Hur upplever instruktörer implementering av och följsamhet till Tidig Upptäckt och Behandling (TUB)?: en intervjustudie på Akademiska sjukhuset

Abstract [sv]

Syftet: Syftet med studien var att undersöka hur implementeringen av och följsamheten till en ny lokal riktlinje, Tidig Upptäckt och Behandling (TUB), upplevdes av TUB-instruktörerna på Akademiska sjukhuset.

Metod: Studien hade en kvalitativ, deskriptiv design och baserades på två fokusgruppsintervjuer. Båda intervjuerna var semistrukturerade och bandades. Bearbetningen av analysen utfördes enligt metoden för kvalitativ innehållsanalys.

Resultat: Resultatet av analysen resulterade i tre huvudkategorier. I domänen Implementering av TUB kunde Utbildning och Rutiner urskiljas och i domänen Följsamhet till TUB identifierades Upplevda effekter. Till varje huvudkategori identifierades hjälpare samt barriärer.

Slutsats: TUB-instruktörerna upplevde att det behövdes ett ledningsengagemang för implementering av TUB. Andra viktiga faktorer var att innehållet i TUB kunde anpassas till enskilda verksamheters förutsättningar och patientklientel, liksom att det bestod av lagom mängd information. I intervjuerna framkom att följsamheten till TUB upplevdes ha minskat antalet patienter med septisk chock inom en klinik. Vidare antyddes att personalen, inom flera kliniker, mer frekvent förmedlade information på ett strukturerat sätt och att det interprofessionella samarbetet hade ökat. Det som angavs ha varit försvårande för implementering och följsamhet omfattade bl.a. personal- och tidsbrister samt brister i datajournalsystemet.

Abstract [en]

Aim: The purpose of this study was to investigate how the implementation of and adherence to a new local guideline Early Detection and Treatment (TUB) were experienced by TUB instructors in the Uppsala County Council.

Method: The study had a qualitative, descriptive approach and was based on two focus group interviews. Both interviews were semi structured and taped. The processing of the assay was performed according to the method of content analysis.

Main results: The result of the analysis resulted in three main categories. For the domain Implementation of TUB could Education and Procedures be identified and for the domain Adherence to TUB were Perceived effects identified. For each broad category were distinguished helpers and barriers.  

Conclusion: TUB-instructors experienced the need for a management commitment to the implementation of TUB. Other important factors were that the contents of TUB could be adapted to individual operations´ conditions and clientele, as it consisted of just the right amount of information. The interviews revealed that adherence to TUB were thought to have reduced the numbers of patients with septic shock in a clinic. Furthermore, it was suggested that staff in several departments, more frequently conveyed information in a structured manner and that the inter-professional collaboration had increased. What was stating to have been aggravating for implementation and adherence included the staff and time constraints and weaknesses in the data log system.
