A COMPARATIVE STUDY OF WORKFLOW FOR PROCESSING OFSENTINEL LYMPH NODES SAMPLES BEFORE AND AFTERPARTICIPATION IN THE SENOMAC TRIAL

Abstract [en]

Sentinel lymph node (SLN) status is an important prognostic factor in breast cancer. SLN isdefined as the first lymph node that receives lymphatic drainage from a primary breast tumor.In 2015, Gävle hospital participated in SENOMAC trial, which aims to evaluate whether it issafe to refrain from axillary lymph node dissection in breast cancer with macrometastases inthe SLN. The main purpose of this study was to map out workflow for processing of SLNsamples before and after participation in the SENOMAC trial. A second purpose was to findout how much work was performed by laboratory staff in the preparation of SLN samples. Forthe first purpose, data was collected from database for years 2014-2016 and then divided intotwo periods the first period before participation in the SENOMAC trial and the second periodwas after participation. Furthermore, a time study was performed using a fresh SLN case andformaldehyde-fixed SLN cases. The results show that the number of formaldehyde-fixed SLNsamples increased significantly after participation in the SENOMAC trial as compared tofresh SLN samples that were reduced. In addition, it more time to prepare formaldehyde-fixedsamples in comparison with the fresh SLN sample. In conclusion, this study showed that morework was performed by laboratory staff after participation in the SENOMAC trial withformaldehyde-fixed, paraffin-embedded SLN samples, than with fresh SLN samples that wereexamined by frozen section.
