Abstract [sv]

Bakgrund: En intensivvårdsavdelning [IVA] avsedd för barn kan saknas på ett mindre sjukhus, vilket gör att många barn vårdas på allmän IVA tillsammans med vuxna patienter. År 2012 vårdades 2709 barn i Sverige mellan 0-18 år, på en allmän IVA. Det finns stora skillnader mellan att vårda barn och vuxna, sjuksköterskor som arbetar på en allmän IVA kan sakna de kunskaper som krävs för att vårda barn.

Syfte: Syftet med studien var att beskriva intensivvårdssjuksköterskors upplevelser av att vårda barn på en allmän IVA.

Metod: Fyra fokusgruppdiskussioner med totalt 13 deltagare genomfördes. Data analyserades med kvalitativ innehållsanalys och resulterade i åtta kategorier och ett tema. Studien genomfördes på ett mindre sjukhus i mellersta Sverige.

Resultat: De kategorier som framkom var; en känslomässig utmaning, behov av kontroll och behov av samarbete och stöd. Utifrån kategorierna framkom huvudtemat; Att vara utlämnad i en krävande situation. Sjuksköterskorna upplevde starka känslor i vården kring barn och det ansågs viktigt att vara väl förberedda då ett barn kommer till IVA, samt att ha ett fungerande samarbete.

Slutsatser: Att vårda barn på en allmän IVA kan vara en både utmanande och utsatt situation för sjuksköterskor. Sjuksköterskorna är i behov av ytterligare utbildning och möjlighet att askultera på andra avdelningar för att kunna lära sig mer om vård av barn.

Abstract [en]

Background: An intensive care unit [ICU] specialized in children might not be available in a smaller hospital, this causes many children to be admitted to a general ICU along with adult patients. In 2012, 2709 children in Sweden, between 0-18 years of age were admitted to a general ICU. There are major differences in caring for children compared to adults, intensive care nurses that work in general ICUs may lack the knowledge that’s required to care for children.

Aim: The aim of this study was to describe intensive care nurses experiences of caring for children in a general ICU.

Method: Four focus group discussions with a total of 13 participants were carried out, data was analysed using qualitative content analysis and eight categories and one theme emerged. The study was carried out in a smaller hospital in the middle of Sweden.

Results: The categories that emerged were; an emotional challenge, a need of control and a need of cooperation and support. From these categories a main theme emerged; being exposed in a demanding situation. The nurses experienced strong emotions when caring for children. They also described the importance of being well prepared when a child was going to be admitted to the ICU and it was also important with a functioning teamwork.

Conclusions: Caring for a child in a general ICU can be a challenging and exposed situation for the nurses. The nurses are in need of further education and the possibility to visit other wards to learn more about caring for children.
