase problems in relation to work environment.
The article problematizes decision-making in the health and care sector during the Covid-19 pandemic. This is done deductively by applying an empi-rically developed decision model to new data that consists of media articles. The decision model can be divided into two dimensions; an employee-orien-ted and an organization-oriented. Each dimension indicates the direction of the decision and ranges from anti to pro. This means that decisions can be made pro/anti employees/organization. We have found evidence for the models’ various dimensions, which are the basis of an analysis of the con-sequences decisions may have on employees, the working life and the civil society as a whole.
Few Swedish studies investigated experiences of childhood maltreatment inadolescents with substance use problems, especially in outpatient care. Thisstudy analyzed experiences of neglect and abuse during childhood in adolescents(mean age 18 years) who started outpatient treatment for substanceuse problems. The results showed that several of them had experience ofchild maltreatment. Child maltreatment was also related to perceived problemsin the upbringing family, e.g. financial difficulties, substance use problems,mental illness and violence in the family. This indicate the importanceto investigate experiences of child maltreatment among adolescents in treatmentfor substance use problems, especially since there is evidence todaythat psychiatric co-morbidity (e.g. trauma reaction and drug problems)should be treated simultaneously to achieve the best effect.
This article highlights the obstacles and opportunities in implementing a screening method to identify problem gambling among persons receiving income support within the social services in Gothenburg. The empirical ma-terial was collected through both a study of problem gambling with the two screening instruments NODS and NODS-PERC, and five interviews with ad-ministrators. The study shows that it is possible to identify problem gamblers in social services with both of the screening interviews, but it also illustrates the difficulties that existed under the implementation of this new approach in the social services.
Studien belyser Coronapandemins eventuella betydelse för de 658 ungdomar som påbörjade behandling för alkohol- och narkotikaproblem på Maria-mottagningarna i Stockholm, Göteborg och Malmö år 2020 vad gäller droganvändning och psykosocial situation jämfört med uppgifter för 2016-2019. I studien observeras flera parallella förändringar kring ungdomarnas alkohol- och cannabisanvändning som kan hänga samman med förändrade umgängesmönster till följd av en ökad social distansering under pandemin. Dessa positiva indikationer kan ställas mot en befarad ökad psykisk ohälsa de kommande åren.
Adolescent who use cannabis, also drink alcohol frequently. This article il-lustrates how risky alcohol consumption among adolescents entering treat-ment at the Maria clinics in Stockholm, Göteborg and Malmö for abuse of cannabis, is related to drog use patterns and to different background and risk factors. The study is based on structured interviews with 887 adolescents and the analysis distinguishes nine different risk factors that are strongly related to their combined use of cannabis and alcohol. A conclusion is that it is important to pay attention to adolescents risky alcohol consumption and, if necessary, also providing specific interventions targeting alcohol problems.
There is a lack of knowledge about the adolescents who are in contact with the outpatient care in Sweden for alcohol and drug problems. The purpose of this article is to describe the adolescents who receive treatment at Maria outpatient care in Stockholm, Gothenburg and Malmö, on the basis of an empirical material from the structured interview UngDOK. The descriptive cross-sectional study is based on a material on 755 adolescents who had begun a treatment contact. The study highlights the importance to include several risk and protective factors in the screening and assessment instruments addressed to adolescents entering care and treatment for substance abuse problems, to identify and pay attention to these factors in the treatment process.
Från ett tillstånd som tidigare har kopplats till en relativt begränsad andelbarnoch ungdomar har ADHD-diagnosen nu fått en större roll och betydelsei detsvenska samhället. Syftet med denna artikel är att analyseraoch diskuteraförekomsten av ADHD-diagnoser hos ungdomar som påbörjaröppenvårdför alkohol- och narkotikaproblem och hur diagnosen kan relaterastillsocialaförhållanden, droganvändning och psykisk hälsa. Studien baseraspå struktureradeintervjuer med 2169 ungdomar från nio städer för åren2013, 2014 och 2015. Resultaten visar att en betydande andel ungdomarsom vändersig till öppenvård också har en ADHD-diagnos, att det har sketten stor ökning de senaste åren, att det är en stor lokal variation och attdennagrupp ungdomar är psykosocialt utsatta med omfattande probleminom många livsområden. Den ökade förekomsten bland både pojkar ochflickor och den stora lokala variationen väcker frågor om ADHD-diagnosersorsak, tillförlitlighet och konsekvenser.
People with low incomes seek financial support via Facebook and other social media. For what is the assistance requested and which individuals seek help? 30 applications from Giving People showed that families with children seek help for basic needs such as food and clothing. This help is compared to help given from the charity organization Sveriges Stadsmissioner and to financial assistance from the social services. The help from Giving People is fast and the organization deliberately circumvents the social services’ requirements for reporting income. The help-seeking individual receives a small supplement but with a risk that poverty in society will not receive enough attention. Sveriges Stadsmissioner and the social services offer professional personal meetings, unlike Giving People. This can be important because financial difficulties are often a marker for other social problems.
När personer som är utsatta för våld i nära relation lämnar förövaren kan skyddade boenden erbjuda en fristad. Dessa boenden har dock olika förutsättningar att möta utsatta vuxnas och barns behov av skydd och stöd. I denna artikel beskriver vi olika utmaningar som skyddade boenden står inför idag och diskuterar de idéburna organisationernas betydelse. Detta görs med utgångspunkt i Nancy Frasers teorier om social rättvisa och socialt skydd. Artikeln visar att den kunskap som finns inom idéburna organisationer är betydelsefull oavsett om skyddade boenden drivs av idéburna organisationer, kommuner eller vinstdrivande aktörer. Dessutom behöver finansieringen säkras för att samtliga skyddade boenden ska ha förutsättningar för att kunna möta de utsattas behov.
