Arbetsterapeuters erfarenheter av stödjande och hindrande faktorer i rehabiliteringen för personer med neglekt

The aim of this study was to describe occupational therapists experiences of resources and hindrances in the rehabilitation of persons with unilateral neglect. A qualitative approach was chosen based on the objective and data was collected by unstructured interviews with six occupational therapists working in stroke rehabilitation units in central Sweden. Data was analysed by qualitative content analysis, which resulted in two categories: "To increase awareness and reduce difficulties in occupation is a challenge" and "To choose between training and facilitating occupation in everyday-life is a balancing act". The results revealed that a decreased awareness of disability and problems in occupation of persons with unilateral neglect was challenging for occupational therapy practice. It was shown that occupational therapists pursue to increase awareness by means of activity and environmental adaptation. The results also emerged that verbal communication can be used as a means to increase awareness, as the person's communication skills proved to be rarely influenced by a right-sided injury. The results also showed that it appears to be a balancing act between maintaining and improving performance capacity through activity or compensation, and that safety must precede independence in occupation. Regarding the results, it can be concluded that clinical practice is challenged by a conflict that involves the request to enable participation in occupation and the need to limit occupational performance in order to provide safety. This study may indicate that further research would be of importance on how decisions on training or compensatory interventions could be facilitated by dynamic assessment. This study may contribute to increased knowledge of occupational therapists experiences in the rehabilitation of persons with unilateral neglect. Furthermore this study may also serve as a complement to former studies that investigated strategies and their effects on unilateral neglect.
