Abstract [sv]

Bakgrund: Inom ambulansorganisationen ordineras läkemedel via telefonordinationer eller genom generella direktiv som erhålles från ambulansöverläkaren. Ordinationer enligt generella direktiv ska utfärdas restriktivt och omprövas regelbundet. För att patienterna ska bedömas på samma sätt så behövs utarbetade bedömningsmallar och riktlinjer som sjuksköterskorna som arbetar i ambulanssjukvård kan arbeta utefter.

Syfte: Att studera hur sjuksköterskor upplever läkemedelshantering inom ambulansorganisationen Västernorrland.

Metod: Samtliga sjuksköterskor inom ambulanssjukvården i Västernorrlands län tillfrågades om deltagande. Totalt kom 74 individer (54%) att delta. Datainsamlingen genomfördes med enkät bestående av slutna frågor och möjlighet att skriva kommentarer till varje fråga.

Resultat: Det framkom att personalen aldrig överskred maxdosen, men gav på eget initiativ läkemedel som de saknade generell ordination på utan att först kontakta läkare. Personalen tyckte inte att läkemedelsinnehållet i ambulanserna skulle vara olika men kände att de saknade vissa läkemedel, så som aktivt kol. Trots att flertalet uppgav att de visste vilken läkare de skulle kontakta för läkemedelsordinationer så var svaren väldigt olika då de skulle ange vilken denne var.

Diskussion: Cirka en tredjedel av personalen gav någon gång läkemedel som de saknade generell ordination på, detta tros vara en handling av personalen som skulle gynna patienten så att de slapp vänta på exempelvis smärtlindring. Drygt hälften tyckte att de generella ordinationerna skulle vara anpassade till vilken typ av utbildning personalen hade. Det tror vi skulle vara bra för att kunna utnyttja den kompetens som finns i ambulansen. 

Abstract [en]

Background: Within the ambulance organization drug prescriptions are prescribed by phone or by general directives received from the ambulance physician. Prescriptions by general directives should be prescribed restrictively and be regularly reviewed. In order to assess patients equal it is needed to develop assessment templates and guidelines that nurses who are working within the emergency medical services can work along with.

Aim: To study how nurses experience handling of drugs within the Västernorrland ambulance organization.

Method: All nurses within the Västernorrland ambulance healthcare was asked to participate. In total 74 individuals (54%) participated. The collection of data was done by a survey containing closed questions and the opportunity to add a comment to each question.

Result: The survey showed that the nurses never exceeded the maximum dose but took initiative by their own and gave the patient drugs that had no general prescription without first contacting a doctor. The nurses did not think that the contain of the drugs inside the ambulances should be different but they felt that they missed certain drugs such as active carbon. Although most of them said that they knew which doctor to contact for drug prescriptions the answers where very different when they were asked to specify who it was.

Discussion: Approximately one third of the staff sometime gave medicines they had no general prescription for, this is believed to be an act of the staff that would benefit the patient so that they did not have to wait for example pain management. Just over half thought that the general prescriptions should be tailored to the type of education the staff had. That we believe would be an advantage so that the expertise that is available in the ambulance is utilized.
