Abstract [sv]

Bakgrund: Demografiska förändringar som förväntas ske de kommande decennierna innebär en kraftig ökning av antalet äldre personer i samhället. Demenssjukdomar och smärta är vanliga i dessa åldersgrupper vilket ställer vården inför en utmaning att förse dessa människor med lämplig vård. Afasi hos demenssjuka personer försvårar smärtbedömningsarbetet.

Syfte: Syftet med studien var att undersöka sjuksköterskors upplevelse av att bedöma smärta hos demenssjuka personer med afasi. 

Metod: En kvalitativ intervjustudie med induktiv ansats genomfördes med semistrukturerade intervjuer. 12 sjuksköterskor, som arbetar inom geriatrisk vård intervjuades. En av intervjuerna har exkluderats. Insamlad data analyserades med hjälp av manifest innehållsanalys.

Resultat: Tre kategorier identifierades med tio tillhörande subkategorier under dataanalysen: Samverkan, Smärtbedömning samt Kunskap och kompetens.

Konklusion: Denna studie ger en inblick i vilka redskap sjuksköterskor kan ha i smärtbedömningsarbetet på en geriatrisk akutvårdsavdelning. Användning av närstående, personal på äldreomsorg, teamet på avdelningen, Abbey Pain Scale, teoretisk kunskap och klinisk erfarenhet kan leda till en effektivare smärthantering hos demenssjuka personer. Mer utbildning, ökad kunskap om smärta, demenssjukdom och afasi samt ett mer specifik smärtbedömningsinstrument behövs för att kunna tillgodose dessa patienters komplexa vårdbehov.

Abstract [en]

Background: Demographic changes in the coming decades imply a substantial increase in the number of elderly in society. Dementia and pain are common in this groups, therefore health care system facing a challenge to provide this people with appropriate health care. Aphasia in dementia patients further complicate the pain assessment.

Aim: To describe the nurses’ experiences of pain assessment in dementia patients with aphasia.

Method: A qualitative interview study with an inductive approach was conducted. Data was collected through semi-structured interviews and analyzed through a manifest content analysis. Twelve nurses working in geriatric care were interviewed.

Results: Three categories were identified with ten associated sub categories during the data analysis: Collaboration, Pain Assessment, and also Knowledge and Competence.

Conclusion: The study provides insight into the tools nurses may use in the pain assessment at a geriatric emergency unit. Utilization of Abbey Pain Scale, nursing staff at nursing home residence, healthcare team at the unit, theoretical knowledge and clinical experience can lead to more effective pain management in people with dementia. More education, increased knowledge about pain, dementia and aphasia and a more specific pain assessment instrument are necessary to meet the complex care needs of these patients.
