Abstract [sv]

Introduktion: Sepsis via centralvenös infart är en vårdrelaterad infektion med risk för ohälsa, förlorade livsår och död. Incidensen av CLABSI kan minimeras. Evidensbaserad vård består av forskningsevidens, klinisk expertkunskap och patientdelaktighet.

Syfte: Syftet var att undersöka sjuksköterskors erfarenheter av att följa aseptiska evidensbaserade rutiner vid hantering av centralvenös infart.

Metod: Studien var en kvalitativ intervjustudie där 6 sjuksköterskor intervjuades om erfarenheter av arbete med centralvenös infart. Tematisk innehållsanalys användes vid databearbetningen.

Resultat: Det var svårt för sjuksköterskan att bedöma om patientens sepsis orsakats via kontamination av den centralvenösa infarten eller från annan infektion. En delaktig patient sågs som en resurs men patienten kunde också vara en risk. Att vara påläst på senaste evidens var viktigt för att kunna utföra ett bra aseptiskt arbete. Det var en norm och självklarhet att desinfektera injektionsmembranet före användning. En engagerad ledning var viktigt för en god patientsäkerhetskultur. Sjuksköterskorna önskade återkoppling av kvalitet i antal CLABSI / 1000 dygn med centralvenös infart.

Slutsats: Hög följsamhet till evidensbaserade rutiner vid hantering av centralvenös infart var en självklarhet och norm. En välinformerad patient som har kapacitet att medverka kunde bidra till att minska riskerna för vårdrelaterad infektion. Sjuksköterskorna ville ha tydligare återkoppling i antal vårdrelaterade sepsis via centralvenös infart / 1000 dygn för den enhet de arbetade på.

Abstract [en]

Introduction: Central line associated sepsis is a healthcare acquired infection which has the potential of prolonged illness, disability, and death. Evidence-based practice is based on research evidence, clinical expertise, and patient values.

Aim: To investigate and describe nurses’ experiences of aseptic routines in central line care according to evidence-based practice.

Methods: Qualitative study. Six registered nurses were interviewed about their experiences of working with central lines. Thematic analysis was utilized processing the data.

Results: It was difficult for the nurse to determine whether the sepsis was caused due to contamination of the central line or by other source of infection. Patient involvement was seen as a resource, but the patient could sometimes be a risk. To be updated on latest research evidence was important for the nurse to be able to perform a good aseptic technique. It was a norm and matter of course to disinfect the connector hub before using it for injection or infusion. A committed management was important for good patient safe culture. The nurses wished for feedback of quality of CLABSI in numbers related to the denominator 1000.

Conclusion: Compliance to evidence-based practice and aseptic technique in central line care was a norm and matter of course. A well informed patient in partnership with the nurse was important to reduce risk for CLABSI, but some patients didn´t have capacity to participate in infection prevention measures. Quality reports in numbers that relates to research evidence could motivate good aseptic routines in central line care.
