Abstract [sv]

Fibromyalgi är en sjukdom som med sin karaktäristiska smärta och många tillhörande symtom orsakar stort lidande för de drabbade. Orsaken till sjukdomen är ännu inte helt klarlagd. Central sensitisering har på senare tid fått ökat stöd i forskningen om fibromyalgi och innebär att det är ett onormalt smärtprocessande i det centrala nervsystemet som orsakar den ökade smärtan. Samband och överlapp mellan fibromyalgi och miljökänsligheter antyder att en mer generell sensorisk överkänslighet grundad i central sensitisering kan vara en gemensam bakomliggande faktor. Sensitiseringen är också relaterad till kognitiva aspekter som oro och uppmärksamhet. Syftet med denna studie var att undersöka miljökänsligheter och oro för att moderna miljöfaktorer påverkar hälsan negativt (modern hälsooro) hos personer med fibromyalgi, för att pröva teorin om central sensitisering. Studien använde data från Miljöhälsostudien i Västerbotten, en stor populationsbaserad enkätstudie fokuserad på miljökänsligheter. En enkät skickades till 8520 personer (18 - 79 år) i Västerbotten, slumpmässigt utvalda och stratifierade för ålder och kön. Av de 3406 personer som svarade (40 %) rapporterade 75 att de var diagnostiserade med fibromyalgi. Dessa jämfördes med deltagare utan fibromyalgi. Resultatet visade statistiskt signifikanta överlapp i prevalens mellan fibromyalgi och miljökänsligheter. Personer med fibromyalgi var i högre grad besvärade av lukt, el och ljud än personer utan fibromyalgi och de hade även mer modern hälsooro. Resultaten stödjer teorin om att personer med fibromyalgi är drabbade av en mer generell sensorisk överkänslighet grundad i central sensitisering.

Abstract [en]

Fibromyalgia is a disorder that with its characteristic pain and many associated symptoms causes great suffering for those affected. The cause of the disease is not yet fully understood. Central sensitization has recently gained support in research of fibromyalgia and means there is an abnormal pain processing in the central nervous system causing the increased pain. Associations and overlaps between fibromyalgia and environmental sensitivities suggest that a more general sensorial hypersensitivity based in central sensitization might be a mutual underlying factor. Sensitization is also related to cognitive aspects such as worry and attention. The aim of this study was to examine environmental sensitivities and worry that modern environmental factors affect the health negatively (modern health worry) in persons with fibromyalgia, to test the theory of central sensitization. The study was using data from the Västerbotten Environmental Health Study, a large population-based questionnaire study focused on environmental sensitivities. A questionnaire was sent to 8520 individuals (18 – 79 years) in Västerbotten, randomly selected and stratified for age and sex. Of the 3406 responders (40 %) 75 reported they were diagnosed with fibromyalgia. These were compared to responders without fibromyalgia. The result showed statistically significant overlaps in prevalence between fibromyalgia and environmental sensitivities. Persons with fibromyalgia were in a higher degree bothered by smell, electricity and sound than persons without fibromyalgia and they also had more modern health worries. The results support the theory that persons with fibromyalgia are suffering from a more general sensorial hypersensitivity based in central sensitization.