Syftet var att undersöka stigma- och skamupplevelser bland långtidssjukskrivna i psykiska och muskuloskeletala diagnoser och om upplevelserna skiljer sig diagnoserna mellan. 2300 personer besvarade en enkät och på basis av enkätsvar bildades även en grupp med båda slagen av besvär. Bland de med psykiatriska och kombinationsdiagnoser tillhörde 33 respektive 34% hög-skamgruppen jämfört med 10% bland de med muskuloskeletala diagnoser. Ju fler tidigare sjukskrivningar desto större andel upplevde skam och stigma och desto vanligare att se förtidspension som en realistisk framtid. Nivåskillnaderna var stora. Bland de med psykiatriska diagnoser och hög grad av stigma var det 16%, som angav förtidspensionsalternativet jämfört med 31% bland de med muskuloskeletal diagnos och 39% bland de med kombinationsdiagnos.
Handledning i omvårdnad startade bland sjuksköterskor inom den psykiatriska vården i England. Under 1960-talet började handledning av sjuksköterskor införas på en psykiatrisk avdelning vid Ullevåls sjukhus i Norge. Utvecklingen av handledning i omvårdnad/yrkesmässig handledning i omvårdnad har sedan dess spridits via den psykiatriska vården till den somatiska (Hermanson m.fl., 1994), och kom i slutet av 1980-talet även till Sverige.
The aim is to propose several concepts as tools to interpret and mess with the management and organization of collaboration in the making. To interpret means that the concepts help practitioners to term phenomena for which they previously did not have a language. To mess means learning to see new things that also challenge, both past beliefs and conclusions and previous research. Instead of describing models, systems or similar support for action, we have focused on concepts that can be helpful in understanding and developing complex processes of collaboration in the making.
This article examines Swedish health research regarding the Indigenous Sami people and the lack of epidemiological data. A historical overview underlines that Swedish research is hampered by Sami people having been the subject of studies by the National Institute of Racial Biology between 1920 and 1950. Public health research has mainly taken place during the last 20 years. There are no current epidemiological studies and this is linked to the fact that Swedish official statistics do not report on ethnic groups in the country. Ethnic statistics is a sensative issue and new ethical principles need to be worked out where the Sami, as constitutionally recognized indigenous people, are allowed to decide whether they want to be part of official statistics, and if so, what statistics would be relevant and how it should be collected.
The aim of this article is to contribute to the dialogue between socio-medical and gender theoretical approaches to parenting support. The article draws on a text analysis of how fathers are described in three Swedish PhD dissertations on fathers’ experiences of parenting support. The analysis identifies four portrayals of fathers: 1) as excluded; 2) as in need; 3) as ambivalent agents in meetings with parenting professionals; 4) as part of a ‘we’. These portrayals of fathers are analyzed in relation to how gender theoretical perspectives and concepts are applied in the dissertations. The article suggests a deepened dialogue between socio-medical and gender theoretical perspectives, which in turn may contribute to a gender equal and gender equality supportive parenting support.
I denna temaledare beskrivs temanumret "Maskulinitet i förändring" som helhet. Detta temanummer tar sin utgångspunkt i den nationella konferensserien med samma namn som temanumret. Konferensserien har arrangerats av Länsstyrelsen i Örebro län sedan 2010. Konferensserien återfinns i skärningspunkten mellan jämställdhetspolitik, jämställdhetsarbete och ett av genusforskningens underområden: kritiska studier av män och maskuliniteter. I temanumret uppmärksammas konferensen i sig själv – som ett exempel på samverkan mellan forskning, politik och praxis. Utöver detta innehåller temanumret artiklar som belyser olika aspekter av de teman som konferensserien tagit upp genom åren: våld, hälsa, idrott, utbildning, makt, föräldraskap, sexualitet, politik, kriminalitet och klimat.
Developing a program theory for a project and evaluate the project based on this program theory has both its merits and limitations. These merits and limitations in turn may be depend on the existing attitude towards a particular program theory. This study is about exploring how different phases and parts of the national learning project Care on equal terms (2011-2014) functioned in relation to the program theory developed in the beginning of the project. The result shows that the program theoretical perspective on the project contributed to a better structuring of the various forms of evaluation, given a broader understanding of the project's components and provided better opportunities to improve monitoring of the project by the research team in collaboration with the project management of the Swedish Association of Local Authorities and Regions (SALAR) and the participating health care units.
Change-projects can be evaluated in different ways. One of the ways is to evaluate with process thinking, which means that evaluators are involved in the process from the very beginning. Involvement of evaluators already in the beginning of a project can mean several challenges for both the project itself and the evaluators. This study is about describing and analyzing how and with what results the perspective of process thinking on evaluation of the national learning project Care on equal terms was used. The results indicate, among other things, that it was a good choice to bring in evaluation perspective at the beginning of the project. Process thinking has, among other things, contributed to the activities throughout the project, and played a role in learning from and implementation of the project itself.
Adolescence is a sensitive time of development and sleep is important for optimal functioning. Sleep changes drastically during this time and many adolescents do not sleep enough. Although sleep deprivation may be temporary, even short-term consequences can be detrimental. Therefore, promoting sleep health in this age group is warranted. Sleep promotion may aim to change adolescents’ sleep behaviors directly or by postponing school start times to ensure sufficient sleep duration. School-based programs that aim to improve adolescents’ sleep habits have shown mixed results but have preliminarily shown long-term benefits. Delaying school-start times has shown clear increases in sleep. However, it is unclear whether the results can be generalized to Swedish schools, which already start at recommended times. Therefore, teaching adolescents how to achieve healthy sleep habits might be a better strategy.
