Abstract [sv]

Syfte: Att beskriva anestesisjuksköterskors syn på samvete, grad av samvetsstress och analysera samband mellan dessa variabler.

Bakgrund: Anestesisjuksköterskan ställs ofta inför etiska situationer som är potentiella källor till samvetsstress. Ett positivt samband mellan samvetsstress och utbrändhet finns. Sjuksköterskans egen syn på samvete kan påverka graden av samvetsstress. Inga studier som beskriver grad av samvetsstress och syn på samvete hos anestesisjuksköterskor har hittats.

Design: Studien är en beskrivande tvärsnittsstudie.

Metod: Datainsamlingen skedde hösten 2015. 34 anestesisjuksköterskor besvarade samvetsstresskalan (SCQ) och skalan för syn på samvete (PCQ). SCQ analyserades med T-test och Cohen´s d, PCQ analyserades med Fisher’s exakta test. Sambandsanalys utfördes med Spearman’s rho.

Resultat: Majoriteten anestesisjuksköterskor kan uttrycka vad deras samvete säger och kan följa sitt samvete på arbetet. De skattar betydligt lägre grad av samvetsstress jämfört med tidigare studier. Anestesisjuksköterskor som arbetar heltid/dagtid skattar högre grad samvetsstress vad gäller att inte kunna leva upp till andras förväntningar på sin arbetsinsats jämfört med de som arbetar deltid/skiftgång. Det finns ett positivt samband mellan anestesisjuksköterskors skattningar av samvetsstress och att uppfatta sitt samvete som en börda.

Konklusion: Att vårda en patient åt gången och inte behöva prioritera mellan olika patienters behov kan möjligen vara en skyddande faktor mot samvetsstress. Fortsatt forskning krävs för att undersöka vilka faktorer som verkar skyddande för anestesisjuksköterskor. Enkäterna för syn på samvete och samvetsstress behöver omarbetas och anpassas till anestesisjuksköterskors arbete.

Abstract [en]

Aim: To describe nurse anaesthetists’ perception of conscience, degree of stress of conscience and to analyse the relationship between these variables.

Background: The nurse anaesthetist is often put in ethical situations that are potential sources to stress of conscience. There is a positive connection between stress of conscience and burnout. The nurse’s own perception of stress of conscience can affect the degree of stress of conscience. No studies were found to describe degree of stress of conscience and perception of conscience among nurse anaesthetists.

Design: The study is a descriptive cross-sectional study.

Methods: The data was collected during autumn of 2015. 34 nurse anaesthetists answered the stress of conscience questionnaire (SCQ) and the perception of conscience questionnaire (PCQ). SCQ was analysed with T-test and Cohen’s d, PCQ was analysed with Fisher’s exact test. Correlation analysis was performed with Spearman’s rho.

Results: A majority of nurse anaesthetists could express what their conscience says and can follow their conscience at work. They estimated considerably lower degree of stress of conscience compared to earlier studies. Nurse anaesthetists working fulltime/daytime estimates higher degree of stress of conscience when not being able to live up to others expectations on their work effort compared to those working part-time/shifts. There is a positive connection between nurse anaesthetists’ estimations of stress of conscience and receiving their conscience as a burden.

Conclusions: To care for one patient at a time and not having to prioritise between different patients’ needs could be a protective factor against stress of conscience. More research is needed to examine which factors are protective for nurse anaesthetists. The questionnaires perception of conscience and stress of conscience need to be developed and adapted to the work of nurse anaesthetists.
